1. Muscle reduction surgery - Non-incisional ultrasound-guided selective nerve block

This method blocks only the motor nerves going to the gastrocnemius muscle, causing the muscle to naturally atrophy without damaging the muscle.

2. Subcutaneous fat bulges - Calf liposuction

This method reduces volume by suctioning the fat between the calf muscle and the skin.

Selective nerve block works by blocking the gastrocnemius nerve branching from the tibial nerve so that the gastrocnemius loses its mobility.

Since a nerve-blocked muscle cannot contract or relax, it loses mobility, and the method uses the principle that the muscle atrophies over time.

It blocks the nerve by using the principle that unused muscles atrophy.

Ultrasound-guided selective nerve block for muscle reduction

With the basic nerve detector, finding nerves that cannot be seen is very difficult and time-consuming.

Also, if the nerve deviates from its normal path, finding it becomes very difficult, and in many cases the procedure is not performed properly.

If the nerve is not found correctly, the treatment effect decreases, side effects may occur, and the recurrence rate is high.

To overcome these drawbacks, ultrasound-guided selective nerve block adds ultrasound imaging to the basic equipment, allowing nerves to be searched quickly and accurately and multiple nerve branches to be effectively blocked in a short time.

It is an advanced procedure that improves treatment effect, has fewer side effects, and significantly lowers the recurrence rate.

⑤ Because the procedure is performed through accurate nerve identification, the recurrence rate is significantly lower.

Problems with selective nerve block performed at other hospitals

① It is technically difficult because invisible nerve branches must be located.

② If the nerve branches are not found properly, the treatment effect is very low or recurrence is likely.

③ If experience is insufficient, other nerves or blood vessels, not the motor nerve to the gastrocnemius, may be damaged.

※ In general, performing the procedure by finding invisible nerve branches with a basic nerve detector often fails to produce meaningful results from the beginning because the nerves cannot be properly located, and it is common for the treatment to seem effective at first but then recur after several months.

A poorly performed nerve block can be worse than not doing it at all, so caution is needed.

Therefore, selective nerve block must not recur and must have a near-100% certain effect. Without directly locating the nerves with visual equipment such as ultrasound imaging and performing the procedure while confirming them visually, it is difficult to obtain good results.

● Post-procedure course and precautions

Indications for calf liposuction

When the cause of thick calves is subcutaneous fat, this method reduces volume by suctioning the fat layer.

If muscle and fat are both developed, liposuction may be performed together with muscle reduction surgery, and in some cases liposuction alone is performed.

How calf liposuction is performed

Subcutaneous fat between the skin and muscle can be treated with liposuction.

For calf liposuction, unlike liposuction in other areas, a thin cannula must be used so as not to damage the skin or nerves, and minimal incisions are made to leave almost no scar.

The procedure can be performed by area, and it must be divided into the inner and outer ankle regions.

The key to the procedure is precise suction without aftereffects such as contours or sensory disturbances.

Definition of varicose veins

When venous blood in the legs does not circulate properly and blood stagnates, causing the blood vessels to dilate, this is called varicose veins.

The symptom is that the vessels become abnormally enlarged and visible through the skin.

If the condition is severe, the vessels may bulge and become twisted, causing cosmetic concerns.

In some cases, blue vessels enlarge and spread like a spider web, and fine red capillaries may also appear like a spider web.

Overview of varicose vein treatment

When thin red vessels or thick blue vessels are visible on the thighs or calves, sclerotherapy or scarless surgery can be used to remove the unsightly vessels.

If thick vessels are visible through the legs, they do not look good even on beautifully shaped legs.

At our clinic, we have introduced advanced European methods, the birthplace of varicose vein treatment, since 2001 to provide fast and safe treatment.

Diagnosis of varicose veins

The simplest and most accurate diagnostic method for varicose veins is Doppler ultrasound, which can accurately assess vessel diameter and reflux.

During the examination, the patient stands while an ultrasound probe is placed on the area with veins, and the responsible vein is identified while checking the screen; the examination takes about 5 to 10 minutes.

Treatment methods for varicose veins

Broadly, there are two methods: surgery and sclerotherapy.

Surgical methods have developed in various ways over the centuries, and treatment has often involved removing the responsible vein from the body.

Currently, even in advanced European countries, surgical methods such as vein stripping and ambulatory phlebectomy are widely used, with sclerotherapy used as a supplement.

Sclerotherapy is a treatment method in which a sclerosant is injected into the blood vessel without surgery. Its advantages are that no anesthesia is needed and no scar is left at all.

In other words, the sclerosant is injected into the blood vessel using a syringe in the same way as a regular intravenous injection.

Other treatment methods include transilluminated powered phlebectomy (Trivex), endovenous laser treatment (EVLT), and dye laser treatment for capillaries; however, these carry concerns about various side effects, and in many cases their treatment effect is inferior to surgery or sclerotherapy, so the vessels that can actually be treated are limited.

These procedures are hardly used in advanced European countries, which are at the forefront of varicose vein treatment worldwide.

Pre- and post-procedure care for varicose vein treatment

● Pre-procedure precautions

1. Do not consume alcohol the day before the procedure. - It may make anesthesia less effective.

2. Avoid overeating on the day of the procedure.

3. If possible, do not drive to the clinic on the day of the procedure; use public transportation instead. - Driving after sedation may be dangerous.

● Post-procedure course and precautions after sclerotherapy

1. After the procedure, walk for about 1 hour if possible so that the sclerosant does not remain inside the blood vessel.

2. Showering is possible starting the day after the procedure.

3. Compression stockings may be washed with soap, but do not wring them out or iron them.

4. Wear the stockings continuously for the specified period, usually about 1 to 3 months.

5. Put the stockings on in the morning after waking up and remove them before going to bed.

6. Blue bruising along the vessel and blood clots forming after sclerotherapy are understood as a normal part of the treatment process.

7. Bruising along the vessel varies in severity, and the time it takes to disappear also varies; it usually fades almost completely in about 2 to 3 weeks.

8. The time required for sclerotherapy to be completed and for the vessel to be fully closed and disappear is about 1 month.

● Follow-up plan after sclerotherapy

1. After the first procedure, visits are usually made 3 to 4 times at intervals of 1 to 2 weeks.

2. Return after 1 to 2 weeks; areas with thrombosis are cleared with a needle, and then compression stockings are worn again. - Additional treatment may be performed on areas that were not fully treated.

3. Second visit after the procedure - 1 to 2 weeks after the second treatment

Blood clot removal from the vessels in the treated area and additional treatment to under-treated areas may be performed.

4. Third visit after the procedure - 1 to 2 weeks after the third treatment

Blood clots in the vessels in the treated area are removed.

The number of times blood clots are removed varies by person, usually about 2 to 4 times.
