In cases of mild tip upturning, the surgery is performed by using a strong supporting cartilage between the columellae to fix the tip cartilage so it does not turn upward, and then transplanting autologous cartilage to the tip to correct the overall nasal line.

In cases of severe upturned nose, strong autologous cartilage is used to sufficiently extend the length of the nose tip, then autologous cartilage is transplanted to the tip and the overall nasal line is corrected.

In the case of upturned nose surgery, it is important to apply a combination of surgical methods according to the individual’s condition so that the nose will not become upturned again over time. The combined correction methods for an upturned nose are divided into skin extension/cartilage grafting, septal extension, and columellar support.

1. What is skin extension / cartilage grafting? It secures the skin space needed for tip correction and transplants autologous cartilage to correct the tip height, creating a clearly elevated tip and completing an overall higher nasal line.

2. What is septal extension? Strong septal cartilage or autologous rib cartilage is used to extend the tip downward and forward, clearly increasing the length of the upturned tip so that the nostrils are not visible.

3. What is a columellar support? Ear cartilage or septal cartilage is transplanted between the inner legs of the alar cartilage (nostril cartilage) and firmly fixed so that the nose tip does not become upturned or droop again.

As such, upturned nose surgery is a highly difficult procedure among nasal surgeries because it involves both lowering the upturned tip and correcting the height of the tip at the same time. For the silicone used in the nasal bridge, we do not use a pre-manufactured silicone implant as is; instead, it is carefully and finely shaped by hand after considering the individual’s overall image as well as the width, length, breadth, and height of the nasal bridge, allowing for a much more natural nasal bridge line.
