Not long ago,
one patient said they were startled
after looking at a group photo taken
at a company dinner.

"When did I get so old..?"
When people reach their 40s or 50s,
there are moments when the face in the photo
starts to feel unfamiliar.
In particular, when forehead wrinkles deepen
and the eyebrows begin to droop,
covering the double eyelid line,
you may end up looking tired or even angry.

These changes are a natural part of aging.
But these days, unlike in the past,
the word “middle age”
does not only suggest a late stage in life.
People are still in their prime, and still young.
Amid these changing times,
more people who want to look a little brighter
and younger naturally become
interested in “forehead lifting.”
Is forehead lifting a surgery that pulls upward?

When people hear about forehead lifting,
most think this way.
“Isn’t it just a surgery that pulls the skin upward
so the eyes look more open
and the forehead wrinkles are smoothed out?”
That is 50% correct.
On the surface,
that description is also true.
However, it is a surgery that works through
more precise and anatomical principles.
The problem may not be just wrinkles.

As we age, skin elasticity decreases,
and the muscles and ligaments
that support that elasticity gradually weaken.
As a result,
the eyebrows, which should stay in their original position,
gradually descend
and begin to cover the eyelids.
Then,
the eyes naturally feel more closed in,
and vision can seem blurred.
At this point, the body
begins to respond instinctively.

You start using the forehead muscles
to help open your eyes.
This is the moment when
deep forehead wrinkles
begin to form.
As all of these factors overlap,
the eye area droops and the impression can appear dull,
creating a vicious cycle.
It is important to accurately identify the cause of forehead wrinkles.

So,
it is important first to understand exactly
why the eyebrows are gradually lowering.
If only the visible skin is pulled,
the problem may not improve.
Our face is not made of skin alone.
The muscles, ligaments,
and skin layers underneath are connected
and move together.
In other words,
to achieve a natural lift,
you need to anatomically understand
why the eyebrows drop
and why wrinkles form.

An endoscopic forehead lift
is a procedure in which four to five small incisions,
about 1–2 cm each, are made in the scalp inside the hairline,
and an endoscope is inserted through them
to pull the forehead skin and muscles upward
and secure them in place.
Because the incision sites are hidden
inside the hair,
one major advantage is that
the surgical marks are not easily visible.
For this reason, an endoscopic forehead lift
can be considered by people who

without being overly noticeable.

However,
if your forehead is already wide,
the forehead may appear even wider,
so it is essential to consult a specialist
and check whether this is the right procedure

| 2. Incisional forehead lift |
After making an incision along the hairline,
the stretched forehead skin is pulled upward
and the excess skin created in the process
is removed.
This method is especially suitable for middle-aged and older patients
whose skin above the eyes has sagged
or whose double eyelid line is being covered.
As the eyebrow position is lifted more stably,
the heavy-looking impression can change more clearly.

Another point is that
if the forehead appears wide,
adjusting the incision location can visually
help balance the overall facial proportions.
Scarring from the incision may occur,
but since the incision is designed within the scalp
or inside the hairline,
it is planned so that it is not easily noticeable
in everyday life.

Above all, a forehead lift
is not simply a surgery that raises the skin.
If needed, the forehead muscles, retaining ligaments,
and even the SMAS layer may be adjusted.
It is a precise and planned surgery
that modifies the entire anatomical structure
between the forehead and the eyes.

| 3. Forehead reduction surgery (combined lifting) |
If the forehead is wide
and eyebrow sagging is also present,
a method that combines forehead reduction surgery
and forehead lifting at the same time
may be considered.

Through a hairline incision,
the forehead skin is pulled backward,
and the hairline is moved forward,
aiming to shorten the length of the forehead
while also improving the upper facial lift.
With one incision,
the forehead length, eyebrow position,
and forehead wrinkles can all be improved together,
so it may be considered for people with multiple concerns.
Every face has a different structure.
