Melasma became darker after laser treatment?
The golden window for preventing the “rebound phenomenon”
Natural Beauty, Bellevue Clinic
Hello, this is Bellevue Clinic, where we study the fundamental roots of skin pigmentation. We understand that some people have felt flustered after starting laser toning with high hopes for pigmentation treatment, only to find that their blemishes appeared darker or that melasma they did not previously notice seemed to emerge after several treatments. Or perhaps you have been postponing treatment after hearing people around you say, “If you get laser treatment incorrectly, your entire face will flare up.”

Medically, this phenomenon is called the “rebound phenomenon” or “post-inflammatory hyperpigmentation (PIH).” Today, we will explain why this phenomenon occurs and reveal how Bellevue Clinic gets through this challenging stage to achieve truly clear skin.

- Why can pigmentation become darker after laser treatment?
Laser treatment inevitably involves delivering “energy (heat)” to the skin as it destroys pigmentation. Heat can be beneficial or harmful. There is nothing inherently wrong with it. We believe that the value of dermatological lasers (heat) lies in the physician’s ability to regulate their intensity and timing appropriately, as well as in the patient’s capacity to recover. However, melanocytes, the skin’s pigment-producing cells, are much more sensitive and reactive than one might expect.
▶The cell’s defense mechanism
When laser energy is excessive or strong stimulation is applied while the skin is sensitive, melanocytes perceive it as an “attack.” To protect themselves, they produce an explosive amount of melanin—more than usual—which can make the pigmentation appear darker.
▶Waking dormant melasma
Melasma is often closely intertwined with blood vessels. If pigmentation is treated aggressively without also managing the blood vessels, the heat stimulation can dilate the vessels, which in turn stimulates the pigment cells and creates a vicious cycle.

- The key to preventing rebound is “low-output energy and aftercare”
We do not believe that using the strongest possible laser is always the answer. The success of pigmentation treatment depends on carefully adjusting the intensity so that only the pigmentation is gently targeted without irritating the skin. At the beginning, Bellevue does not try to eliminate pigmentation all at once. Using high-performance Revlite or Pico lasers, it may be preferable to use a low-output approach that breaks the pigment particles into smaller pieces without stimulating the melanocytes. In addition, the 15 minutes immediately after laser treatment are, in fact, a time that can determine the skin’s outcome. Immediately after treatment, Bellevue quickly neutralizes the residual heat before it can stimulate the pigment cells through immediate Cryo cooling care and the infusion of a soothing ampoule. Toning that includes aftercare involves somewhat less concern about rebound.

- Why does Bellevue combine vascular treatment (Excel V)?
As emphasized in an earlier post, one reason pigmentation keeps becoming darker is the presence of “dilated blood vessels” hidden beneath it. Blood vessels are like pipelines supplying nutrients to pigment cells. This is where the Excel V laser comes into play! By addressing abnormal blood vessels in advance with Excel V, the pigment cells cannot obtain the energy they need to become active again after laser treatment. Once the blood vessels are organized and the complexion looks clearer, the effects of laser toning become visibly apparent much faster. This is the medical basis for the “Black and Red” package proposed by Bellevue.

“A physician’s skill comes from adjustment.”
Many people think that something went wrong when their pigmentation becomes darker after laser treatment, and they consider changing clinics. However, this may be the most important “turning point in treatment.” When a physician detects signs that the skin is reacting sensitively, immediately lowering the energy and focusing on soothing care to calm the skin is what demonstrates true clinical skill: flexible management.
Bellevue Clinic carefully checks each patient’s skin condition before every treatment. The laser parameters are adjusted in increments of 0.1 depending on factors such as whether the patient slept poorly the night before, is in their menstrual cycle, or has recently spent time outdoors. These details help create safe treatment without rebound.

We recommend Bellevue’s
pigmentation-care program for people like these!
★People who have previously received laser toning but experienced no effect or found that their pigmentation became darker
★People who have both melasma and facial flushing and are unsure which treatment to receive first
★People with thin, sensitive skin who are concerned about laser stimulation
★People who want premium care that includes thorough soothing management after treatment
Laser pigmentation treatment is a kind of “push and pull” with the skin. Together with Bellevue Clinic, we will help you experience genuine brightening that becomes clearer from within, without worrying about rebound. Are you wondering whether your pigmentation is currently ready for treatment or whether your skin needs a period of rest to be soothed first? When you visit Bellevue, we will check the inflammation levels and melanin activity within your skin through Mark-Vu polarized-light analysis. Thank you.

“Focused on specialized skin analysis”
Bellevue Clinic
The Mark-Vu skin analysis test can identify various skin concerns at once, including skin texture, pigmentation, pores, wrinkles, skin tone, dark circles, radiance, facial flushing, melanin, sebum, and acne-causing bacteria, which are difficult to assess with the naked eye.
Based on these results, Bellevue scientifically analyzes your skin and makes a final determination of the necessary treatments when appropriate. Through detailed consultations, we will strive to provide the treatments that are most effective for each patient.
