You may be thinking, “Don’t all consultations happen while sitting down?”
But this is the first important and basic step that determines whether Ulthera succeeds or fails.
It’s not the consultation, but the design, that should be done while sitting down.
Q. Why do you have to design while sitting down?
A. Because true sagging only shows when you’re “sitting down.”
Most of the time we spend living our lives, the face is affected by gravity.
Naturally, the skin and fat are pulled downward.

If you only receive the procedure while lying in a bed,
the cheek fat that had sagged downward spreads to the side or upward, hiding the real problem area.
Now, if you look at the photo below, the fat in the cheek area (gray) appears to move toward the cheekbone area (yellow), right?

What happens if you treat only based on that state?
The laser ends up being applied to the wrong area, and unnecessary heat is delivered to the fat that should be preserved.
Then the fat cells shrink, causing the cheeks to sink in like a peanut-shaped face.
In the end, instead of being satisfied after spending a lot of money, you end up feeling even more stressed—a frustrating situation.

That is why I start all Ulthera consultations and design while “sitting down.”
If this is not followed, there is no need to look any further.

"Doctor, isn’t Ulthera always done while looking at the ultrasound screen?"
Yes, that’s right. But how clearly that screen can be seen determines the procedure’s details.

Q. What does an equipment upgrade have to do with the patient?
A. The better the doctor can see, the more precise and safe the procedure becomes.
Of course, doctors who have done a lot of Ulthera have know-how that comes from experience.
But from the patient’s perspective, it’s nearly impossible to know that.
So I recommend choosing a place that has the “Ultherapy Prime” device, which helps increase the procedure’s accuracy and safety.
The ultrasound screen is much larger and clearer, which improves the accuracy when delivering shots.

If the existing device showed up to 4.5 mm deep into the skin,
the Prime device shows up to 8 mm.
That means the procedure can be performed while understanding in greater detail the depth and shape of the fascia layer, which differs for each patient.
(This is especially good news for people with thicker skin.)

The last one is the key technique that raises satisfaction with the results.
Q. Isn’t it enough to have a lot of shots?
A. Not necessarily. In reality, you have to consider your budget, and there are “pillars” in the face that need priority.

The key is to accurately read the direction in which each person’s face is sagging,
and to focus shots on the “core support areas” that will help restore a collapsed face.
That may sound a bit difficult, so let me explain with an example.

If a patient is concerned about deep cheek fat, for example, we didn’t simply deliver energy only to the sagging area,
but instead applied shots so that the entire central cheek area would be pulled upward toward the upper face and ear side.
Only when the central cheek area, which serves as the core support, is firmly lifted does the nasolabial fold and deep cheek fat below it naturally smooth out as well.
If you tell the doctor about the area of concern during the consultation, they should be able to explain this kind of detailed shot design well.
