Nose fat grafting

In what cases can nose surgery be performed using fat grafting?

The greatest advantage of fat grafting is that

it is simple to perform

and has almost no side effects.

If you feel burdened by surgery using implants

or need to return to daily life immediately after the procedure,

nose surgery with microfat grafting is possible.

Fat grafting can be applied when the nasal bridge is low and you only want to raise it slightly.

▷ When the nasal tip is fine, but only the nasal bridge is low

▷ When the glabella (between the eyebrows) is low

▷ When the nose appears slightly crooked

▷ When there is a depressed scar on the nasal bridge

▷ When there is depression on the nasal bridge (saddle nose) or a bend

▷ When silicone or a similar implant has been inserted and the border line is visible, and you want to soften it

There are methods to raise the nose with implants such as silicone or Gore-Tex, but when raising the nose with autologous fat grafting, the advantages include being able to return to daily life right away, having little swelling and bruising, and causing less side effects and rejection because the procedure uses your own tissue.

When raising the nose with fat, the indication should be carefully determined, and the procedure should only be performed in cases where it can produce results.

There are also procedures using fillers (Artecoll, Restylane, Aquamid), but fillers are not products that should be used to raise the nose.

Fillers should only be used for purposes such as correcting fine lines around the eyes, fine forehead wrinkles, or small depressed scars.

Injecting filler into the nose is completely inappropriate for its intended use, and because of concerns about side effects such as skin necrosis or inflammation, the procedure must be approached with caution.

For natural augmentation, fat grafting can be considered one of the good methods.

Method of fat grafting for nose surgery

Usually, fat is harvested from the lower abdomen. The reason the lower abdomen is a good donor site is that fat can be harvested easily, and swelling and bruising are minimal, allowing for a quick return to daily life and recovery.

If there is not much fat in the lower abdomen, fat is sometimes harvested from the thighs.

Usually, about 40 to 60 cc of fat is harvested in preparation for a second procedure.

Immediately harvested fat is usually about 80% impurities.

The largest component of the impurities is local anesthetic, followed by oily components trapped in the fat cells.

In addition, there is serum and a small amount of blood mixed in, so all impurities except the fat cells must be removed.

Removing impurities cannot be done by centrifugation alone; in addition to centrifugation, a process to remove oil is added.

The refined fat is transferred into a 1 cc syringe and the microfat grafting cannula is attached before the procedure begins.

We use a micro-needle (micro grafting cannula) developed in-house at our clinic.

The micro-needle is a special grafting cannula used only by our clinic, enabling ultra-precise grafting.

The procedure is performed as the needle is inserted from the nasal tip, and usually about 1 to 2 cc of fat is grafted.

Examples of fat grafting for nose surgery

Cases suitable for nasal fat grafting

Nasal fat grafting is not a procedure that can be applied to everyone.

First, the thickness of your skin must be to some extent sufficient,

and the nasal skin must also have a certain degree of flexibility for the fat to survive.

When raising the nasal bridge with nose fat grafting, the amount is usually about 3 mm,

and the procedure should be planned with a second session in mind.

On the other hand, in cases where the skin is thin, firm, and lacks flexibility,

there are cases where even if the procedure is performed, all of the fat is absorbed and there is little to no effect.

Whether the procedure will be effective can be confirmed through an in-person examination.

Cases in which nasal fat grafting is difficult

1. When additional augmentation is desired while silicone or another implant is already inserted Fat cannot survive on top of silicone. However, fat grafting is possible to soften the boundary line at the edge of the silicone.
2. Within 3 months after removing silicone or another implant If the capsule surrounding the silicone does not dissolve, the grafted fat will all be absorbed. Therefore, after implant removal, at least 3 to 6 months must pass before grafting is possible.
3. When the skin of the nasal bridge is thin and firm For fat to survive, blood circulation must be good and the pressure from the skin must be low. Thin, firm skin has poor circulation and strong pressure, which compresses the fat and causes it to dissolve.

What are the advantages of nose surgery (augmentation) with fat grafting?

① The procedure is simple.

② Because no foreign material such as silicone is used, there is no need to worry about side effects.

③ Because the skin is not incised, the burden of surgery is low.

④ Unlike surgery, there is no need to fix the area with tape for several days immediately after the operation.

⑤ There is little swelling or bruising.

⑥ Daily life is possible right after the procedure.

⑦ Because your own tissue is used, natural augmentation is possible.

Effects and recovery course immediately after nose surgery with fat grafting

Immediately after the procedure, the nasal bridge becomes more elevated than expected, then gradually settles down.

In most cases, because there is little swelling or bruising, most people can resume daily life right away.

As it continues to settle for 3 to 6 months and becomes fully established with no further changes, a second procedure is performed to raise it further.

What are the precautions when undergoing nose surgery with fat grafting?

① Because some of the grafted fat is absorbed after transplantation, one procedure may not be enough; in such cases, a repeat procedure should be performed 2 to 3 months later.
② The nasal tip has thick skin, so it is difficult to create shape with fat grafting.
③ When the nasal bridge is severely low, there is a limit to how much the nose can be raised with fat grafting.

Number of nose fat grafting sessions

On average, nose fat grafting can produce satisfactory results after two sessions.

In some cases, satisfactory results are achieved with just one procedure,

and this depends on differences in the individual’s fat survival rate.

If you return for the next session at least 3 months to within 10 months,

then at the time of the repeat procedure, the fat that was previously harvested will be thawed and used.

Lasting effects of nose fat grafting

Some people say that fat grafting is ineffective because much of the fat is absorbed,

but this applies to cases performed at clinics with somewhat limited experience.

If fat grafting is performed accurately, satisfactory results can be obtained,

and once the grafted fat survives,

it is no longer absorbed and remains as your own fat tissue for life, making it one of the safest and most effective methods.

On average, after 1 to 2 fat grafting sessions, a permanent and satisfactory augmentation effect can be achieved.

Effect of nose fat grafting for correcting a low nose

Nose fat grafting is a procedure intended to naturally augment only the nasal bridge.

It is often insufficient for raising the nasal tip as well.

When raising the nasal bridge with nose fat grafting, the amount is usually about 3 mm,

and this varies depending on the individual’s skin thickness and flexibility.

The average survival rate of nose fat is around 30 to 50%,

and in many cases, a second procedure is performed after 3 months.

The reason the survival rate is lower than in other areas is that

the skin of the nose continuously presses downward on the fat, compressing it.

Nose fat grafting is mainly applied in cases where only a slight, natural augmentation is desired or when the area between the eyes appears low.

In cases where the nasal bones have collapsed from a strong impact or when a nasal bone fracture was not properly treated, the nose may become crooked.

Among the impurities in harvested fat, the component with the highest proportion is local anesthetic, followed by oil.

This means dividing the fat into multiple layers and grafting small amounts evenly in each area.

We use a microfat grafting cannula for precise fat grafting.

In the past, methods using fat injection machines caused various problems because fat was forced under the skin at high pressure.

The absorption rate of the fat was high, survival was inconsistent, the contour became uneven, and it was difficult to graft in delicate areas such as under the eyes.

To solve these problems, the fat is transferred again into a thin 1 cc syringe during fat grafting and then grafted into the desired area.

Fat grafting can be performed in the upper eyelids, lower eyelids, forehead wrinkles, glabellar wrinkles, nasolabial folds, and the nasal bridge, making fat grafting possible in almost all cases except fine wrinkles.

Through molding immediately after fat grafting, a uniform and soft facial contour is created.

Right after fat grafting, delicate molding work is performed so that the grafted fat spreads evenly and a soft shape is created.

Molding is the final stage of fat grafting and one of the most important steps.

Rapid freezing and aseptic storage of remaining fat simplifies the second procedure.

When a second procedure is performed 3 months after fat grafting, the remaining fat from the first procedure is used instead of harvesting fat again.

The remaining fat after the first procedure is rapidly frozen and stored under aseptic conditions.

At the time of the second procedure, the stored fat is thawed, allowing the treatment to be performed more conveniently.

The storage period for fat is up to 1 year, and if you visit within 1 year, the procedure will be performed with the stored fat.
