What Is Fat Grafting?

​Fat grafting is also called fat injection.

Because it is performed with delicate and precise fat grafting, it is also referred to as microfat grafting.

Using a syringe, your own fat is harvested and then injected again with a syringe into the desired area to correct the contours of the face.

The key to microfat grafting lies in expressing natural, soft contours and achieving a high take rate.

Using three-dimensional fat grafting and two-dimensional flat grafting, the most ideal fat grafting is performed for each area.

Except for bone surgery such as cheekbone and jawbone surgery, contour correction for sunken areas can be achieved with fat grafting.

The procedure is simple, has no side effects, and can naturally correct any area as much as desired, making it one of the best facial contouring procedures.

Understanding Fat Grafting

When Can Wrinkles Be Removed Using Fat Grafting?

It is mainly an effective procedure for deep expression lines on the face.

Commercial fillers (Restylane, Juvederm, Artecoll, Rofilane) are mainly used for fine wrinkles, while deep and broad expression lines (nasolabial folds, glabellar wrinkles, forehead wrinkles, under-eye hollow wrinkles) can achieve satisfactory results with fat grafting.

Effects of Wrinkle Removal Fat Grafting

● Rather than injecting a large amount into one spot using the tunneling technique, we perform three-dimensional microfat grafting with 100% concentrated fat, evenly and uniformly across multiple layers.

Customized design is possible using a delicate and careful injection method.

Fat grafting also includes a process called “tunneling,” which allows the grafted area to make maximum contact with the surrounding abundant blood vessels so that it can receive nutrients continuously.

● Tunneling means creating a space for the fat just beneath the skin using a micro-needle. Because the space is prepared like drilling a tunnel and the fat is injected afterward, even after all the fat has been placed, it does not droop in the direction of gravity or become uneven.

● The tunneling technique includes a dissection process for wrinkles, so the fat is grafted beneath the dissected wrinkle area, resulting in much clearer wrinkle improvement than fat grafting at other clinics.

Fat Grafting Procedure for Wrinkle Removal

1. Fat is harvested from areas where your own fat is stored in large amounts, such as the lower abdomen, waist, or thighs.

• Since local anesthesia (sedation pain-free anesthesia) is used, there is no pain at all.

• A thin liposuction cannula is used to collect the fat into a syringe.

2. The harvested fat is placed in a centrifuge to obtain pure concentrated fat.

The purpose of centrifugation is to obtain pure concentrated fat, reduce side effects after grafting, and increase the fat take rate.

3. After centrifugation, water and oil are removed through a second concentration process.

By removing water and oil, concentrated fat that is nearly 100% pure is obtained.

4. The fat is transferred again into a thin 1cc syringe and grafted into the desired area.

At our clinic, we use a self-developed microneedle (micro grafting cannula).

The microneedle is a special grafting cannula used only at our clinic, enabling ultra-precise grafting.

Examples of Wrinkle Correction with Fat Grafting

1. Forehead hollowing and wrinkles

For the forehead, fat grafting is performed when there are sunken areas or deep forehead wrinkles.

2. Wrinkles between the eyebrows

For glabellar wrinkles, fillers such as Restylane or Juvederm can be used for fine wrinkles, but better results can be achieved by performing fat grafting for deeply etched vertical glabellar lines.

3. Under-eye wrinkles

For under-eye wrinkles, satisfactory results can be seen in sunken wrinkles between the eyes and nose.

4. Fat grafting for mouth-area nasolabial folds

If fat grafting is performed for nasolabial folds beside the nose or wrinkles around the mouth, sufficient correction can be achieved even in severely sunken wrinkles.

Precautions When Having Wrinkle Plastic Surgery with Fat Grafting

1. Because some of the fat is absorbed, one procedure may not be enough, and a second treatment may be needed after 2 to 3 months.

2. It is difficult to use for fine wrinkles around the eyes or under the eyes.

Number of Fat Grafting Sessions for Wrinkles

For most fat grafting procedures, including mouth-area nasolabial folds, about two sessions on average are enough to achieve satisfactory results.

The interval between procedures is 2 to 3 months, and for the second procedure, the fat that was stored when the first fat grafting was performed is thawed and used.

Duration of Fat Grafting Results

Some people say fat grafting is ineffective because much of the fat is absorbed, but if fat grafting is performed correctly, satisfactory results can be obtained.

Once the transplanted fat takes hold, it is no longer absorbed and remains as your own fat tissue for life, so it is considered one of the safest and most effective methods.

On average, receiving fat grafting twice can provide semi-permanent, satisfying wrinkle improvement.

Side Effects of Fat Grafting

Of course, good results are the goal, but depending on the surgeon’s skill and technique, unavoidable issues may sometimes arise.

Poor results due to a low take rate, unnatural shape, asymmetry, severe swelling, inflammation, fat necrosis, calcification, contour irregularities, sagging, and pigmentation changes may occur.

Side Effects Issues of Fat Grafting

Because fat grafting uses your own fat, unlike commercial filler materials, there are almost no side effects.

Aside from slight swelling and bruising after the procedure, it can be said that there are essentially no side effects.

Therefore, it can be considered safer than any other cosmetic procedure.

However, this is based on the case where fat grafting is performed properly. If the procedure is done against the basic principles, side effects can occasionally occur, such as uneven contours, fat necrosis and hardening, inflammation, or an unnatural shape due to excessive injection.

These are determined by the practitioner’s experience and skill and are absolutely not an overall problem with fat grafting itself.

Therefore, if the procedure follows the correct principles, these side effects are rarely seen.

How to Perform Fat Grafting Without Side Effects

◆ Perform the procedure in a sterile state

As with all surgeries, fat grafting must also be performed using completely sterilized instruments and materials.

After the procedure, any remaining fat residue inside the cannula or grafting tube should be thoroughly cleaned with a brush, then disinfected with a high-pressure steam sterilizer. Anesthetic agents and saline must also be carefully managed so that they are not contaminated.

Thorough disinfection and material management can prevent bacterial infection and inflammation.

◆ Several stages of fat purification

Fat purification must completely remove impurities through several stages to maximize the effect.

The harvested fat usually contains about 80% impurities, and the usable portion is only around 20%.

If the remaining impurities (blood, serum, anesthetic, oil, water, etc.) are not removed, the results will be poor no matter how well the grafting is done.

◆ Preliminary preparation before fat grafting

A high take rate should make it possible to achieve satisfactory results with just one procedure, if possible.

Microfat must be evenly placed in multiple layers, and before that, space must be secured at the grafting site so that previously grafted fat is not compressed by the surrounding tissue. Only then can the fat avoid pressure and take hold properly.

◆ Precise and even grafting into multiple tissue layers

Expressing soft and natural contours is the key to fat grafting.

For a soft contour, it is best to graft using a three-dimensional method that distributes the fat evenly across multiple layers.

The grafting locations include the subcutaneous layer, muscle layer, and fascia layer, and the fat should be spread through the tissue like sowing seeds evenly to achieve a smooth contour without irregularities.

The Process of a Second Fat Grafting Procedure

Fat grafting is not always completed in a single session.

The take rate differs by area, and there are individual differences as well.

Since about 50% of the grafted fat is absorbed on average, the shape may look good at first but gradually be absorbed over time, so in some cases a second procedure may be needed.

• You will visit the plastic surgery clinic again between 3 months and 1 year after the first fat grafting to check the progress.

• If a second fat grafting procedure is deemed necessary, the frozen fat stored from the first procedure is thawed and injected again after local anesthesia.

• Since the fat harvesting process is omitted compared with the first procedure, the procedure time is much shorter, and the cost is about half that of the first fat grafting procedure.

• If the fat takes well after the first procedure and the shape is considered good, no further fat grafting is needed.

In other words, a second procedure is performed only when necessary, and the decision can be made after checking the appearance yourself.

Fat grafting usually involves one to two sessions.

If a second procedure is needed, it is performed between 3 months and 1 year later.

If an average of two procedures is performed, satisfying facial contour correction results can be obtained.

Procedure Before and After Fat Grafting, and Post-Procedure Care

This is the pre- and post-procedure information for receiving fat grafting surgery.

1. Make a consultation appointment by phone or online.

2. Visit at the scheduled consultation time and receive the consultation.

3. If you wish to proceed, make a surgery appointment immediately after the consultation and leave.

You can also make a surgery appointment later by phone or online.

4. Arrive 30 minutes before the scheduled surgery time, receive an explanation of the fat grafting procedure, and have the design planned.

5. After the design is finished, change clothes and receive an intravenous injection for pain-free sedation anesthesia.

6. The procedure time for fat grafting varies by area, and if several areas are treated at the same time, it may take longer.

On average, it takes from 30 minutes to as long as 2 hours.

7. After the procedure, you will receive antibiotic and anti-inflammatory injections and rest in the recovery room for about 30 minutes.

8. You will be given post-procedure precautions and will schedule the next visit date.

9. Visit two days after the procedure to check the progress of the fat grafting area, and the fat-harvesting area will be disinfected.

Showering is possible starting two days after the procedure.

Visit on day 7 to check the progress and remove the stitches from the fat-harvesting area (navel).

If your home is too far away and it is difficult to visit, you do not necessarily have to come in.

10. If a second procedure is needed, you will visit again between 3 months and 1 year later for treatment.

Other Procedures

● Method using fillers (Juvederm, Artecoll, Teosyal, Evolence)

Mouth-area wrinkles may also be corrected with fillers or collagen, but the use of fillers is very limited.

Fillers can be tried for fine wrinkles like small cracks.

However, for deep sunken wrinkles, fillers are almost ineffective, and the injected filler may be pushed outward by the force of smiling, making the nasolabial folds even deeper.

In severe cases, there is also concern about various side effects such as inflammation.

Nasolabial folds are mostly deep sunken wrinkles.

Therefore, it is difficult to expect satisfactory results from filler treatment.

● Implant insertion method (silicone, Gore-Tex)

Implants are widely used in rhinoplasty, but they are not suitable for wrinkle plastic surgery.

A small implant is sometimes inserted through an incision inside the nose or mouth in what is commonly called a noble surgery, but the purpose is to correct slight hollowness rather than wrinkles.

However, it is not an appropriate surgical method for correcting long wrinkles, and various side effects may occur.

Because it is a foreign material, people may feel it when smiling, the contour may protrude, or the implant may shift due to the force of smiling.

In some cases, it may also cause problems such as inflammation, so inserting an implant into the nasolabial folds is not advisable.

In principle, implants should not be inserted in moving areas that are involved in facial expressions.

● Magic lift (a procedure that pulls the facial skin with threads)

There is also a procedure called a magic lift that pulls the face with threads, but it has almost no effect on overall facial wrinkles, as well as nasolabial folds.

There is a limit to pulling facial skin with the force of threads, and because it quickly recurs or causes contour deformities on the face, caution is needed.

● Inserting or injecting artificial dermis AlloDerm (SureDerm)

Correction is sometimes performed using artificial dermis such as AlloDerm, made by processing human dermal tissue, but precise treatment is difficult.

Also, liquid AlloDerm is absorbed to a considerable extent, making it difficult to predict the correction effect, and dissatisfaction with the procedure is common.

Solid AlloDerm is mainly used for tip-plasty to create a slightly upturned nose and is not an advisable procedure for correcting nasolabial folds.
