I surprisingly hear this question very often.
“Between under-eye fat repositioning
and dual lower blepharoplasty,
which lasts longer?”
“The opinions in online reviews are all different,
so I don’t know what to trust.”
If you searched for this article and came here,
you may have similar doubts.
No matter how many dual lower blepharoplasty reviews you look up,
it only seems like promotional content,
so it’s hard to feel sure whether it really works.
That’s why today, I’ve brought a somewhat special story
that may help break that prejudice.

This is also the story of how I, who used to operate on patients’ under-eyes every day,
lay down on the operating table myself this time
and became a patient.
I’ll tell you honestly what concern I wanted to solve,
and among so many methods,
why I chose “dual lower blepharoplasty.”
Why did the doctor who knows dual lower blepharoplasty best
choose to have it done personally?

“Do you look tired?” “Did you not sleep last night?”
You’ve probably heard this often from people around you, right?
I was no different.

Every time I looked in the mirror, the fat protruding under my eyes
was the first thing I noticed, and even when I hid it with glasses,
the dull, tired look didn’t go away.
More than anything, as I spoke to patients about
the improvement that surgery could bring,
I also felt a growing desire to properly address this concern myself.

I judged that actually experiencing it myself
would allow me to understand the anxiety and expectations
that patients feel much more accurately.
And based on that experience,
I believed that the words I could share,
would become a much more realistic standard than online reviews
and could be helpful to you.
Why was it “dual lower blepharoplasty”
instead of “under-eye fat repositioning”?
Because of concerns about scarring,
many people first consider transconjunctival repositioning.
I understand very well that such a choice is perfectly natural.

But in my case, dual lower blepharoplasty, which includes skin excision,
was the more suitable choice than simple repositioning.
The reason is simple.
The first thing I checked after deciding on surgery
was the elasticity of the skin under my eyes.

When I checked, I found that after gently pulling the skin and letting go,
it returned to its original position noticeably slowly.
In this state, if only the fat is adjusted
or only repositioning is performed,
the protruding puffiness may be reduced,
but the skin that has lost elasticity may not be able to keep up with that change.

Then the remaining tissue may fold like wrinkles,
or fine lines may appear more prominent.
In the end, the fat may be addressed,
but the skin elasticity problem remains,
which can make the result disappointing.
This is also why dual lower blepharoplasty is fundamentally different
from conventional under-eye fat repositioning.
Dual lower blepharoplasty is not a surgery that only moves fat;
it combines fat repositioning with skin excision,
making it a “structural correction” procedure that addresses
under-eye contour and sagging at the same time.
Dual lower blepharoplasty is generally performed in three major steps.
First, the surgeon approaches through the conjunctiva (the inside of the eye)
and moves the protruding fat to the sunken tear trough area
to smooth the under-eye contour.
At this stage, the loosened orbital septum is also tightened and fixed
to help prevent recurrence.
Next, the excess skin just below the eyelashes is removed in a very minimal amount
to prevent wrinkles or folds from remaining.
This step is necessary so that even after the fat is smoothed out,
the under-eye area can follow naturally.
The final step is fine suturing and tightening.
While gently pulling the skin upward and suturing it,
the under-eye area is shaped to maintain a smooth tension,
and the area is refined so the scar is as inconspicuous as possible.
You can think of this finishing detail as the step
that determines the completeness of the surgical result.
There may be concerns about scarring,
but in my case, I judged that minimizing that possibility
while also addressing the lax skin
would lead to a more stable long-term result.
How can you check in 10 seconds whether you are a candidate
for dual lower blepharoplasty?
Dual lower blepharoplasty is not necessary for everyone.
For people with firm skin elasticity,
fat repositioning alone may be enough
to achieve a satisfying result.
However, if even one of the features below applies to you,
I recommend considering dual lower blepharoplasty
rather than insisting on repositioning alone.

People whose under-eye fat is bulging
while fine wrinkles on the skin are also noticeable
People whose outer eye corner area is sagging
and who feel as if there is leftover skin
Also, this is a reliable self-check method
that I personally tried.
It’s a very simple way to check in front of a mirror.

- Gently pull the skin under your eyes downward.

- When you let go, check whether the skin returns to its place immediately with a “pop.”

If the skin you pulled returns slowly
or with a wrinkled appearance,
you may suspect that its elasticity has decreased
compared to before.
In that case, under-eye fat repositioning alone may make it difficult
to solve the fine-wrinkle problem,
and dual lower blepharoplasty, which removes excess skin,
may be more suitable.
However, since the diagnostic method I mentioned is only a simple reference,
please make the final surgical decision through a doctor’s examination
and a detailed evaluation.

the difference between under-eye fat repositioning and dual lower blepharoplasty,
and also the diagnostic method that can help you determine
which surgery is right for you.
The fact that I underwent dual lower blepharoplasty myself
