and an unfamiliar procedure name, upper blepharoplasty, also comes up.
Today, I’d like to briefly explain why double-eyelid surgery for people in their 20s and midlife eyelid surgery after the 40s need to be approached differently.
If you read to the end,
you’ll get a clearer sense of why upper blepharoplasty is not just a simple double-eyelid surgery,
and from what perspective your own eyes should be evaluated.
The eyes in your 40s and 60s start from different “baseline conditions” than eyes in your 20s

Eyes after the 40s and eyes in your 20s differ from the very start in the conditions under which surgery begins.
In your 20s, the eyelid skin is generally supported by good elasticity,
and the fat is also in its proper place.
So for double-eyelid surgery at this age,
the main focus is deciding what kind of line shape to create.
In contrast, eyes after the 40s show two types of changes.
First,
the eyelid skin begins to sag downward.
As the elastic fibers that used to keep the skin firm lose strength compared with before,
the skin gradually drops downward.

As a result, the sagging skin covers the double-eyelid line,
and the eyelid often gives the impression of having collapsed over the eyelashes.
Second,
the fat either becomes hollow or shifts position.
The fat in the upper eyelid gradually loses volume over time
and becomes sunken inward.

By contrast, under-eye fat becomes more prominent as the membrane that used to hold it in place weakens,
causing it to push forward and bulge.
This is why the same face can look sunken around the eyes while also appearing tired.
Because these two changes progress together,
midlife eyelid surgery is less like creating a new line
and more like a procedure that “reorganizes the structure above the eyes.”
At the center of that is upper blepharoplasty.
Upper blepharoplasty vs. regular double-eyelid surgery: what’s actually different?
Upper blepharoplasty is one type of double-eyelid surgery for middle-aged patients.
It is a procedure that removes and organizes the sagging skin of the upper eyelid through an incision,
and, if necessary, also addresses the drooping muscle inside it or fat that has moved out of place.
Because it can be seen as a surgery that “just removes a little skin,”
it is easy to think of it as a simple procedure,
but in reality it requires quite a refined plan.

That’s because the amount of skin laxity,
the direction in which the fat has shifted,
and the strength of the muscles used to open the eyes are all different from person to person.
There are three major factors that should be checked in an upper blepharoplasty consultation.
First, we look at whether you use your forehead muscles when opening your eyes.
If you tend to use your forehead a lot, there is a possibility that the eyelid muscles have weakened,
so simply removing the skin with upper blepharoplasty may not fully restore a clear, refreshed impression.
In such cases, eye-opening correction or a brow lift may also be considered.

Next, we check the thickness of the eyelid and whether the eyes are more protruding or more sunken.
Even with the same upper blepharoplasty,
where the incision line is placed
and how much skin should be removed will differ.
If this analysis is skipped and only the procedure name is decided first,
there is a greater chance the result will be far from what you wanted.

That is why, in midlife eyelid surgery,
what matters much more than “which surgery to get” is
“how to design it to fit my eyes.”
