A granuloma refers to a mass formed when the surgical site fails to heal properly,

causing abnormal tissue to clump together along with an inflammatory response.

Many people describe the pain of a granuloma

as feeling as if “sand is rolling around.”

Granulomas can develop anywhere on the skin,

but after under-eye fat repositioning surgery,

they most commonly develop inside the conjunctiva.

This type of under-eye fat repositioning side effect mainly occurs during the final stage of surgery,

namely, the process of suturing the conjunctiva.

If the two sides of the incised mucous membrane are not aligned precisely,

or if they are sutured while a tiny gap remains,

excessive tissue may grow through the gap and form a granuloma.

Afterward, a foreign-body sensation may be felt inside the eye,

accompanied by pain that feels as if sand is rolling around

every time the eyes are closed and opened.

In some cases, it may also appear uneven on the outside,

causing an aesthetic concern.

Because granulomas rarely disappear on their own,

most must be removed through surgery.

The important point is not simply to remove the granuloma,

but also to resuture the conjunctiva precisely so that it does not recur.

Entropion refers to a condition in which the lower eyelid rolls inward,

causing the eyelashes to continuously irritate the eyeball.

Each time the eyes blink, the eyelashes scratch the cornea,

causing severe pain and a foreign-body sensation,

as well as continuous tearing.

In severe cases, it can lead to corneal damage and even reduced vision,

making it a relatively serious side effect.

Why, then, does entropion occur

as a side effect of under-eye fat repositioning?

During the recovery process after surgery,

firm scar tissue contracts,

causing the eyelid to roll inward.

To prevent this, during under-eye fat repositioning surgery,

it is necessary to maintain a safe distance of at least 4–6 mm

from the tarsal plate, the supporting structure of the eyelid,

when making the conjunctival incision.

How can entropion be treated if it develops?

Treatment for entropion is carried out in stages.

Initially, steroid injections may be administered

to gently loosen the scar tissue,

and there are also quite a few cases in which the condition improves at this stage.

However, if the symptoms persist, surgical treatment is ultimately required

to remove the scar tissue and relieve the tension.

Through this treatment, the inward-rolled eyelid

can return to its normal, original position.
