Fat grafting for facial contour correction

Fat grafting is also called fat injection.

Because it is performed with delicate and precise fat grafting, it is also referred to as micro fat grafting.

Using a syringe, fat is harvested from your own body and then injected into the desired area with a syringe to correct the facial contour.

The key to fat grafting lies in expressing a natural, smooth contour and achieving a high graft survival rate.

Using 3D three-dimensional fat grafting and 2D flat fat grafting, we perform the most ideal fat grafting for each area.

Contour plastic surgery for sunken areas other than bone surgery such as the cheekbones and jawbone can be corrected with fat grafting.

The procedure is simple, there are no side effects, and any area can be naturally corrected to the desired degree, so fat grafting is one of the effective facial contour plastic surgery procedures.

3D Fat Grafting

3D three-dimensional fat grafting means evenly grafting small amounts in multiple places and multiple layers in order to increase the fat graft survival rate.

The advantages of 3D three-dimensional fat grafting are that it offers a high fat graft survival rate and allows a uniform, smooth contour to be achieved.

If a large amount of fat is grafted in one area, the shape becomes awkward and the fat may necrotize or be absorbed, so good results cannot be obtained.

2D Flat Fat Grafting

2D flat fat grafting means grafting into only one layer, so a large amount of fat cannot be grafted, and because the fat is grafted into only one layer, there is a problem of a high absorption rate.

However, there are some areas where 2D flat fat grafting must be performed.

For example, in the upper eyelid, grafting must be done only into the submuscular orbital fat layer, so 2D fat grafting is performed.

Results when a large amount of fat is grafted into one area

When too much fat is grafted into one area, blood vessels cannot grow all the way into the center of the graft, and the fat tissue becomes necrotic.

In other words, because it is absorbed and disappears, the effect can be reduced even if a large amount of fat is injected into one area.

Therefore, the most effective method of fat grafting is to graft small amounts evenly across multiple layers and multiple areas.

Advantages of Fat Grafting

1. Because it is an autologous tissue graft, there are almost no side effects.

2. Because no incision is made on the face like in surgery, there is no scarring, and the procedure is simple.

3. Any sunken area of the face can be naturally corrected.

4. Since no bandage or tape is applied to the face after the procedure, you can return to daily life immediately.

5. If you do not like the shape, it can also be removed by suction.

Other Methods Using Fillers

There are cases where contour correction is performed not with fat grafting but with filler materials (Aquamid, Restylane, Artecoll, etc.).

These materials were developed for removing fine wrinkles and correcting small depressed scars.

They have various problems, including strain during the procedure, side effects, and reduced treatment effectiveness.

They are also more expensive than fat grafting.

Many people experience side effects after filler is injected into the face.

Fillers must be used only after carefully selecting the proper indications to avoid side effects.

Fillers are intended only for very fine wrinkles around the eyes, between the eyebrows, the forehead, or small depressed scars from pox.

Col-fat Fat Grafting

Micro fat grafting takes hold after transplantation as capillaries form around the graft.

The graft survival process takes at least three months.

During this time, it may dissolve because it has not become vascularized, or it may be crushed and disappear due to pressure from surrounding tissues.

Therefore, fat volume alone may not always provide sufficient volume increase.

To compensate for this, collagen is generated on its own to increase the fat graft survival rate while creating youthful, elastic skin and contour.

This is Col-fat fat grafting.

Features of Col-fat Fat Grafting

1. It supplies serum, which is the nutritional source in the early stage of fat graft survival.

2. It increases the graft survival rate by minimizing pressure from surrounding tissues.

3. Because the fat is grafted evenly, it does not create unevenness.

4. Due to the self-generation of collagen, the contour correction effect is greater.

5. The tissue and skin gain elasticity, providing wrinkle correction and a rejuvenating effect. 

Col-fat Fat Grafting Procedure

The procedure is performed using micro needles independently developed by our clinic.

The core of the micro needle is a precision-machined tip on the Furubu, designed to induce collagen production during subcutaneous tunneling.

It also makes possible ultra-precise fat grafting with the Col-fat technique.

Because it has the advantage of allowing precise treatment in 1–2 mm increments even in areas requiring great delicacy, such as very fine wrinkles or the under-eye and upper-eye areas, it can be performed without problems even in the most difficult areas.

For under-eye dark circle correction, treatment using micro Furubu makes it possible to correct dark circles effectively with almost no unevenness.

Through tunneling using micro Furubu and micro fat grafting using micro needles, even the most difficult under-eye procedures can be performed effectively.

Effects of Col-fat Fat Grafting

At our clinic, we perform multilayer (layer-by-layer within the skin) fat grafting based on more than 20 years of rich clinical experience and know-how.

The key to fat grafting is enabling fat cells, as living cells, to have a high graft survival rate.

To increase the graft survival rate of fat, we do not inject a large amount in one place; instead, we graft evenly and uniformly across multiple layers.

Between the skin and bone there are multiple tissues such as the fat layer and muscle layer, and the fat must be injected in a multilayer structure so that it does not overlap between areas such as between the periosteum above the bone and between the fascia above and below the muscle.

Multilayer fat grafting lasts 3 to 4 times longer than conventional methods and also has a higher fat survival rate.

Our fat grafting procedure also includes a process called "tunneling," which allows the graft to contact as much of the abundant surrounding blood vessels as possible so that it can continuously receive nutrients.

Tunneling means creating a space for the fat just under the skin using micro needles. In other words, since a space is created like drilling a tunnel and then the fat is injected, the fat does not sag in the direction of gravity or become bumpy even after all of it has been injected.

It also allows customized design using a delicate injection method.

After fat grafting, we provide careful follow-up observation until the fat has fully taken hold by thoroughly managing postoperative care.

During the follow-up period, if necessary, we try to minimize dissatisfaction through swelling-reducing injections, shape correction, and molding.
