Removing Under-Eye Hollows?
Why Director Park Si-hyeon recommends under-eye fat repositioning

Hello.
This is Obje Plastic Surgery.
When you look in the mirror,
your under-eyes may appear dark and sunken,
or your face in photos may seem
especially tired.
Even after getting enough sleep,
the shadow like a dark circle does not go away,
and even if you try to cover it with makeup,
it does not look naturally even, so
in frustration,
you may have searched for ways to remove under-eye hollows
and ended up reading this post.
The surgery we can recommend to you is
“under-eye fat repositioning,”
also called under-eye fat repositioning surgery.

Today, we will explain why
under-eye fat repositioning is recommended
for concerns about under-eye hollows,
and how to check whether
you really need this surgery.
We hope you can spend just 3 minutes
and get the answer to your concern.
Why do under-eyes look dark?
The identity of the “under-eye hollow”
When the under-eye area looks dark,
people often assume the dark circles have become deeper
or the skin has become thinner.
However, when we examine patients’ under-eyes,
we often find that the cause is not just pigmentation,
but rather a difference in height
within the under-eye structure itself.
Beneath the eyes, there is originally
a small pad of fat.

It acts like a cushion
that protects the eyes.
And a thin membrane called the orbital septum
holds it in place so it does not shift forward.
The problem is that as time passes,
this membrane gradually loosens.
When the membrane becomes lax, the fat inside
slowly pushes forward,
and the area where it has moved forward becomes bulged,
while the area just below it
looks relatively sunken.

As the height difference becomes more pronounced,
the areas hit by light and the areas in shadow
become clearly divided.
The shadow line created this way
is the under-eye hollow.
In other words, it is not that the area looks dark because of color,
but that the shape itself creates the dark appearance.
This is where many people get confused.
They try to fill only the sunken area,
or they try to only smooth out the bulging area.
If you simply fill in a situation where one side protrudes and the other side is sunken,
the upper part becomes even more prominent,
and if you only address the bulging side,
the lower part becomes even more deeply sunken.
That is why the approach we recommend
is to “move” the fat.

Moving the fat that has shifted forward
into the hollow area right below it
to create a smooth under-eye line,
that is the core of under-eye fat repositioning surgery,
and the reason we recommend it first
for people troubled by under-eye hollows.
Under-eye fat repositioning scars? Recurrence?
Don’t worry!
Now let us explain
how under-eye fat repositioning is performed.
The part most people are curious about
is the scar.
At Obje, we perform the procedure by making an incision
inside the conjunctiva of the eyelid
and approaching through that inner route.
Because there is no incision on the outer skin,
a major feature is that no trace remains
on the surface of the face after recovery.

After entering through the conjunctiva,
the forward-shifted fat is carefully separated
and arranged so it can be moved,
then relocated into the hollow area below
and positioned there.
That is the general flow.
What Obje pays extra attention to
is “double fixation.”
As mentioned earlier,
the orbital septum that held the fat in place
so it would not move forward
gradually loosens over time.

If only the fat is moved while leaving this membrane as it is,
there remains a possibility that the inner fat may be pushed forward again
as time passes.
That is why Obje secures two layers together.
The first is the orbital septum,
which had been wrapping and holding the fat.
This step restores its role as a barrier
that supported the fat by firmly re-suturing the loosened area
so the fat does not push forward.

The second is the periosteum,
a firm membrane beneath that layer
that surrounds the bone under the eye.
The periosteum is not a structure that moves like a muscle;
it stays almost completely stable in one place,
so fat fixed deeply to it is less likely to become displaced.
The membrane that had wrapped and held the fat,
and the firm membrane surrounding the bone,
by securing both layers at the same time,
even as time passes, the fat is less likely
to be pushed forward again,
and this plays a major role in helping the refined line last.

Because the surgery is performed through a conjunctival incision,
you do not need to return to have stitches removed,
and while swelling and bruising vary from person to person,
in about 1 to 2 weeks,
it is generally less burdensome to return to daily life.
Is under-eye fat repositioning right for me?
<Self-check checklist>
If you have read this far,
you may want to judge whether your under-eye area is at the stage
where this surgery is needed.
Please check the criteria below.
✓ Your under-eyes bulge out noticeably,
and people often tell you that you look tired
✓ The under-eye hollow has become deeper,
and the shadow is clearly visible
✓ Even after getting enough sleep,
your dark circles do not lighten
✓ Even when you tidy it with makeup,
it does not naturally conceal it
✓ You have recently been hearing more often
that you look gloomier than people your age
✓ Your reflection in the mirror looks much more exhausted
than your actual condition
If even one of these applies,
it is a point worth seriously considering under-eye fat repositioning.
That is because there is a possibility that fat has already shifted forward
within the inner under-eye structure,
creating a height difference.
In such cases, cosmetics or massage
have limitations in producing the change you want.

Methods that only tidy the surface
can temporarily refine the skin texture or color,
but they cannot make the shape itself flat.
However, even if something looks like the same kind of shadow,
the surgical direction can differ depending on
fat position, skin thickness, and eyelid elasticity,
so rather than deciding based only on self-checking,
we recommend going through an in-person examination
of your under-eye condition.
Today, we looked at why the under-eye area
appears dark and deep,
and why under-eye fat repositioning surgery
is recommended as the solution.
Director Park Si-hyeon of Obje Plastic Surgery does not simply remove fat,
but instead moves the fat that has shifted forward
into the hollow area and fills it there,
thereby addressing both the bulging upper part
and the deeply sunken lower part at the same time,
so the shadow like a dark circle under the eyes becomes lighter.

In addition, to help prevent the relocated fat
from being pushed forward again over time,
it is secured with a double-fixation method,
helping the refined line last longer.
Because the surgery is performed through a conjunctival incision,
no trace remains on the surface of the face after recovery,
and in addition, we carefully guide patients
through swelling and bruising management as well.
We hope your careful research leads to a result you are happy with.
This has been Obje Plastic Surgery.
Thank you.
