What is Col-fat fat grafting?

After micro fat grafting is performed, the grafted fat takes hold as capillary formation occurs around it.

The process of engraftment takes at least three months, and during that time the fat may be absorbed because it has not been vascularized, or it may be compressed and disappear due to pressure from the surrounding tissue.

Therefore, there are cases where increasing volume with fat alone is not enough. To supplement this, Col-fat fat grafting is designed to promote collagen production on its own, increase the fat graft survival rate, and create youthful, elastic skin and contours.

① It supplies serum, which is the nutritional source in the early stage of fat engraftment.

② It minimizes pressure from surrounding tissue and increases the engraftment rate.

Col-fat fat grafting procedure

At our clinic, the procedure is performed using micro needles developed in-house.

The key feature of the micro needle is that the tip of the probe is precisely processed to induce collagen production during subcutaneous tunneling.

We have also realized ultra-precise fat grafting with the Col-fat technique.

Because it has the advantage of enabling precise procedures in 1–2 mm units even in delicate areas such as very fine wrinkles, under the eyes, and above the eyes, the procedure can be performed without difficulty even in the most challenging areas.

Using a micro probe for under-eye dark circle correction makes it possible to achieve effective dark circle correction with almost no contour irregularity.

Through tunneling with a micro probe and micro fat grafting using micro needles, even the most difficult under-eye procedures can be performed effectively.

Fat grafting for facial contour correction

Fat grafting is also called fat injection.

It refers to using a syringe to harvest fat from your own body and then injecting it into the desired area with a syringe to correct facial contours.

It is often called micro fat grafting, which means the procedure is performed more precisely and delicately.

It can effectively correct sunken areas, irregular contours, and deep wrinkle lines on the face, and it can be applied to most parts of the face.

Because the procedure is simple, has no side effects, and can be naturally corrected in any area to the desired degree, fat grafting is one of the effective facial contour plastic surgery procedures.

Misconceptions about fat grafting

Some say that fat grafting is quickly absorbed and therefore ineffective, but this is a misunderstanding.

In principle, fat grafting is effective only when performed properly.

The results vary depending on the practitioner's experience and skill.

Good results can be achieved only when everything is done accurately and carefully, from harvesting the fat to refining it and grafting it.

Procedures done without principles may have little effect or may even cause side effects.

Proper fat grafting often brings satisfactory results with just one procedure, and the effects last for a very long time.

In areas such as the cheeks, the effects are often permanent.

Comparison between fat grafting and filling materials (fillers)

Various filling materials (fillers) are sold and used in plastic surgery and dermatology to correct facial wrinkles and scars.

Examples include collagen, Alloderm, Restylane, Artecoll, Juvederm, Aquamid, and Interpol.

These filling materials are mainly used to correct fine wrinkles rather than facial contour correction.

They are too small in volume, expensive, and inappropriate for contour correction, and improper use can lead to side effects, so caution is needed.

For facial contour correction, fat grafting can be considered an effective and safe procedure.

All anesthesia is performed with local anesthesia (sedation anesthesia).

All fat grafting procedures are performed under local anesthesia, regardless of the area being treated.

Local anesthesia is administered together with sedation anesthesia, so the procedure can be carried out without pain either during anesthesia or after it ends.

Depending on the area, hyaluronidase, a swelling-prevention agent, is mixed into the local anesthetic and administered.

Hyaluronidase is an expensive anti-swelling agent that has a tissue-fluid-diffusing effect, helping prevent swelling from occurring almost immediately after the procedure.

We have a systematic and advanced fat grafting system.

The success of fat grafting depends on the engraftment rate.

To maximize the engraftment rate, the following are necessary.

■ Living fat cells must be harvested.

■ To increase fat purity, the fat is refined as much as possible through centrifugation and removal of unnecessary oil.

■ Before fat grafting, subcutaneous tunneling in the graft site secures space for the fat to be grafted.

■ Using micro fat grafting cannulas, precise and delicate fat grafting is performed for each area.

■ By grafting microscopically in multiple layers in a three-dimensional manner, the engraftment rate is increased and a soft contour is created.

We select appropriate and various fat-harvesting sites.

The most commonly used site for fat harvesting is the lower abdomen. Because the lower abdomen causes less pain after the procedure, with less swelling and bruising and is comfortable for daily life, it is the first choice for fat harvesting.

If there is insufficient fat in the lower abdomen or if the person wants another area, fat can be harvested from other sites as well. The next most commonly chosen area is the inner thigh, followed by the outer thigh.

If there is not enough fat in the thighs either, the lower buttocks or the flanks may also be used.

There is no fixed rule for which harvesting site is best for fat engraftment.

Results are similar regardless of the harvesting site. The ideal harvesting site is the part of your body with the most excess fat, but it is best to choose the site while considering the amount of fat to be harvested and the activities after grafting.

A two-step purification process for the harvested fat maximizes the engraftment rate.

The harvested fat goes through a two-step purification process to remove impurities other than fat cells.

Among the impurities in the harvested fat, the largest proportion is local anesthetic, followed by oil.

It also contains impurities such as serum and red blood cells.

In the first step, it undergoes centrifugation for about 3–4 minutes, and in the second step, it enters the oil-removal process.

The parts purified by centrifugation are local anesthetic, red blood cells, serum, and so on.

Oil is removed manually through natural absorption.

Through the two-step purification process, nearly 100% concentrated fat is obtained, maximizing the effect of fat grafting while increasing the engraftment rate as much as possible.

Subcutaneous tunneling secures space for fat grafting.

Subcutaneous tunneling is a necessary process for securing space in advance in the area where the fat will be grafted, in order to achieve a high engraftment rate and even grafting.

It is the process of moving a fine probe repeatedly in a fan-like pattern under the skin to create space for the fat to be grafted.

The effect of subcutaneous tunneling is to increase the engraftment rate and prevent the fat from clumping, creating a uniform and soft contour.

Three-dimensional micro fat grafting can create a soft and even facial contour.

Although there are exceptions depending on the area, the basic method of fat grafting is a three-dimensional technique.

By grafting small amounts evenly across multiple layers in each area, the engraftment rate of the fat is increased, and engraftment becomes uniform, helping create a soft contour.

Micro fat grafting cannulas are used for precise fat grafting.

In the past, methods using fat-injection machines pushed fat under the skin with high pressure, which caused various problems.

The absorption rate of the fat was high, engraftment was uneven, the contour became bumpy, and grafting into delicate areas such as under the eyes was difficult.

To solve these problems, when performing fat grafting, the fat is transferred again into a fine 1 cc syringe and then grafted into the desired area.

At our clinic, we use micro needles (micro grafting cannulas) developed in-house.

Micro needles are special grafting cannulas used only at our clinic, enabling ultra-precise grafting.

Fat grafting can also be performed on the upper eyelids, lower eyelids, forehead wrinkles, glabella wrinkles, nasolabial folds, and the bridge of the nose, making fat grafting possible in almost all cases except for fine wrinkles.

After the fat grafting, molding is performed to create a uniform and soft facial contour.

Immediately after the fat grafting, delicate molding work is performed so that the grafted fat spreads evenly and creates a soft shape.

Molding is the final step in fat grafting and one of the most important processes.

The remaining fat is cryogenically frozen and stored aseptically to simplify the second procedure.

If a second procedure is performed three months after fat grafting, the fat used in the first procedure is not harvested again; instead, the leftover fat from the first procedure is used.

The remaining fat after the first procedure is cryogenically frozen and stored aseptically.

For the second procedure, the stored fat is thawed, allowing the procedure to be performed more easily.

The storage period for the fat is up to 10 months. If you visit within 10 months, the procedure will be performed using the stored fat.

Thorough aftercare system

After fat grafting, we provide thorough aftercare and carefully monitor the progress until the fat has fully engrafted.

During follow-up, if necessary, we try to minimize dissatisfaction by using swelling-reduction injections, shape adjustments, and molding.

1st fat grafting procedure

Fat grafting is performed through a staged and systematized process.

The procedure involves multiple stages, including painless sedation anesthesia, fat harvesting, purification, tunneling, and grafting.

The procedure can take as little as 30 minutes, and when grafting a wide area, it may take more than 2 hours.

1. The area to be grafted is designed with marking ink.

2. An intravenous injection is given for painless sedation anesthesia.

Sedation anesthesia is an auxiliary form of anesthesia that helps you sleep for the five minutes needed for local anesthesia, eliminating the pain of local anesthesia.

3. While sedation anesthesia is being administered, local anesthesia is applied to the fat-harvesting area and the grafting area.

Because you are under sedation anesthesia, there is no pain at all during the local anesthesia.

After the local anesthesia is complete, you wake up from sleep, and afterward the area remains anesthetized, so the procedure is performed without any pain.

In some cases, hyaluronidase, a swelling-prevention agent, is mixed into the local anesthetic and administered.

Hyaluronidase is an expensive anti-swelling agent with a tissue-fluid-diffusing effect that helps prevent swelling from occurring almost immediately after the procedure.

4. Fat is harvested from areas where a lot of your own fat is stored, such as the lower abdomen, thighs, or flanks.

A thin liposuction cannula is inserted, and the required amount of fat is collected into the syringe.

The amount may range from 60 cc in smaller cases to 200 cc when a large grafting area is involved.

5. After the necessary amount of fat is harvested, it is left standing for a short time.

After a few minutes, impurities settle to the bottom.

The settled impurities consist of local anesthetic, water, serum, and red blood cells.

After all the impurities have settled, the impurities at the bottom are discarded and the remaining fat undergoes the first-stage centrifugation process for fat concentration.

6. After centrifugation, the fat enters the third concentration step, which removes the oil.

The fat that has been refined by centrifugation is placed on sterile gauze, wrapped, and allowed to naturally absorb out unnecessary oil and moisture.

After the second purification process is complete, nearly 100% concentrated fat is obtained.

The engraftment rate of the fat depends on how properly the fat is concentrated.

The more it is concentrated, the more fat cells can be grafted into the same area.

There is a big difference in the procedure results between fat concentrated to nearly 100% and fat that is not, so concentration by centrifugation alone is not enough; an additional step to remove the oil is essential.

7. Before injecting the fat, subcutaneous tunneling is performed in the graft site.

This process creates space to allow the grafted fat to engraft smoothly under the skin.

It is also a step to ensure that the fat is grafted evenly and creates a soft contour.

In other words, subcutaneous tunneling is a preparatory step for increasing the engraftment rate while creating a soft contour.

8. The concentrated fat is grafted into the area where subcutaneous tunneling has been completed.

We use micro needles (micro grafting cannulas) developed in-house.

Micro needles are special grafting cannulas used only at our clinic, enabling ultra-precise grafting.

The grafting is performed evenly and in small amounts across multiple layers so that the fat engraftment rate can be maximized.

This is commonly called three-dimensional fat grafting.

9. After the fat grafting is complete, molding is performed to create a soft contour.

Molding gently presses or smooths the grafted area so that the fat spreads evenly.

Molding is the final stage of fat grafting and also the most important process for shaping.

10. After the fat grafting is complete, the remaining fat is rapidly frozen, processed aseptically, and stored.

Fat is usually stored for up to one year, and when a second procedure is needed, the fat is thawed so that a second fat grafting can be done easily.

The fat is first thawed in a refrigerator, then thawed again at room temperature a few minutes before fat grafting.

11. After fat grafting, regardless of the area treated, no bandages or dressings are applied to the face.

You can go home immediately after the procedure and return to daily life.

Procedure process for second fat grafting

Fat grafting does not always end with a single session.

Engraftment rates differ by area, and there are individual differences as well.

Because the grafted fat is absorbed by an average of about 50%, the shape may look good at first but gradually be absorbed over time, so in some cases a second procedure may be needed.

□ Three months to one year after the first fat grafting, you visit the plastic surgery clinic again to check the progress.

□ If a second fat grafting is judged to be necessary, the frozen fat stored from the first procedure is thawed and injected again after local anesthesia.

□ Compared with the first procedure, the fat-harvesting step is omitted, so the procedure time is much shorter, and the cost is about half of the first fat grafting procedure.

□ If the shape looks good because engraftment went well after the first procedure, no further fat grafting is necessary.

In other words, the second procedure is performed only when necessary, and you can decide whether you need it based on the appearance.

Fat grafting is usually performed in one to two sessions.

If a second procedure is needed, it is performed between three and ten months later.

On average, after one to two sessions, satisfactory facial contour correction can be achieved.

At our clinic,

based on more than 20 years of rich clinical experience and know-how,

we perform multi-layer fat grafting.

Fat grafting uses living cells,

so the key is to ensure that the fat cells have a high engraftment rate.

To increase the engraftment rate, we do not inject a large amount into one place,

but instead graft it evenly and uniformly across multiple layers.

Between the skin and bone there are multiple tissue layers, such as the fat layer and muscle layer,

and the fat must be injected in a multi-layer structure without overlapping,

such as between the periosteum above the bone and between the fascia above and below the muscle.

Multi-layer fat grafting

has a duration 3–4 times longer than before and also has a higher fat survival rate.

Our clinic’s fat grafting also includes a process called tunneling,

which ensures maximum contact with the abundant surrounding blood vessels so that nutrients can be supplied continuously.

Tunneling is the process of using micro needles just under the skin

to create a place for the fat to go.

In other words, because space is created as if drilling a tunnel and then the fat is injected,

even after all the fat has been placed, it does not sag in the direction of gravity or become bumpy.

In addition, a delicate injection method allows for customized design.

After fat grafting, we provide thorough aftercare

and carefully monitor the progress until the fat has fully engrafted.

During follow-up, if necessary, we try to minimize dissatisfaction

by using swelling-reduction injections, shape corrections, and molding.
