Micro fat grafting for lips

If the lips lack volume or have many wrinkles, this procedure smooths the wrinkles while creating fuller lips.

Sometimes it is also performed to reduce the difference when there is a significant volume gap between the upper and lower lips.

Fat grafting procedure for lip correction

1. Fat is harvested from areas of the body where fat is stored in large amounts, such as the lower abdomen, waist, or thighs.

• Because local anesthesia (painless sedation anesthesia) is used, there is no pain at all.

• A fine liposuction cannula is used to collect the fat into a syringe.

2. The harvested fat is placed in a centrifuge to remove local anesthetic and serum.

The purpose of centrifugation is to remove impurities, reduce side effects after grafting, and increase fat survival.

3. After centrifugation, water and oil are removed through a second concentration process.

To achieve a fat survival rate of at least about 50%,

a two-step purification process after centrifugation is essential to remove oil and water.

The grafted fat must be free of impurities in order to improve survival and effectiveness.

In practice, a single centrifugation step is often not enough to sufficiently concentrate the fat.

By removing oil and water, concentrated fat that is close to 100% can be obtained.

The purified fat is measured in the required amount using 1cc syringes.

For fat grafting in the lip area, about 2cc of concentrated fat is grafted into each of the upper and lower lips.

For the chin (mandibular area), about 5cc to 7cc of concentrated fat is grafted.

4. The fat is grafted into the area to be treated, which has already been locally anesthetized.

A micro needle (micro grafting cannula) developed in-house is used.

The micro needle is a special grafting cannula used only at our clinic, enabling ultra-precise grafting.

For fat grafting, after the tunneling process is complete, space is secured in the area where the fat will be grafted before the grafting is performed.

The principle of fat grafting is to distribute the graft evenly and uniformly across the grafting area

so that the fat does not clump and the survival rate can be maximized.

To do this, a small amount of concentrated fat must be finely grafted per unit area

and spread evenly across multiple areas.

Even if a large amount of fat is grafted into one area,

fat without blood supply will eventually necrose and be resorbed.

Example of lip fat grafting procedure

If the lip line is thin, a three-dimensional fat grafting procedure is performed.

The fat is mainly grafted into the orbicularis oris muscle of the lips,

and it is distributed evenly into both the deep and superficial parts of the muscle layer

to maximize fat survival while creating a soft lip shape.

An added benefit of fat grafting is that it smooths lip wrinkles.

Advantages of lip correction with fat grafting

The procedure is simple.
Unlike filler materials such as Restylane and Artecoll, it rarely causes the lips to become hard or lead to inflammation.
Because the procedure does not require skin incisions, there is no concern about scars.
Since it uses autologous tissue, it is soft and natural in shape.
The surviving fat remains permanently as your own tissue, so the effect can last for a long time.
It has the effect of softening vertical lip wrinkles.

Disadvantages of lip correction with fat grafting

The lip area tends to swell more after the procedure than other areas.
The lip area has a relatively high fat absorption rate, so second procedures are more common.

Precautions when correcting with fat grafting

After the procedure, do not apply pressure or impact until the fat has settled.
Running exercises should be avoided for about a month because the movement can reduce fat survival.
Avoid alcohol and smoking for about a week.
Avoid opening or puckering the mouth too widely so that fat absorption is reduced.

Number of micro fat grafting procedures for the lips

In the lip area, the survival rate of fat is lower than in other areas.

If the average survival rate of fat grafting is considered 50%, the lip area is around 30%.

Because the lips are constantly moving, the conditions for fat cells to survive are not ideal.

Compared with areas that move less, such as the chin and cheeks, the survival rate is lower,

so an average of about two fat grafting sessions should be considered.

At the first grafting, it is possible to graft as much as possible so that even if much of the fat is absorbed,

a larger total amount can still survive,

but if too much is grafted at the beginning,

the lips may swell significantly right after the procedure and interfere with daily life.

Also, excessive fat grafting can actually reduce survival rates

and increase the possibility of side effects, so it is better to graft an appropriate amount.

Long-lasting effects of fat grafting

Some people say that fat grafting has no effect because much of the fat is absorbed,

but this applies to cases where the procedure was done at a clinic without experience.

If the procedure is performed correctly based on principles,

satisfactory results can be achieved.

Once the grafted fat survives, it is no longer absorbed

and remains as your own fat tissue for life, making it one of the safest and most effective procedures.

On average, after one to as many as two fat grafting sessions,

a permanently satisfactory facial contour correction effect can be obtained.

Side effects of fat grafting

Fat grafting is among the procedures with the fewest side effects.

However, side effects may occasionally occur due to inexperience on the part of the practitioner.

Excessive fat injection can cause severe swelling and fat necrosis,

leading to calcification, uneven contours, reduced treatment effect due to excessive fat absorption,

and redness caused by inflammation; these are issues related to the surgeon's experience and skill.

It is advisable not to undergo fat grafting from a practitioner with insufficient experience or from someone who is not a board-certified plastic surgeon.

Types of side effects from fat grafting

Of course, good results are the goal,

but depending on the surgeon's skill and technique,

sometimes unavoidable problems can occur.

Poor results due to low survival rates, awkward shape, asymmetry, severe swelling, inflammation,

fat necrosis, calcification, contour irregularities, sagging, and pigmentation may occur.

Inflammation	Occurs when bacteria enter during the procedure or when fat that fails to survive liquefies during the grafting process
Necrosis	A state in which fat that failed to survive due to insufficient blood supply during the grafting process rots and leaves behind remnants that have dissolved
Calcification	A phenomenon in which fat that failed to survive due to insufficient blood supply hardens and calcium deposits form, turning it into a firm lump
Contour irregularity	A phenomenon in which the shape becomes uneven because fat is not grafted evenly or is concentrated in only one area
Sagging	If too much is grafted or grafting is performed in the wrong place, sagging of the face may occur.
Pigmentation	If bruising and swelling are severe, iron remaining in the bruise may deposit in the skin and oxidize, turning brown depending on the person's constitution; this can sometimes occur after under-eye procedures.
Asymmetry	When the survival rate differs between left and right sides, or when the amount is not properly adjusted during the first procedure, resulting in imbalance
High absorption rate	A phenomenon in which the fat does not survive enough to produce the desired contour, much of it is absorbed, and the shape before and after the procedure does not change much
Awkward shape	A phenomenon in which fat survives excessively or is grafted into the wrong area instead of the appropriate one, disrupting the overall balance of the face

Because fat grafting uses autologous fat,

it has almost no side effects compared with commercial filler materials.

Except for slight swelling and bruising after the procedure, it can be considered free of side effects.

Therefore, it can be said to be safer than many other cosmetic procedures.

However, this is based on cases where the fat grafting is done properly,

and if the procedure goes against the principles, side effects may occasionally occur,

such as an uneven shape,

fat necrosis and hardening,

inflammation,

or an awkward appearance due to excessive injection.

These are determined by the practitioner's experience and skill and are by no means an overall problem with fat grafting itself.

Methods for a side effect-free fat grafting procedure

Procedure in a sterile environment	As with all surgeries, fat grafting must be performed with completely sterilized instruments and materials. After the procedure, the inside of the suction cannula should be thoroughly cleaned with a brush and sterilized in a high-pressure steam sterilizer so that no fat residue remains inside the cannula used. Anesthesia solution and saline should also be carefully managed to prevent contamination. Thorough disinfection and material management can prevent bacterial infection and inflammation.
Multi-step purification of fat	Fat purification should be carried out in several stages to completely remove impurities and maximize the effect. Harvested fat usually contains about 80% impurities, and only about 20% is actually usable. If the remaining impurities (blood, serum, anesthetic, oil, water, etc.) are not removed, the result will not be good no matter how well the graft is performed.
Preconditioning before fat grafting	With a high survival rate, a satisfying effect should ideally be achieved with a single procedure. Fine fat should be placed evenly in multiple layers, and before that, space should be secured at the grafting site so that previously grafted fat is not compressed by surrounding tissue; this allows the fat to avoid pressure and survive well.

local anesthetic, followed by oil.

It also contains impurities such as serum and red blood cells.

The remaining fat after the first procedure is rapidly frozen, sterilized, and stored.

For the second procedure, the stored fat is thawed and the treatment can be performed more easily.

The storage period for fat is up to 1 year, and if you visit within 1 year, the stored fat will be used for the procedure.
