How much scar remains depends on the surgeon's skill and the individual's constitutional factors.

In particular, scars tend to occur more often in people with thick skin and orbicularis oculi muscle.

Once a scar occurs, there is no way to remove it,

but in cases of severe scarring, scar revision surgery can be performed to make the scar less noticeable.

Therefore, at the time of the first surgery, it is important to remove the appropriate amount of skin and perform precise suturing as much as possible

so that the scar remains minimal.

The eyelids should appear thin.

The causes of eyelids appearing puffy include thick skin or orbicularis oculi muscle,

or a large amount of orbital fat or preaponeurotic fat.

It is better to resolve these issues while performing double eyelid surgery whenever possible.

The most common cause is excess orbital fat,

and this can be addressed by removing the orbital fat during double eyelid surgery.

Also, when the orbicularis oculi muscle layer beneath the skin is thick, part of the muscle can be removed.

The orbicularis oculi muscle below the incision line can be partially removed,

whereas the orbicularis oculi muscle above the incision line is difficult to remove.

If there is an epicanthal fold, it is better to remove it.

In some Asian people, there is occasionally an epicanthal fold (medial canthal fold),

which can cover the inner corner skin of the eye and create a cramped appearance.

If this is removed, the horizontal width of the eyes increases and they look brighter, and the double eyelid line becomes clearer all the way to the inner side.

However, surgery must be performed carefully with consideration of the overall eye shape to avoid unsatisfactory results.

If the eyes do not appear far apart and there is little to no epicanthal fold at the inner corners, then performing epicanthoplasty may instead make the eyes look too close together or give the eyes a harsher appearance.

Therefore, after an accurate physical examination, it should only be performed when the indications are met.

If ptosis is present, it should be corrected at the same time.

Ptosis refers to a condition in which the upper eyelid droops because the function of the levator palpebrae superioris, which lifts the eyelid, is weakened, causing the upper lid to cover the pupil and making the eyes appear sleepy. If this is overlooked during surgery, the double eyelid line may not look attractive.

Before double eyelid surgery, the eyes should appear large and clear for the double eyelids to look beautiful as well.

Depending on the degree of ptosis, if ptosis correction is judged to be necessary, it is better to correct it at the same time using the incision method.
