Lower Eyelid Fat Repositioning and Fat Grafting
Would it be better to get them together?
Dr. Shin Jong-in Answers!

Hello.
This is Obje Plastic Surgery.
When you are worried about under-eye puffiness
and start looking into surgery,
at some point another concern comes up.
Should you only get lower eyelid fat repositioning,
or should you also
get lower eyelid fat grafting?
If you search online,
there is a lot of information
about lower eyelid fat grafting.
Some places say it is absolutely necessary,
while some reviews say
you should never do it.
Honestly, it probably just made
you even more confused.
This is especially true for people with a slim build,
or for those whose under-eyes look puffy
but also sunken at the same time.
Today, I will explain step by step
when you should consider
lower eyelid fat repositioning
together with lower eyelid fat grafting,
and why that method may be needed.
If you read until the end,
it will help you determine
which method is right for you.
If the front cheekbone is flat?
Fixing only the under-eye area
can make it look even more hollow
When you go in for a consultation about lower eyelid fat repositioning,
you often hear that the "front cheekbone is sunken."
Why is the front cheekbone
so important?
First, think of the typical East Asian face shape.
The side cheekbones are slightly prominent,
and the front cheekbones are relatively flat.
Because of this structure,
East Asian faces often appear
more flat overall.
But if you only reposition
the puffy area under the eyes
in this situation,
what happens?
If the volume of the front cheekbone
stays the same while only the under-eye area
becomes flatter,
the entire midface can actually look
even flatter and more sunken.
To help you understand,
let me give you a simple example.
If there are objects stacked on a desk,
and you only remove the middle part,
the sides will stand out more.
The face is the same.
If you only tidy up the under-eye area,
the front cheekbone around it can appear
relatively more sunken.
So what is the solution?
While doing lower eyelid fat repositioning,
fat is grafted to the front cheekbone
to slightly restore volume.
This creates a natural S-shaped line
from the under-eye area to the cheekbone,
which can give a much livelier
and younger-looking effect.
Many people do not even realize
that their front cheekbone is hollow.
They may only be focused on the under-eye area,
but in reality, the entire midface
may lack contour.
For these people,
getting lower eyelid fat repositioning alone
can certainly improve things,
but it may still leave something to be desired.

"It definitely looks better,
but somehow my face doesn't
look brighter than I expected?"
That kind of feeling may come up.
So if you also consider the condition
of the front cheekbone and supplement the volume,
you can expect much more satisfying results.
If it is both puffy and sunken?
Repositioning alone has its limits
The second situation to consider is
when the under-eye area is puffy
but also has hollow areas at the same time.
What does that mean?
It refers to a complex condition where part of the under-eye area protrudes,
but the boundary of the orbital bone just below it
is deeply sunken.
When you look in the mirror,
the under-eye area appears puffy,
but at the same time,
the area below the eye looks
hollowed out and shadowed.
In such cases,
simply repositioning the protruding part
does not fully solve the problem.
Why not?
Repositioning literally means
"moving the position."

It is a method of moving the bulging area
down into the hollow area below
to make it look flatter.
However, if the boundary of the orbital bone
is significantly sunken,
the amount of fat used for repositioning alone
may not be enough to fully fill that depression.
In addition, if too much fat is moved downward,
the balance of the entire under-eye area
can become disrupted.
So what should people in this situation do?
The protruding area is addressed
with lower eyelid fat repositioning,
and the deeply sunken area at the orbital bone boundary
is treated separately with fat grafting
to soften the contour.

What matters here is
how the fat is grafted.
The traditional microfat grafting method,
when applied to such a delicate area as the under-eye region,
can sometimes make the surface look uneven
depending on the injection site and depth.
By contrast, single fat cell grafting
separates the fat to a level close to individual cells
and delicately grafts it into a thin layer
just beneath the skin,
so it can produce a much more natural
and smoother result.
In other words, the bulging area is corrected with repositioning,
while the sunken area is addressed
with single fat cell grafting.
This allows the complex under-eye concerns
to be improved all at once,
and can ultimately lead to a much more satisfying surgery.
Should you always do it, or never do it?
Neither is the correct answer
During consultations for lower eyelid fat repositioning,
some people firmly say,
"I will never get lower eyelid fat grafting."
They may have seen a lot of negative comments
about lower eyelid fat grafting
in online reviews or from people around them.
Of course, those concerns are understandable.
Improper lower eyelid fat grafting
can lead to lumpy or unnatural results.

But on the other hand,
it is also not correct to think that
lower eyelid fat grafting must always be done.
What matters is whether you need it.
As explained earlier,
if the front cheekbone is hollow,
or if the under-eye area is puffy
while also having sunken areas,
better results can be achieved
when lower eyelid fat grafting is performed together.

However,
there are also many people for whom that is not the case.
If the issue is simply protruding under-eye fat,
then repositioning alone can still produce
satisfying results.
So rather than going in with a fixed conclusion
before your consultation,
it would be better to approach it like this.
"Let me hear why my face shape
may need lower eyelid fat grafting."
If you go into the consultation
with an open mind like that,
it will be much easier to determine
what method you truly need.
Today, we looked at cases where
lower eyelid fat repositioning and lower eyelid fat grafting
should be considered together.
The most important thing is
to accurately understand the condition of your face.
You should make your judgment based on
whether this surgery is needed for you.
