Hello, I’m Dr. Jo Won-jin, and I have been seeing patients for 10 years.

I’ve heard that if jaw Botox is injected incorrectly, it can cause cheek hollowing,
so I’m afraid. Is it still okay for me to get it?
Botox is a popular procedure, but if it is performed without considering the facial anatomy,
it can lead to a side effect where the cheeks appear sunken and the face looks gaunt.
Under the topic “How to Get Jaw Botox in Hapjeong Without Cheek Hollowing,”
I will explain why cheek hollowing can occur as a side effect of jaw Botox treatment,
and how to prevent it and safely get jaw Botox.

- The importance of the Botox injection point
The success of jaw Botox depends more on targeting—where it is injected—than on simply reducing muscle volume.
The masseter muscle, commonly called the square-jaw muscle, is broader and thicker than many people think.

If the medication spreads too much to the upper part of the muscle near the cheekbone,
or toward the side closer to the mouth,
what happens?
As the muscle volume that supports the area below the cheekbone suddenly disappears, shadows form and the so-called cheek hollowing appears.

That is why patients are often asked to clench their teeth before the procedure.
This is to directly identify, with the fingertips, the center of the thickest part of the muscle—the safe zone.
The upper part of the muscle that can cause cheek hollowing is excluded, and injections are placed mainly in the thickest lower portion.
If you look at the photo below, the injection should be placed in the lower part of the muscle, like the yellow area,
and if it is injected incorrectly at the red X-marked area, cheek hollowing can occur.

Another point to pay attention to here is the muscle marked in the black box, the muscle that lifts the corner of the mouth.
This muscle is in a shallower layer than the jaw muscle.
But if the injection point goes too far forward, or if the injection is placed too superficially instead of deep enough,
or if the dose is too much and spreads forward,
this muscle that lifts the corner of the mouth will also be affected.
Then, when smiling, the corners of the mouth may not lift enough and look awkward,
or one side may lift while the other does not, which can be unfortunate.
So this is another area that must be considered when selecting the injection point.
The medication spread to the muscle that lifts the corner of the mouth.

2. Treatment decisions considering skin elasticity
Before treatment, one thing that is just as important as the amount of muscle is the current skin elasticity.
Risks when elasticity is reduced
What happens if a large dose is injected when skin elasticity has decreased due to aging or rapid weight loss?
As the supporting structure of the muscle suddenly becomes smaller,
the skin covering it may be pulled downward by gravity and sag significantly.
This can go beyond cheek hollowing and make the marionette lines around the mouth more pronounced.

If cheek hollowing has already occurred?
If you are unfortunately experiencing cheek hollowing after treatment,
the first step is to wait until the medication ingredients are broken down in the body (about 3 to 6 months).
Over time, as the muscle grows back, it improves to some extent,
but in the field, there are also unfortunate cases where it does not recover completely.

Consider lifting procedures first
For those who already have significant hollowing or are concerned about elasticity, rather than Botox alone,
it may be better to first consider lifting procedures such as Ulthera or Shurink (of course, the cheek area should be avoided as much as possible),
or fillers and collagen boosters such as Juvelook that can restore lost volume.
It is like filling the reduced space of the muscle with elasticity so that even if the muscle volume decreases, support remains.
However, since these issues can be prevented before treatment,
it is better to consult with medical staff who have extensive experience with both Botox and lifting.

3. A safety checklist to prevent cheek hollowing
For a successful procedure, be sure to check the following during your consultation.
-
Precise technique: Confirm that the area below the cheekbone and the front cheek line are not being encroached on
-
Customized diagnosis: Get a consultation to see whether your skin elasticity can tolerate Botox
-
Aftercare: Confirm whether the clinic can respond immediately if signs of cheek hollowing appear
-
Principle of appropriate dosage: Inject an amount suitable for the size of the muscle, not simply a large dose

It is not simply a matter of saying, “Please inject this many units,”
but rather of sufficiently communicating with the doctor about your facial shape before proceeding with the procedure, which is how you reduce risk.
There are many low-cost event-priced options for jaw Botox in Hapjeong,
so cases of side effects due to incorrect injections are also common.
I hope you will carefully review the points I mentioned today and receive treatment safely.
If choosing a clinic is difficult, please read the post below.
At the very least, it will greatly reduce trial-and-error that you may regret after experiencing it.
Thank you. This was Jo Won-jin.
