Our clinic’s features

We do not use general anesthesia; instead, we perform painless sedation anesthesia.

All fat grafting procedures are performed under local anesthesia only, regardless of the area being treated.

With local anesthesia using painless sedation, you can undergo the procedure without any pain during the anesthesia process or after the anesthesia is finished.

The fat graft survival rate is high.

The success of fat grafting depends on the survival rate.

To increase the survival rate, the following are necessary.

◇ Live fat cells must be harvested.

◇ To increase the purity of the fat, centrifugation purification and removal of unnecessary oil are performed to maximize fat purity.

◇ Before fat grafting, subcutaneous tunneling is performed in the grafted area to secure space for the fat to be transplanted.

◇ A micro fat grafting cannula (18–19 gauge) is used for precise grafting in each area.

Appropriate and various fat harvest sites are selected for collection.

All fat harvesting is performed under local anesthesia only, regardless of the area.

With local anesthesia using painless sedation, fat can be harvested without any pain during the anesthesia process or after the anesthesia is finished.

The most commonly used area for fat harvesting is the lower abdomen. The lower abdomen is the first choice because there is less pain after the procedure, less swelling and bruising, and it is a comfortable area for daily life.

If there is not enough fat in the lower abdomen or if the patient prefers another area, fat is harvested from other areas as well. The next most commonly selected area is the inner thigh, followed by the outer thigh.

If there is not enough fat in the thighs, it is harvested from the lower buttocks or the flanks.

We maximize the fat graft survival rate through a two-step purification process for the harvested fat.

The harvested fat undergoes a two-step purification process to remove impurities.

The first step is 5 minutes of centrifugation, and the second step removes oil and moisture.

Through the second-stage concentration process, we obtain nearly 100% concentrated fat, maximizing the effect of the fat graft while raising the survival rate as much as possible.

We secure space for fat grafting through subcutaneous tunneling.

Subcutaneous tunneling is an essential process for securing space in advance in the area where fat will be grafted, in order to achieve a high survival rate and even grafting.

Using a fine probe, the tissue under the skin is moved repeatedly in a fan-like pattern to create space for the fat to be grafted.

A smooth contour can be achieved.

At our clinic, we use our own newly developed microneedle (micro grafting cannula).

The microneedle is a special grafting cannula used only at our clinic, enabling ultra-precise grafting.

It would be problematic if uneven contours occurred after fat grafting.

To prevent this, we inject small amounts evenly into each area using a fine fat grafting cannula, increasing the survival rate and allowing the graft to take evenly so that a smooth contour is formed.

For precise fat grafting, we use a micro fat grafting cannula.

In the past, methods that used fat injection machines forced fat under the skin with high pressure, which caused several problems.

The absorption rate of fat was high, grafting was not consistent, the contour became uneven, and grafting was difficult in delicate areas such as under the eyes.

To solve these problems, our clinic developed its own microneedle (micro grafting cannula), making ultra-precise grafting possible.

Fat grafting can be performed on the upper eyelids, lower eyelids, forehead wrinkles, frown lines, nasolabial folds, and the bridge of the nose, making it possible in almost all cases except fine wrinkles.

Finishing touch through molding immediately after fat grafting

Immediately after fat grafting, delicate molding work is performed so the grafted fat spreads evenly to create a smooth shape.

Molding is the final stage of fat grafting and one of the most important steps.

We simplify second procedures by sterile storage with rapid freezing of the remaining fat.

If a second procedure is performed 3 months after fat grafting, fat is not harvested again; instead, the fat left over from the first procedure is used.

The remaining fat after the first procedure is rapidly frozen, sterilized, and stored.

For the second procedure, the stored fat is thawed so the treatment can be performed more easily.

Thorough aftercare system

After fat grafting, we provide thorough aftercare and carefully monitor the progress until the fat has fully taken.

During follow-up, if necessary, we strive to minimize dissatisfaction by using swelling-reducing injections, shape correction, and molding.

Fat grafting process

1. The area to be grafted is designed with marking ink.

2. A blood vessel injection is given for painless sedation anesthesia.

Sedation anesthesia means sleeping for the 5 minutes during which local anesthesia is administered, and local anesthesia is given while the patient is asleep.

After the anesthesia is finished, you wake up from sleep, and because the area is then anesthetized, the procedure is performed without any pain.

3. While sedation anesthesia is being performed, local anesthesia is administered to the fat-harvesting area and the grafting area.

Because you are under sedation anesthesia, there is no pain at all during the anesthesia.

4. Fat is harvested from areas where a large amount of your own fat is stored, such as the lower abdomen, thighs, or flanks.

A thin liposuction cannula is inserted to collect the required amount of fat into a syringe.

Usually, as little as 20 cc is collected, and in cases where many areas are to be grafted, as much as 150 cc may be harvested.

5. After collecting the required amount of fat, it is temporarily stored upright at room temperature.

After a few minutes, we wait for impurities to settle to the bottom.

Once all impurities have settled, the impurities at the bottom are discarded, and the remaining fat undergoes centrifugation, the first step of fat concentration.

6. This is the second-stage concentration process that removes oil and moisture from the fat.

In the second stage, the fat that has finished centrifugation is placed on sterile gauze and wrapped so that unnecessary oil and moisture are removed naturally through absorption.

Once the second purification process is complete, nearly 100% concentrated fat is obtained.

The survival rate of fat depends on how properly the fat is concentrated.

The more it is concentrated, the more fat cells can be grafted into the same area.

Concentration by centrifugation alone is not enough, so an additional step to remove fat oil is absolutely necessary.

7. The fat is transferred into a 1 cc syringe and tested before injection.

8. Before injecting the fat, subcutaneous tunneling is performed in advance in the grafting area.

This process creates space so the grafted fat can take smoothly under the skin.

It is also a step to ensure the fat is grafted evenly and forms a smooth contour.

9. The concentrated fat is grafted into the area where the subcutaneous tunneling has been completed.

We use our own newly developed microneedle (micro grafting cannula).

The microneedle is a special grafting cannula used only at our clinic, enabling ultra-precise grafting.

The grafting is performed evenly and in small amounts across multiple layers so that the fat survival rate can be maximized.

10. After fat grafting is complete, molding is performed to create a smooth contour.

Molding gently presses or pushes the grafted area so that the fat spreads evenly.

11. After fat grafting is complete, any remaining fat is rapidly frozen, sterilized, and stored.

The fat is usually stored for up to 10 months, and when a second procedure is needed, the fat is thawed so a second fat grafting procedure can be performed easily.

The fat is first thawed in the refrigerator, then thawed again at room temperature a few minutes before fat grafting.

12. After the procedure is finished, you can apply makeup immediately, go home, and resume daily life.

The course after fat grafting

◇ Fat grafting is not always finished in one session.

Because about 50% of the grafted fat takes on average, the shape may look a little swollen at first and may look good, but over time it is gradually absorbed.

Depending on the survival rate, some cases are completed with one procedure, and in some cases a second procedure is needed.

◇ Three months after the first fat grafting procedure, you visit the plastic surgery clinic again for follow-up.

◇ If a second fat grafting procedure is deemed necessary, the frozen fat stored from the first procedure is thawed and injected again under local anesthesia.

Compared with the first procedure, the fat-harvesting step is omitted, so the procedure time is much shorter, and the cost is about half of the first fat grafting procedure.

The number of fat grafting sessions varies depending on the individual and the area, but in general, 1 to 2 sessions can produce satisfactory results, making it a safe procedure method.
