Hello. We are Oji Plastic Surgery & NPlus, thinking only about your beautiful and healthy nose for the past 16 years.
The content prepared this time is about upturned nose surgery.

In modern times, many people are highly interested in self-care, so they try various things such as exercising, skin care, or changing their hairstyle. As a result, they seek these changes to feel satisfied with their transformed appearance or to present a favorable image to others. Even so, the nose is one area that is difficult to change, and because it sits at the center of the face and adjusts the overall balance of the facial features, interest in nose surgery has increased when the shape of the nose is thought not to be straight and well-proportioned.

In general, people who are looking into nose surgery want their nose to be improved into a straight and balanced shape, whether it is low, crooked, humped, or upturned. Among these various nose shapes, when the nose is short and upturned, more of the nostrils are visible from the front, which can give a somewhat unsophisticated impression, so many people look into upturned nose surgery in greater detail.

What is upturned nose surgery? It refers to a nose where the tip is turned upward, so that the nostrils are highly visible from the front. This can be caused by congenital factors, and it can also appear after nasal surgery due to contracture, where the tip contracts and the nose becomes shorter. Therefore, upturned nose surgery is an operation that lowers the upturned tip so that the nose tip is corrected and no longer turns upward.

Therefore, for those considering upturned nose surgery, the aesthetic aspect inevitably takes priority first because of the appearance of the upturned nose. However, the nose is not only an important aesthetic feature of the face; it is also an organ responsible for breathing and smell, so functional aspects must also be checked at the same time. In particular, for an upturned nose, while the aesthetic aspect is of course important, it is essential to accurately identify the cause of the upturned nose and perform surgery according to that cause.

At Oji Plastic Surgery & NPlus, to ensure safe and satisfying upturned nose surgery, the cause of the upturned nose is accurately analyzed through preoperative 3D-CT scans, internal endoscopy, various allergy tests, and nasal functional tests. Based on the extracted data, the method and materials are selected so that the nose will not become upturned again, and surgery tailored to each individual is performed.

The surgical methods for upturned nose surgery are divided into cases of mild tip upturning and severe tip upturning.

In cases of mild tip upturning, the surgery is performed by using a strong supporting cartilage between the columellae to fix the tip cartilage so it does not turn upward, and then transplanting autologous cartilage to the tip to correct the overall nasal line.

In cases of severe upturned nose, strong autologous cartilage is used to sufficiently extend the length of the nose tip, then autologous cartilage is transplanted to the tip and the overall nasal line is corrected.

In the case of upturned nose surgery, it is important to apply a combination of surgical methods according to the individual’s condition so that the nose will not become upturned again over time. The combined correction methods for an upturned nose are divided into skin extension/cartilage grafting, septal extension, and columellar support.
-
What is skin extension / cartilage grafting? It secures the skin space needed for tip correction and transplants autologous cartilage to correct the tip height, creating a clearly elevated tip and completing an overall higher nasal line.
-
What is septal extension? Strong septal cartilage or autologous rib cartilage is used to extend the tip downward and forward, clearly increasing the length of the upturned tip so that the nostrils are not visible.
-
What is a columellar support? Ear cartilage or septal cartilage is transplanted between the inner legs of the alar cartilage (nostril cartilage) and firmly fixed so that the nose tip does not become upturned or droop again.

As such, upturned nose surgery is a highly difficult procedure among nasal surgeries because it involves both lowering the upturned tip and correcting the height of the tip at the same time. At NPlus, to significantly improve short, upturned nose tips, we perform at least two or more combined correction procedures. Because the surgery includes the 'septal extension' that lowers the upturned tip, the 'columellar support' that firmly fixes the tip so it does not turn upward again, and the 'skin extension & cartilage grafting' that satisfies both the height and length of the tip, a more satisfying upturned nose surgery can be achieved.

The materials used in upturned nose surgery are silicone for the bridge of the nose and autologous cartilage for the tip. Autologous cartilage includes ear cartilage, septal cartilage, and autologous rib cartilage, and the materials are selected for each person through preoperative screening.

For the silicone used in the nasal bridge, we do not use a pre-manufactured silicone implant as is; instead, it is carefully and finely shaped by hand after considering the individual’s overall image as well as the width, length, breadth, and height of the nasal bridge, allowing for a much more natural nasal bridge line.
