When we talk about nose surgery, many people ask what the most important thing to consider first is.
The most important thing is to accurately understand why you are considering nose surgery in the first place.

Some people may have trouble sleeping at night because their nose is blocked, or may be considering rhinoplasty for functional reasons and think, “It would be nice if I could add aesthetic beauty at the same time.” Others may not have major functional issues but may be dissatisfied with the appearance of their nose, or may simply have a preferred nose shape.
The ideal nose shape can also differ between men and women.
For women, popular rhinoplasty lines include the “swan-like nose,” the “half swan-like nose,” and the recently common “straight half swan-like nose.”

Unlike women, men are not well suited to a curved look.
That is why a straight nose is preferred, and when consulting with male patients, I usually recommend a straight nose line.
When the tip of the nose and the glabella are connected in a straight line, and the middle part of the nasal bridge dips by about 3 mm, this is called a “swan-like nose.”
Because it can look quite artificial, it is not performed as often these days.
A “half swan-like nose” refers to about 2 mm, a “straight half swan-like nose” to about 1.5–1 mm, and a “straight nose” literally means a straight line.
In the past, the “half swan-like nose” was very popular, but these days there is a trend toward something more natural, so the “straight half swan-like nose” is more preferred.
People often say a nose shape that is popular at the moment is a nose that suits their face, but that can vary depending on the face.
For example, for people with longer faces, a “straight nose” can make the nose look even longer and may contribute to an aged appearance.
Sometimes people with this kind of long face ask to have the “columella” removed,
but that can also make the nose look longer and may not suit their face.
On the other hand, some people have very small and low noses.
For those people, a “straight half swan-like” nose or something close to a straight nose is better than a “half swan-like” or “swan-like” nose.
Raising the middle part of the nasal bridge creates the effect of making the nose look higher.
People with small and low noses may have the advantage of looking younger, but they may lack a feminine, sophisticated impression.
In that case, if the bridge is raised sufficiently and made closer to a straight line, the face can look more refined.

Next, there is also the relationship between the tip of the nose and the lips.
This is called the nasolabial angle. (The angle formed by the columella and the philtrum)

For men, the angle is usually around 90 to 95 degrees, or even around 100 degrees, making it almost straight. When performing surgery, men generally do not prefer their nostrils to be visible from the front,
so the procedure is performed in a direction that keeps the nostrils from being seen from the front.

For women, it is better if the angle formed by the columella and the nostrils has a slight visibility of the nostrils, like the shape of a flying seagull.
A slightly upturned nose can make a woman look much more feminine, lively, and sophisticated.
Some people have a low nose but want it raised a lot.
However, if the forehead is flat, the nasal bridge cannot be raised very much.
Simply put, the nasal bridge cannot be made higher than the forehead.
Even if the height of the forehead and the nasal bridge become the same, it will look awkward. In such cases, to raise the bridge a bit,
you first need to increase the forehead height through fat grafting, fillers, or implants, and then raise the nasal bridge so the proportions are balanced.

Next, there are people with a protruding mouth.
In such cases, if the nasolabial angle is about 105 to 110 degrees, the nose becomes very upturned, and tip correction is performed.
Even if the angle is brought closer to the lips, around 80 to 85 degrees, it is more natural if the nostrils are as invisible as possible from the front.
Also, during nose surgery, if the columella is pulled slightly forward, the impression of a protruding mouth is greatly reduced.
Simply put, you can see an effect similar to having double-jaw surgery.
What types of noses require osteotomy?
There are mainly three reasons to perform osteotomy.

Hump nose: when there is a hump and osteotomy is needed to shave down the hump area

Crooked nose: when the bone part of the nose itself is bent, and osteotomy is needed to align it with the center line

Wide nose: even when the nose looks wide and thick from the front, osteotomy is needed to refine the line so it looks slimmer and more sophisticated.
Is there a nose shape that suits your face shape?
The shape of the nose can also vary depending on the face shape.
Usually, when looking at the starting point of the nose, it is best for the nasal bridge to begin at around the level of the eyelashes or about halfway up the black pupil as it rises upward.

Therefore, if we look at the starting point of the nasal bridge and consider the length from there to the tip of the nose as the nose length, it is best if it is the same length as the forehead.
However, some people have relatively long faces, and in such cases the height needs to be lowered a little.
If the bridge is uniformly raised from the glabella to the eyelashes, the nose can look too large and longer.
Although the nose itself may look beautiful, it may not suit the face.
Therefore, during consultation, we explain that if the area between the glabella and the nose is raised on a long face, the face may look longer, more aged, and more androgynous,
and in severe cases it may even take on a masculine impression, so it is better to keep the glabella lower.
From the front, rather than the line, people look at the width of the nose.
Koreans often have a wide nose width, commonly referred to as a bulbous nose, and many people want to reduce it as much as possible.
For people with somewhat prominent cheekbones, reducing the width of the nose too much is not good.
Because the nose appears too small, the empty space around it draws more attention.
On the other hand, for people with very small cheekbones, reducing the width of the nose is the right approach.
There are ways to reduce nose width, such as alar reduction. Also, if the tip of the nose is raised, the nostrils can come closer together slightly, which can help reduce width.
Alar reduction alone often has limited effect.
So, whenever possible, for a more natural look, we explain that slightly raising the nose tip so that the nostrils naturally narrow is more effective. If we judge that alar reduction alone will be sufficient, we sometimes perform only that procedure.

Why does the impression change so much after nose surgery?
The nose is truly fascinating. When we first meet someone, talk for about 10 to 20 minutes, and then part ways,
if we think about that situation later, it is not easy to remember the shape of that person’s nose.
Unless the person has very distinctive features, such as having no hair or no double eyelids,
the shape of the nose usually does not come to mind clearly.
Then does that mean the nose is not very important in terms of appearance?
Not at all. Because the nose disperses attention from the center of the face, it does not stand out too much on its own,
while playing a role in highlighting the surrounding facial features.
