What is a sunken scar?

It refers to a crater-like, depressed scar that occurs after having chickenpox or chronic acne.

What is DHL treatment for correcting sunken scars?

In the past, correction of sunken scars involved mechanical resurfacing or laser resurfacing,

or chemical peels, and sometimes the use of a CoolTouch laser,

to make scars shallower or less visible.

This approach is suitable for scars about 1–2 mm in size, but for large sunken scars over 5 mm,

the results are less than expected.

The DHL treatment performed at our clinic is an approach that more effectively corrects sunken scars

by performing three procedures at the same time: Subcutaneous Dissection,

Hyaluronic acid,

and Laser.

Hyaluronic acid used in DHL for correcting sunken scars

Hyaluronic acid is an intercellular substance found between the dermis and epidermis of the skin,

and it is a biological substance that adheres cells to one another and acts as a lubricant between cells.

The hyaluronic acid used here is non-animal hyaluronic acid.

Hyaluronic acid is the most widely used filler material,

and because it is a biological substance, it is a safe filler with a very low chance of side effects or inflammation.

It is widely used not only for correcting sunken scars, but also for fine lines on the forehead, around the eyes, between the brows, and around the mouth.

The effect of DHL treatment for sunken scars

No matter what kind of scar it is, once a scar forms, it is difficult to remove completely.

That is why it is called scar correction,

and the term scar removal does not exist medically.

Even with DHL treatment, the sunken area becomes shallower and is improved to some extent, but it cannot be removed completely.

The correct understanding of DHL treatment is that it turns deep, dark scars into shallower, less noticeable scars.

Sunken scars suitable for DHL treatment

DHL treatment can be applied to sunken scars measuring about 3–8 mm among depressed scars.

If the sunken scars are too small, subcutaneous dissection may not be performed well, so compared with larger scars,

the treatment effect may be lower or the procedure may not be possible.

For very small sunken scars,

chemical peels or

laser resurfacing may be better.

For large sunken scars over 8 mm, compared with DHL treatment,

fat grafting may be more effective for correction.

DHL treatment method for correcting sunken scars

1. Disinfect the skin and mark the area with the sunken scar using a design pen.

2. Perform local anesthesia in the area where the sunken scar is located.

3. After anesthesia, subcutaneous dissection is performed to create a space under the skin.

4. Inject hyaluronic acid (Juvederm) into the dissected space beneath the skin.

Juvederm pushes the skin in the sunken area upward, making the scar shallower.

5. Use a carbon dioxide laser to resurface the corner edges of the sunken scar.

6. After the procedure is finished, apply ointment and go home right away after applying makeup.

Examples of DHL treatment for sunken scars

If you receive DHC treatment, the sunken area becomes shallower and is improved to some extent.

The goal of the procedure is to correct deep, dark scars into a shallower, less visible state.

Advantages of DHL treatment

The procedure is simple and causes swelling and bruising only within a limited range.
The cost is low.
Daily life can be resumed immediately after the procedure.
The treatment effect is excellent.
A lasting, permanent effect can be obtained.

Disadvantages of DHL treatment

It is not suitable for sunken scars across the entire face, such as smallpox scars.
It cannot be applied to very small sunken scars (less than 2 mm in diameter) or may have little effect.
It is difficult to apply to darkly pigmented scars or white depigmented scars.

Number of DHL treatment sessions

Usually, satisfactory results can be obtained after 1–2 sessions.

If a repeat procedure is needed, it is performed at intervals of about 3 months.

For large scars, there are occasional cases where up to 3 sessions are performed.

Recovery process after DHL treatment

Bruising occurs after the procedure, and it usually fades after about 2 weeks.

Swelling also occurs, leaving the area slightly raised compared with the surrounding tissue before it settles down.

Bruising occurs only in a small area at the treatment site, so it is not very noticeable,

and since it can be covered with concealer or makeup, it does not pose much problem for social life.

Lasting effect of DHL treatment

It takes about 3 months for the treatment area to fully settle, and after that, there is almost no change.

After 3 months, a second procedure is decided upon; if the result is satisfactory, treatment ends with one session.

If the result is insufficient, a repeat procedure is performed.

Once the final result has been achieved, there are no further changes, so the lasting effect is permanent.

Side effects of DHL treatment

Inflammation may occur, but this is rare; if inflammation develops, the injected filler is removed.
Overinjection may cause the area to protrude more than the surrounding tissue; in such cases, a small amount of filler is removed, or hyaluronidase, which dissolves filler, is injected to dissolve the filler and reduce the swelling.
