Could it be that your eyelids are not actually sagging?
When many people say “my eyes are sagging,”
they simply think the eyelid skin has stretched.
So they assume that removing only the loose skin
will make things feel much better.
But we need to look at the structure around the eyes
in a more three-dimensional way.
Above the eyelids are the eyebrows,
and above them is the forehead.
As we age, the forehead and eyebrows
gradually descend due to decreased skin elasticity
and the effects of gravity.
The problem is that the way we open our eyes also changes at this time.
Normally, when we open our eyes, the muscle that lifts the eyelid
mainly does the work.
But when the forehead and eyebrows have dropped heavily,
the eyelid-lifting muscle alone is not enough.

So the body unconsciously brings in the forehead muscles as well,
pulling up the eyebrows to open the eyes.
As this habit repeats, forehead wrinkles deepen,
and you feel tired every time you open your eyes.
In other words, the eyelid skin itself may not be the problem,
but the heavy forehead and eyebrows above it press downward,
causing the eyes to become covered.
If you remove only the covered eyelid skin without addressing the cause,

So how can you tell whether the problem is your eyes
or your forehead?
There is a way to check accurately at home
before coming to the clinic.
All you need is a mirror and your palm.
3-second self-check

Step 1. Stand in front of a mirror
and gently close your eyes.

Step 2. Open your palm and place it on your forehead
above your eyebrows.
→ The key is to press so that the forehead muscles cannot move.

Step 3. Keep your forehead pressed down
and slowly open your eyes.

The eyebrow position you see in this state
is the actual eyebrow position.
When we open our eyes, we unconsciously use the forehead muscles,
so our usual eyebrows may be positioned higher than they really are.
Now we can divide the results into two cases.

- If the eyebrows are below the brow bone?
→ This is when you should consider a brow lift.
If your eyebrows droop downward when you open your eyes
and you feel like your eyes are being pressed down,
this is not only a problem with the eyelid skin.
It means the entire forehead has descended,
causing the eyebrows to sag as well.
In this case, a brow lift is needed.
A brow lift is a surgery that raises and fixes the sagging forehead and eyebrows
back to their original position.
It pulls the forehead skin and tissue upward,
then secures them to strong bone or the fascia over the muscle.
This raises the eyebrows that were covering the eyes,
removes the pressure on the eyelids,
and allows you to feel that your eyes open much more clearly.
At the same time, you can also expect improvement in forehead wrinkles.
However, the height to which the eyebrows are lifted should not be
too high or too low.
If they are lifted too high, you may look as if you were startled,
and if they are too low, your eyes may still look heavy.
That is why an experienced medical team considers the patient’s facial proportions,
the position of the brow bone,
and habits of forehead muscle use in a comprehensive way
to decide the appropriate height.
Only then can the result look natural and satisfying.

- If the eyebrow position stays the same,
but only the eyelid skin is wrinkled?
→ Upper blepharoplasty is appropriate.
On the other hand, in some cases the forehead and eyebrow position
are already well fixed above the bone,
but only the eyelid skin has become thin and loose,
blocking the field of vision.
For these patients,
upper blepharoplasty, which trims loose skin,
can be enough to create a clean eye shape.
Upper blepharoplasty removes the loose skin along the double-eyelid line
and then sutures it closed.
The important point here is “how much should be removed?”
If too much skin is removed,
the eyes may not fully close when you shut them.
If too little is removed,
the feeling of heaviness may still remain.
That is why precise design before surgery is necessary.
An experienced medical team carefully measures how much extra skin the patient has
and where the double-eyelid line will look natural.
If muscle strength is weak,
simply removing skin may not make the eyes open clearly.
