Guide to upper blepharoplasty

Indications for upper blepharoplasty

1. When the eyelids sag so much that you feel tired
2. When the eyelids droop and cover the pupils, causing vision problems
3. When the sagging eyelids make you look older

What is upper blepharoplasty?

As people age, the upper eyelids droop downward. This surgery reshapes the sagging upper eyelids to create a more attractive appearance, while also addressing tired eyes and vision problems. 

Upper blepharoplasty procedure (sagging upper eyelids)

1. Before surgery, the width of skin to be removed is determined.

At this stage, the amount of remaining skin is pinched and measured, and the design is drawn on the upper eyelid skin with special ink before anesthesia.

If the outer side of the eye is more sagging, the design is extended by about 5 to 10 mm beyond the outer canthus.

2. Local anesthesia is administered along the planned design line.

Since local anesthesia is performed under sedation, there is no pain at all during the anesthesia.

3. After anesthesia, an incision is made in the skin along the design line.

4. After the incision, the skin tissue inside the incision line is removed.

Sometimes the muscle layer is also removed at the same time.

5. If there is a lot of fat, orbital fat may also be removed.

After removing the fat, if a double eyelid is desired, a three-point suture may be used in about three places to create the crease.

6. Finally, the skin is sutured.

To minimize scarring, the stitches are placed closely at 1 mm intervals.

After suturing is complete, the area is disinfected and you can go home.

The stitches are removed after 4 to 5 days.

Postoperative care instructions

1. For the first 3 days after surgery, use the cold pack provided by the hospital for cold compresses, 2 to 3 hours a day, for only 3 days.

The cold compresses may be applied continuously for 2 hours or broken up into 20 to 30 minute sessions.

It is usually more convenient to do this before going to sleep.

2. Visit the clinic 1 to 2 days after surgery to disinfect the surgical site.

3. Visit the clinic on the 4th or 5th day after surgery to remove the stitches from the surgical site.

4. After the stitches are removed, you may wash your face and apply makeup, and you may also shower or use a sauna or jjimjilbang.

5. Swelling after surgery varies from person to person, and it usually subsides to some extent within 7 to 14 days.

6. Refrain from drinking alcohol and smoking for about a week after surgery.

7. Visit the clinic one month after surgery for a follow-up.

3. Preoperative precautions

4. On the day of surgery

• Arrive 30 minutes before the scheduled surgery time and receive an explanation of the precautions to follow during surgery.

• Change into the surgical gown and receive an IV injection for preoperative treatment.

In the case of local anesthesia, an intravenous injection is administered for painless anesthesia.

About 5 minutes after the IV injection, you enter a sedated state, and for patients undergoing double eyelid surgery or nose surgery, local anesthesia is administered to the surgical site while in that sedated state.

When you wake up from the sedated state after 5 minutes, the anesthesia will be completely finished.

The patient does not feel any pain during anesthesia.

• When the surgery is finished, you will receive antibiotic and anti-inflammatory intramuscular injections and rest in the recovery room for about 30 minutes.

For double eyelid or nose surgery, you will receive cold compresses with a cool pack.

• If you had surgery under general anesthesia, you will rest in the recovery room for about 1 to 2 hours, and once the dizziness from the anesthetic has completely disappeared, you may go home.

• When leaving, you will receive postoperative instructions from the surgeon in charge, schedule a follow-up visit, and receive a prescription to purchase medication at the pharmacy and take it.

5. Return on the scheduled date for a follow-up examination to check your postoperative progress.
