Cheek fat grafting is one of the contouring procedures that can make a big improvement in facial appearance.

If the cheeks are gaunt and the cheekbones or jawbone stand out, fat grafting should be considered before bone surgery.

The ultimate goal of fat grafting is to create a soft, smooth oval face without sharp lines.

Candidates for cheek fat grafting

Fat grafting method for cheek contouring

1. Fat is harvested from areas where a lot of fat is stored, such as the lower abdomen, waist, or thighs.

• Because local anesthesia (painless sedation anesthesia) is used, there is no pain at all.

• A thin liposuction cannula is used to collect the fat into a syringe.

2. The harvested fat is placed in a centrifuge to obtain pure concentrated fat.

The purpose of centrifugation is to remove unnecessary components, reduce the side effects of fat grafting,

and increase the fat take rate.

3. After centrifugation, water and oil are removed through a second concentration process.

By removing water and oil, almost 100% concentrated fat is obtained.

The fat is measured into 1 cc syringes as needed.

Fat grafting for facial contour correction usually requires about 10 to 20 cc of concentrated fat.

4. The fat is grafted into the area under local anesthesia.

Fat grafting is performed after creating space for the fat through the tunneling process.

We use the micro-needle (fine grafting cannula) developed in-house.

The micro-needle is a special grafting cannula used only by our clinic for ultra-precise grafting.

The principle of fat grafting is to distribute the fat evenly over the grafted area so that it does not clump together and

to maximize the take rate as much as possible.

Effects of the fat grafting tunneling technique

Rather than injecting a large amount into one area with the tunneling technique, we perform 3D micro fat grafting with 100% concentrated fat, evenly and uniformly, in multiple layers. A delicate and thorough injection method allows for customized design. Fat grafting also includes a process called "tunneling," which allows the grafted fat to come into maximum contact with abundant surrounding blood vessels so it can continuously receive nutrients.
Tunneling means using a micro-needle just beneath the skin to create a space where the fat can go. Because space is prepared like drilling a tunnel and then the fat is injected, the fat does not sag in the direction of gravity or become bumpy after all of it has been placed.
Since the tunneling technique includes a separation process for wrinkles, fat is grafted beneath the separated wrinkle area, resulting in clearer wrinkle improvement than fat grafting at other clinics.

Examples of cheek fat grafting

Severely gaunt, wrinkled face

If the face looks gaunt, it can make you appear older and also give a worn-out impression, so it is better for the cheeks to have some volume.

There is no method other than fat grafting to add volume to the cheeks.

Commercial filler materials such as collagen or Restylane cannot compare with fat grafting in terms of cost or effectiveness.

Narrow inverted triangle face

If the face is shaped like an inverted triangle, fat grafting can

transform it into a fuller-looking face.

In such cases, fat grafting is performed from below the cheekbones to the jawline

to create volume.

From the front, the face can be changed into a fuller-looking face.

When the cheekbones appear prominent

If the cheeks are sparse or the sides are hollow,

the cheekbones will appear relatively more prominent.

Before choosing cheekbone reduction surgery to push the cheekbones inward,

one can first consider cheek fat grafting below the cheekbones

to make the cheekbones appear relatively less prominent.

With cheek fat grafting,

if you get enough corrective effect on the cheekbones,

there may be no need to undergo difficult cheekbone surgery.

In the case of a square jaw

Even when the jaw angle is large and the face appears square-jawed,

a corrective effect can be achieved with fat grafting.

By grafting fat into the sunken area above the square jaw,

a visual corrective effect can be achieved without jawbone surgery.

As the cheeks become fuller, the square-jaw area

appears relatively less prominent.

If you are considering jawbone surgery for a square jaw,

it is better to first consider cheek fat grafting.

If cheek fat grafting provides enough corrective effect for the square jaw,

there may be no need to undergo difficult jawbone surgery.

Sunken lines on the side of the cheeks

When the side of the face is sunken, creating a field-ditch-like hollow,

the hollowed area can be filled naturally

The side of the cheeks is an area where the bone and tissue are tightly attached,

making it very difficult to create natural volume.

The most difficult part of cheek fat grafting,

Cheek fat grafting is one of the most effective procedures for changing to a softer impression.

When contour lines form after fat grafting at another clinic

Many people tend to think fat grafting is easy,

but fat grafting is absolutely not a simple or easy procedure.

Some people think they can get good results by having fat grafting done at any clinic,

but fat grafting is a more demanding procedure than any other type of plastic surgery,

and it requires a very complex process along with extensive experience, know-how,

and very precise manual skill.

There are many clinics that perform fat grafting,

but not many that do it properly.

It is easy to perform, but difficult to create proper results.

If you undergo the procedure at a clinic that merely imitates it carelessly,

you can easily end up disappointed.

In principle, fat grafting can only produce good results when it is performed accurately.

It is better not to be treated by someone without much experience,

or by a non-specialist without board certification in plastic surgery.

When the cheeks become too full after fat grafting at another clinic

After fat grafting, there are cases where the cheeks appear saggy or overly full due to over-injection or excessive fat take.

In such cases, a fine cannula is used to suction and correct the shape.

The timing of suction should, in principle, be decided after the result is confirmed, about 3 months later.

However, if there has been excessive overfilling and the shape looks too awkward,

there are cases where suction needs to be done earlier.

Advantages of correcting wrinkles with fat grafting

The procedure is simple.
Because no foreign materials are used, unlike fillers (Restylane, Radiesse, Artecoll), there is no concern about side effects.
Because the procedure does not involve cutting the skin, there is no concern about scarring.
Because it uses the patient's own tissue, the shape looks natural.
There is almost no swelling or bruising.
You can return to daily life immediately after the procedure.
Since the fat tissue takes hold, the effect can last for a long time.

Ideal cheek fat grafting method

Several stages of fat purification	Fat purification should remove impurities completely through several stages to maximize the effect. The harvested fat usually contains about 80% impurities, and the usable portion is only about 20%. If the remaining impurities (blood, serum, anesthetic, oil, water, etc.) are not removed, the results will be poor no matter how well the grafting is done.
High fat take rate	With a high take rate, a satisfactory effect should ideally be achieved with just one procedure. Fine fat should be placed evenly in multiple layers, and before that, space should be prepared in the grafting area so the previously grafted fat is not compressed by surrounding tissue, allowing it to take hold well without pressure.
Expression of a soft contour	Expressing a soft, natural contour is the key to fat grafting. For a soft contour, it is better to graft in a 3D method that distributes the fat evenly across multiple layers. The grafting sites include the subcutaneous layer, muscle layer, and fascial layer. The fat should be spread through the tissue like sowing seeds evenly to achieve a soft contour without lines or irregularities.

Points to keep in mind when correcting cheek volume with fat grafting

Fat grafting requires skilled technique and experience. The result varies greatly depending on the surgeon's skill level, so careless fat grafting can actually make the impression worse, and caution is needed.
After fat grafting on the face, bandages or taping are not applied. Aside from swelling and some bruising, makeup can be applied and daily life can resume immediately, but for 1 to 2 weeks it may look slightly awkward due to swelling.
Running exercise after fat grafting has a negative effect on fat take, so running should be avoided for about a month. No drinking or smoking should be done for about a week. Except in cases where fat was harvested from the legs, walking, weight training, and stretching can be resumed after about a week.
Since cheek fat grafting takes hold over about 3 months, it is helpful for fat take to avoid strongly rubbing or pressing the cheeks while the fat is settling.

Number of cheek fat grafting sessions

Because the cheeks have a high fat take rate,

satisfactory results can usually be achieved with just one procedure.

In some cases, a second procedure is performed.

The second procedure is done between 3 and 12 months after the first,

and when performing the revision procedure, the fat harvested at the time of the first fat grafting is thawed and used.

However, if more than a year has passed before the second procedure, the fat must be harvested again.

Long-lasting effects of cheek fat grafting

Some people say fat grafting has little effect because much of the fat is absorbed,

but if fat grafting is done properly, a satisfactory effect can be achieved.

Fat grafting can produce good results only when performed based on skilled technique and experience.

Once the transplanted fat takes hold, it is no longer absorbed,

and it remains your own fat tissue for life,

so it is considered one of the safest and most effective methods.

On average, one fat grafting procedure, or rarely two,

can provide permanent, satisfying facial contour correction.

Side effects of fat grafting

Because fat grafting uses the patient's own fat,

unlike commercial filler materials, there are almost no side effects.

Aside from slight swelling and bruising after the procedure, it can be said that there are no side effects at all.

Therefore, it can be considered safer than any other plastic surgery.

However, this refers to cases where fat grafting is performed properly,

and if the procedure deviates from the principles, side effects can occasionally occur,

such as uneven contours,

fat necrosis leading to hardening,

inflammation,

or an awkward shape due to excessive injection.

These are issues determined by the practitioner's experience and skill,

and are by no means a general problem with fat grafting itself.

Therefore, if the procedure is performed according to the principles, these side effects are rare.
