What is under-eye fat repositioning?

Instead of removing the orbital fat under the eyes, it refers to a method of moving the fat downward to a protruding area and spreading it out.

This is a method intended to correct both the protruding area and the sunken area at the same time.

Fat removal can address the protruding area, but it cannot solve the sunken area, so this method was designed to make up for that drawback.

Candidates for under-eye fat repositioning

​This is for cases where protruding under-eye orbital fat is accompanied by sunken wrinkles.

Problems with under-eye fat repositioning

The problem with under-eye fat repositioning is that it is difficult to achieve the desired result properly.

Because the surgery is performed through the conjunctiva, the field of view is limited and the operation is not carried out properly, and there are not many patients who are ideal candidates for achieving good results through fat repositioning.

As a simple example, if there is land with a pond-like depression and the goal is to make the whole area flat by taking soil from the hill and filling the pond, then when the hill is not that large, even after cutting away all of the hill’s soil and filling the pond, the hill may end up becoming a hollowed-out basin instead.

This is the strange phenomenon in which, when fat is forcibly pulled and repositioned while the amount of fat is not large, the protruding area instead becomes sunken.

In another case, the pond is larger than the hill, so even after using all the soil from the hill to fill the pond, the depth of the pond is only slightly reduced and it still remains a pond.

This refers to a case where a small amount of fat has been pulled and repositioned, but the sunken area is not corrected well and remains sunken.

In yet another case, the hill is large, so even after filling the pond completely, the hill only becomes slightly smaller and still remains a hill.

This refers to a case where there is a lot of protruding fat, so even after repositioning, fat remains and the under-eye protrusion is still present.

Four sources of dissatisfaction with under-eye fat repositioning

1. When the amount of fat is insufficient, the protruding area where the fat was located becomes hollowed out after fat repositioning
2. When the amount of fat is large, the original protruding area still appears protruded even after fat repositioning
3. After fat repositioning, the original sunken area instead appears protruded
4. When the amount of fat is small, the original sunken area is not properly filled even after fat repositioning, so it still appears sunken and shadowed

When performing under-eye fat repositioning, surgery should be done only after carefully distinguishing the appropriate shape and indications to obtain a satisfying result, and fat repositioning in cases that are not indicated may become an ambiguous surgery that is neither one thing nor the other.

Also, because more tissue must be dissected than with fat removal, swelling and bruising are much more severe.

If the indications are not suitable, fat removal can provide a more definite result than fat repositioning, and if needed after fat removal, micro fat grafting can additionally be performed to fill in the sunken area.​

If under-eye fat repositioning is applied carelessly to patients who do not have much orbital fat, in most cases the actual result is unsatisfactory or awkward.

Also, little improvement in under-eye color can be expected through fat repositioning, and it can only be performed for contour correction purposes.

However, cases in which the desired flat under-eye area is created are extremely rare.

There are many cases where another clinic recommends fat repositioning even for eyes with little or no under-eye fat protrusion, and this should be considered carefully.

After under-eye fat repositioning, rather than the under-eye area becoming flatter, there are cases where the under-eye area becomes hollow or protrudes, leading people to contact our clinic for correction, and in cases of hollowness, correction with micro fat grafting is possible again.

Postoperative care instructions after under-eye fat repositioning surgery

1. After fat repositioning, use the cold compress pack provided by the hospital for about 3 days.
2. On the day of surgery, you should sleep with your head elevated about 30 degrees to reduce bruising and swelling.
3. To reduce swelling and bruising, try not to lie down on the day of surgery and stay seated as much as possible.
4. Face washing and makeup are allowed starting the next day.
5. Contact lenses may be worn starting the day after surgery.
6. Use the eye drops prescribed by the hospital for about one week.
7. Since the conjunctiva is sutured with dissolvable thread, there is no need for stitch removal.
8. Swelling and bruising are usually mild and will mostly subside within about a week.

If the under-eye area is not hollow, but the under-eye orbital fat is protruding and making it look puffy, then simply removing the under-eye fat can help alleviate the symptoms.

However, if there is hollowness along with protrusion under the eyes, it can be improved with under-eye fat repositioning surgery, which moves the protruding fat to the sunken area.

This is not about removing under-eye fat, but about moving fat to the area where volume is needed.
