Hello. I’m Wonjin Jo, the medical director of Chai Clinic, where I have been providing dermatology care in Hongdae for 10 years.
I received Sculptra incorrectly and developed
hard nodules on my face... 😭

The thing people consulting about Sculptra fear most is nodules.
However, the reason nodules develop is actually clear: the basic principles were not followed.
When properly prepared and administered according to the principles, Sculptra is an excellent treatment that creates natural-looking volume—more naturally than any other procedure.
Today, I’ll explain how to receive treatment without worrying about nodules by following the 233 rule that I adhere to as a strict principle.

Frequently Asked Questions (FAQ)
Book 2 days in advance: Allow sufficient hydration time
“Can I have the treatment immediately after today’s consultation?”
If you visit without an appointment and can receive the treatment immediately that same day, it may be worth questioning this.
(A consultation alone on the same day is fine.)
Sculptra is a powdered medication, so it must be diluted with saline and undergo a hydration process in which it is soaked in liquid.

As shown in the photo above, Sculptra is also a powdered medication, so soaking it in liquid is essential.
Like dried seaweed or rice, it becomes too hard to use if it is not soaked sufficiently.
Sculptra particles are the same way. If they are injected before being sufficiently hydrated, particles that have not fully dissolved can clump together and form nodules.
Therefore, it must be sufficiently hydrated for at least 24 hours so that the particles disperse evenly.
Administering the treatment immediately on the same day means there is a possibility that medication of unknown hydration time is being used, or that medication mixed in a hurry and not fully dispersed is being used.
That is why you should choose a clinic that prepares the product properly through an appointment made 2 days in advance and knows how to address even possible nodules.

Sculptra is rolled on a rotating roller, as shown in the photo above, so that it mixes evenly with the liquid.
3 areas: Recommended areas for Sculptra
There are clearly defined areas where Sculptra may be administered and areas that should absolutely be avoided.
The three recommended areas are the temples, lateral cheeks, and anterior cheeks.
It is suitable for areas with broad depressions, gently filling a wide area by promoting collagen regeneration.
However, areas with thin skin, such as under the eyes, have a higher risk of nodules, and the treatment itself is not suitable for them.
Filler is more suitable for areas that require firm structural support or have a lot of muscle movement, such as the forehead, nasal bridge, and chin tip.

You can see in the photo above that the volume in the lateral cheek has been filled naturally.
3 sessions: Divide the appropriate amount across the appropriate layers
Because Sculptra comes by the vial, some people try to inject a large amount at once so as not to waste it, or receive it in every area they think they need. This is risky.
Injecting too much at once can sharply increase the risk of nodules.
The standard approach is to divide the treatment into 3 sessions at intervals of 1 to 1.5 months.

The areas marked in purple in the photo above are the nodule sites.
The depth of injection is also important.
If injected into the epidermis or upper dermis, the risk of clumping increases. If it enters the fat layer too deeply, the effect may be reduced.
For the treatment to work properly, it must be accurately placed in the lower dermis, where collagen is produced most effectively. This depends entirely on the practitioner’s sensitivity and control at the point of injection.

Because skin thickness varies from person to person, the amount that each area can accommodate must be calculated and injected accurately.
After the treatment, you should massage the area at home for 5 days—5 times a day, for 5 minutes each time (5-5-5)—to help the medication spread evenly instead of clumping in one spot.

Still worried about nodules? (Alternatives to Sculptra)
“I’m very sensitive by nature, so I feel like I won’t even be able to sleep because I’m worried about nodules.”
For people like this, I recommend Juvelook Volume.
Its particles are smooth and round, with a noticeably lower risk of nodules.

(However, Sculptra is more effective when it comes to creating dramatic volume.)
A clinic should have extensive experience with different cases for each treatment and present both Sculptra and alternatives according to your preferences and the treatment area.
Whether the clinic can resolve nodules calmly if they develop is also an important criterion when choosing an experienced medical professional.
Frequently Asked Questions (FAQ)
Q. Do Sculptra nodules always develop?
A. No. They can be sufficiently prevented by ensuring adequate hydration, choosing the appropriate area and amount, injecting into the lower dermis, and performing post-treatment massage.
The key is to find a medical professional who follows these principles.
Q. When do the effects become visible?
A. Because this treatment gradually produces a natural result as collagen builds up, the area fills progressively over multiple sessions rather than becoming dramatically fuller all at once.

In summary, for Hongdae Sculptra, you can reduce the risk of nodules by following the 233 rule—booking 2 days in advance for hydration, focusing on 3 areas, and dividing the treatment into 3 sessions—along with post-treatment massage.
Please receive treatment safely at a clinic that operates according to these principles.
Thank you. This was Wonjin Jo.
