Correction of asymmetric fold line

There are various causes of an asymmetric fold line.

There are also various methods for correction.

Causes of an asymmetric fold line

② When ptosis is present in one eye

④ When the skin on one side is sagging

⑤ When the orbit fat is insufficient and the eyelid becomes sunken, causing the line to be pulled upward

⑥ When the line is pulled upward due to scar adhesion

Methods for correcting an asymmetric fold line

● When the height of the double eyelid crease differs after buried suture method

If the surgery was done with the buried suture method, there is a high chance that it will come loose again even if it is redone with the buried suture method.

A new line is made at the desired position using the partial incision method.

The existing double eyelid line naturally flattens out and disappears as the new line is created.

However, if the existing line is too distinct, it may not be corrected with the partial incision method, so a full incision method may also be necessary.

● When the height of the double eyelid crease differs after full incision method

1. When correcting a small line into a larger line to match the larger line

If one side’s line is lower and the smaller line needs to be made larger, a new double eyelid is created above the existing double eyelid line.

In general, this is corrected with the partial incision method, and if the existing scar line is dark, it is corrected with the full incision method.

2. When correcting a large line into a smaller line to match the smaller line

To reduce a large line created by the full incision method, the full incision method is performed again.

The existing scar line is excised and removed, and a new double eyelid is created to match the other side.

To reduce the large line, the tissue is dissected and completely repositioned.

● Correction method when ptosis is present on one side

A procedure is performed to make the eye with ptosis open wider.

Using the full incision method, the levator palpebrae superioris muscle (eyelid lifting muscle) is pulled and attached to the tarsal plate through levator shortening.

● When one double eyelid line has loosened

If one side’s line has loosened, the loosened side’s double eyelid line is corrected using the partial incision method or the full incision method.

If the skin is very thick or the scar is not severe, the principle is to first correct it with the partial incision method; if it still loosens, correction is done with the full incision method.

● When the skin on one side is sagging

If the heights of the double eyelid lines are the same but asymmetry occurs due to sagging skin, correction is performed by removing the sagging skin on the affected side using the full incision method.

● When both sides are asymmetric and both lines look awkward

If the left and right sides are asymmetric and both lines look unattractive and awkward, both sides need revision surgery.

The lines are designed at the same height, the existing scar lines are removed, and complete repositioning is performed to create new lines.

● When one eyelid becomes sunken and the line is pulled upward, causing asymmetry

When the orbit fat is insufficient and the eyelid becomes sunken, the line is pulled upward and appears larger.

In such cases, it can be simply corrected with microfat grafting without revision surgery.

Microfat grafting is a simple procedure that injects fat with a syringe.

Eyelash eversion

This refers to a case where the skin below the incision line is pulled upward tightly,

causing the mucosa beneath the eyelashes to become visible.

If the mucosa is excessively exposed,

it can give a poor impression,

so correction is recommended.

Causes of eyelash eversion

○ Excessive skin removal

○ During suturing, the tissue was pulled too far upward and sutured under excessive tension

○ There was not enough excess skin, so the skin below the incision line was pulled upward

○ Severe adhesion of the scar line

Surgical method for eyelash eversion

Correction is possible only through the full incision method.

If surgery is delayed, the appearance of eyelash eversion may become fixed, making correction impossible.

In cases of eyelash eversion, surgery should be performed as soon as possible to achieve a corrective effect,

but if correction is done months or years later, there are cases where it cannot be properly corrected.

In other words, surgery should be performed before the shape becomes fixed in order for any correction to be possible.

The scar line is incised, the skin below the incision line is lifted,

and the tissue is dissected downward to release the tension of the skin that had been pulled tightly upward.

Usually, the dissection is carried out to near the eyelashes, and in some cases, part of the orbicularis oculi muscle is also removed.

After adjusting the loosened skin to an appropriate height,

it is fixed with suture fixation so that it does not move, and the skin is then sutured.
