Causes of Vascular Dilation, a Case of Skin Flare-Up and Sensitivity After Facial Paralysis, and Excel V

Hello, in the previous post I introduced Excel V. Recovery also took quite a long time afterward, and even now they still experience discomfort such as very sensitive skin and frequent redness.

The steroids usually prescribed for facial paralysis are from the prednisolone class, and for about the first week, a very high dose is taken. The skin problems that occur at this time are due to systemic hormonal disruption. High-dose steroids directly stimulate the sebaceous glands, causing sebum production to surge dramatically in a short period of time. My acquaintance described it as “turning into a sea squirt,” which was because the suddenly increased sebum clogged pores, causing both comedones (small bumps) and pustular inflammation to appear across the face at the same time. In addition, because steroids suppress strong, spreading inflammation, they also lower immunity. As a result, it becomes an environment where bacteria and Demodex mites in the skin pores can multiply easily, so even skin that usually has no trouble can flare up in an instant. Of course, since this is a problem caused by medication, symptoms gradually improve once the medication is stopped, but the scars and marks left behind during that process can be severe enough to require laser treatment (such as Excel V), so this person also had to receive follow-up treatment like that.

Then what about topical (applied) steroids?

Compared with oral medication, topical medication has fewer systemic side effects, but it is more frightening in that it fundamentally destroys the skin structure of the treated area. If steroid ointment is applied for a long time, collagen production in the dermis stops, the skin becomes paper-thin, stretch marks (striae) may appear, and telangiectasia, with blood vessels clearly visible, can remain permanently. If you look at elderly people who have applied medication for a long time because of knee pain, you can see that the skin on the knees has become very thin. The same symptoms appear on the face as well. In addition, the skin loses its ability to regulate blood vessel activity, so even a slight change in temperature can make the face burn red and not calm down easily. This is much more difficult and takes far longer to treat than oral medication.

In conclusion, oral high-dose steroids taken for a short period to treat facial paralysis, like in my acquaintance’s case, can cause a very shocking and distressing skin flare-up right away, but if the medication is stopped and appropriate care is provided, it can be reversed. On the other hand, topical steroids bought at a pharmacy and applied to the face for months without much thought can damage the skin’s “foundation” (the dermis) in an irreversible way, so in the long term they can be considered more dangerous.

Actually, taking steroids for facial paralysis treatment should never be stopped on your own, because nerve recovery is the priority. Skin flare-ups are painful, but preventing nerve damage comes first. Instead, if you manage the dilated blood vessels and inflammatory marks with a laser such as Excel V during or right after taking the medication, and combine that with skin booster procedures that help strengthen the skin’s immunity, you can restore the damage left by steroids much faster.
