Facial contouring (cheeks, forehead, temples) fat grafting

Fat grafting is also called fat injection.

Because it is a delicate and precise fat grafting procedure, it is also called microfat grafting.

This involves harvesting your own fat with a syringe and then injecting it back into the desired area with a syringe to correct the facial contour.

The key to fat grafting lies in creating a natural, soft contour and achieving a high take rate.

Using three-dimensional volumetric fat grafting and two-dimensional flat fat grafting, the most ideal fat grafting is performed for each area.

Fat grafting procedure for facial contour correction

1. Fat is harvested from areas where your own fat is stored in large amounts, such as the lower abdomen, waist, or thighs.

• Because local anesthesia (painless sedation anesthesia) is used, there is no pain at all.

• A thin liposuction cannula is used to collect the fat into a syringe.

2. The harvested fat is placed in a centrifuge to remove impurities.

The purpose of centrifugation is to filter out impurities contained in the harvested fat, such as local anesthetic, serum, and red blood cells.

This process is performed to reduce side effects and increase the take rate of the fat.

3. After centrifugation, moisture and oil are removed in a second concentration process.

By removing moisture and oil, concentrated fat that is almost 100% pure is obtained.

The fat is measured into 1 cc syringes as needed.

Usually, about 10 to 20 cc of concentrated fat is needed for facial contour correction.

4. The fat is grafted into the forehead area, which has already been locally anesthetized.

Fat grafting is performed after the tunneling process is complete and space has been secured for the fat to be grafted.

At our clinic, we use micro-needles (micro grafting cannulas) developed in-house.

The micro-needle is a special grafting cannula used only at our clinic that enables ultra-precise grafting.

The principle of fat grafting is to distribute the grafted fat evenly throughout the target area so that it does not clump, while maximizing the take rate as much as possible.

Examples of fat grafting procedures

1. Forehead hollowness

In the case of the forehead, this is performed when there are sunken areas or a flat forehead.

Implants such as silicone or AlloDerm can also be inserted, but there is the fear of having to make an incision in the scalp, as well as the burden and side effects of inserting a foreign material. Fat grafting can simply correct the forehead contour while solving these issues.

2. Temple hollowness

For cases where temple hollowness makes the cheekbones look prominent or the face appear longer, fat grafting can replace the method of inserting silicone. It can naturally lift the sunken area and create a soft facial contour.

3. Thin cheeks

① A severely thin, wrinkled face

If the face looks thin, it can make you appear older and give a gaunt impression, so it is better for the cheeks to have some fullness.

There is no other way to add fullness to the cheeks besides fat grafting.

Commercial fillers such as collagen or Restylane cannot compare with fat grafting in terms of cost or effectiveness.

② A narrow inverted-triangle face

If the face is shaped like an inverted triangle, fat grafting can be used to make the face look fuller.

In such cases, fat is grafted from below the cheekbones down to the jawline to create volume.

With fat grafting, the face can change into a fuller-looking face from the front.

③ When the cheekbones appear prominent

If the cheeks are insufficient or the sides of the face are sunken, the cheekbones can appear relatively prominent.

Before considering cheekbone reduction surgery to push the cheekbones inward because they look prominent, you can first consider fat grafting to the cheeks below the cheekbones so that the cheekbones appear relatively less prominent.

If cheek fat grafting achieves enough corrective effect on the cheekbones, there is no need to undergo the difficult cheekbone surgery.

④ In the case of a square jaw

Even when the jaw angle is large and the face appears square, correction can be achieved with fat grafting.

By grafting fat into the hollow area above the square jaw, a visual correction effect can be achieved without jawbone surgery.

This is because the cheeks become fuller, making the square jaw area appear relatively less prominent.

If you are considering jawbone surgery for a square jaw, it is a good idea to first consider cheek fat grafting. If cheek fat grafting provides enough corrective effect for the square jaw, there is no need to undergo the difficult jawbone surgery.

⑤ A sunken line on the side of the cheeks

If the side of the face is sunken and indented like a field furrow, the hollow area can be naturally filled to create a soft contour.

The sides of the cheeks are tightly attached to the bone and tissue, making it a very difficult area in which to create natural volume.

The most difficult part of cheek fat grafting, requiring the most skill and know-how, is filling in the side hollows.

⑥ In the case of facial asymmetry

Fat grafting can also produce good results in cases of facial asymmetry caused by a sunken side of the face or abnormal development of the jawbone or cheekbones.

By grafting fat only on the sunken side or applying different amounts to each side, the facial asymmetry can be corrected to a similar degree.

⑦ Thin cheeks in men

Even for men, if the face is angular or sunken, it can make them look older or give too strong of an impression.

For men as well, it is better to have a softer impression rather than a harsh and strong one.

Cheek fat grafting is one of the most effective procedures for creating a softer impression.

⑧ When contour irregularities occur after fat grafting at another clinic

People often tend to think of fat grafting as something simple, but fat grafting is by no means a simple or easy procedure.

Some people think that getting fat grafting at any clinic will produce good results, but fat grafting is a procedure that is more demanding than any other plastic surgery, and it requires extensive experience, know-how, and highly precise manual skill along with a very complex process.

There are many clinics that perform fat grafting, but not many that do it properly.

It is easy to perform, but difficult to create the proper result. That is fat grafting.

If you undergo the procedure at a clinic that imitates it carelessly, it is easy to end up disappointed.

In principle, fat grafting can only produce good results when it is performed precisely.

It is best not to undergo the procedure by someone with little experience or by a non-specialist who does not have board certification in plastic surgery.

⑨ When over-injection after fat grafting at another clinic makes the cheeks look bulging

After fat grafting, there are cases where the cheeks appear saggy or bulging due to over-injection or excessive take.

In such cases, correction is performed by suction with a microcannula.

As a rule, the timing of suction should be decided after waiting 3 months for the results to appear.

However, if the over-injection is excessive and the shape looks too unnatural, suction may need to be performed earlier.

What are the advantages of correcting facial contour with fat grafting?

1. The procedure is simple.

2. Because it does not use foreign materials such as commercial fillers like silicone, Gore-Tex, or AlloDerm, there is no need to worry about side effects.

3. Because the procedure is performed without cutting the skin, there is no need to worry about scars.

4. Because it uses your own tissue, the shape looks natural.

5. There is almost no swelling or bruising.

6. You can return to daily life immediately after the procedure.

7. Because the fat tissue takes hold, the effect can be maintained for a long time.

What should you be careful about when correcting facial contour with fat grafting?

1. Fat grafting requires skilled technique and experience.

The results can vary greatly depending on the skill of the practitioner, so careless fat grafting can actually make the appearance worse, and caution is needed.

2. After facial fat grafting, no bandages or taping are applied.

Other than swelling and slight bruising, you can apply makeup and return to daily life immediately, but for about 1 to 2 weeks, the swelling may make things look slightly awkward.

3. After fat grafting, avoid running or vigorous exercise for about a month, and refrain from drinking alcohol and smoking for about a week.

Except when fat was harvested from the legs, walking, weight training, and stretching can be resumed after about a week.

4. Since cheek fat grafting takes hold over the course of 3 months, avoiding rubbing or pressing the cheeks hard while the fat is taking hold helps improve fat survival.

Number of fat grafting procedures

Cheek fat grafting is an area with a high take rate, so usually one session is enough to achieve a satisfactory effect, and in some cases, two sessions are performed.

The interval between procedures is usually from 3 months up to within 10 months, and the procedure can be done using the remaining frozen fat.

For a repeat procedure, the fat that was collected and stored during the first fat grafting can be thawed and used.

However, if a second procedure is performed after 10 months have passed, the fat must be harvested again.

Long-term effect of fat grafting

Some people say fat grafting is ineffective because much of it is absorbed, but if fat grafting is performed correctly, a satisfactory effect can be achieved.

Fat grafting can produce good results when performed based on skilled technique and experience.

Only when fat grafting is performed properly can a satisfactory result be obtained, and once the transplanted fat takes hold, it is no longer absorbed and remains as your own fat tissue for life, making it one of the safest and most effective methods.

On average, receiving fat grafting 1 to 2 times can provide a permanent and satisfactory facial contour correction effect.

Side effects of fat grafting

Because fat grafting involves transplanting your own tissue, it is true that side effects do not occur in itself.

However, unsatisfactory results or side effects can occur due to inexperience on the part of the practitioner.

The series of processes from fat harvesting to purification and grafting must be carried out very delicately, carefully, and in principle to achieve good results.

Careless fat grafting can lead to results worse than not having the procedure at all, so in principle, it is best not to undergo the procedure with someone who has little experience or who is not a plastic surgery specialist.
