What is lower blepharoplasty (under-eye wrinkle removal surgery)?

It is a surgery designed to make the lower eyelids that have sagged downward with age, or fine-wrinkle-type dark circles caused by high skin density, look smoother so that the face appears younger and the overall impression is improved.

The principle of dark circle correction through lower blepharoplasty

Under-eyes with many fine wrinkles caused by excessive skin density are inevitably darker in color.

In the case of dark circles caused by high skin density, the skin density needs to be lowered. If the condition is not severe, correction may be possible with fat grafting, but when the skin has too many wrinkles, there is a limit to how much fat grafting can expand the skin.

In such cases, it is unavoidable to remove the excess skin and smooth it out, and that method is lower blepharoplasty.

By removing and smoothing the skin, the skin density is lowered and the wrinkles are improved, which can help lighten the skin tone.

Painless anesthesia lower blepharoplasty procedure

Painless anesthesia is usually also called sleep anesthesia, and before local anesthesia is administered, an intravenous injection is placed in the arm in advance.

Then you enter a sleep state without realizing it, and local anesthetic is injected into the surgical area at that time.

There is no pain at all while the local anesthetic is being administered.

After the anesthesia is complete, you wake up from the sleep state, so the patient does not remember having been anesthetized, and the anesthesia ends without the patient even noticing.

The surgery is performed while you are awake.

The difference from general anesthesia, in which anesthetic gas is continuously administered through the airway during surgery and the patient is put into a state of unconsciousness, is this.

The difference is whether or not you are conscious during the operation.

Sometimes the patient is kept in a sleep state throughout the surgery, but there is still a clear difference from general anesthesia.

① Design - The position for fat removal is marked with ink.

The area where fat is located is marked with design ink, and in most cases the inner and middle areas are marked.

In some cases, only the middle area is marked.

The design is made by deciding which area’s fat will be removed depending on the distribution and amount of fat.

② Painless sleep anesthesia / local anesthesia - Anesthesia is performed with local anesthesia to the conjunctiva under painless sleep anesthesia.

Anesthesia is given so that you fall asleep through sleep anesthesia.

Then local anesthesia is administered again, but since you are already asleep, you cannot feel any pain.

③ Incision with a scalpel - After local anesthesia, an incision is made along the design line with a scalpel.

The incision is made along the planned line that has been designed.

④ Dissection and excision - After separating the skin and muscle from the fat, the remaining skin is excised.

Sufficient dissection is performed between the muscle layer and the fat layer down to below the wrinkle line, lifting the muscle and skin.

If there is a large amount of fat, the fat is pulled downward to fill the depressed area and is repositioned and fixed with sutures to smooth it out.

Fat repositioning can be applied only when there is a large amount of fat, and it cannot be applied when there is little fat or when the eye area is already hollowed out.

⑤ Suturing and completion of surgery - The lower blepharoplasty is completed by closing with dissolving sutures.

After skin excision, the skin is meticulously sutured with fine thread.

Once the suturing is finished, the area is disinfected, a skin-colored bandage is applied, and the patient goes home.

The stitches are removed after 4 to 5 days.

Postoperative care after lower blepharoplasty

1. For the three days after surgery, it is enough to apply cold compresses with the cool packs provided by the hospital for 2 to 3 hours a day.

The cold compresses may be applied continuously for 2 hours, or they may be divided into 20 to 30 minute sessions.

2. Visit 1 to 2 days after surgery to disinfect the surgical area.

3. Visit on day 4 or 5 after surgery to have the stitches removed.

4. After the stitches are removed, you may wash your face and apply makeup, and you may also shower or use a sauna or jjimjilbang.

5. Swelling after surgery varies by person, and it generally subsides to some extent within 7 to 14 days.

6. Avoid alcohol and smoking for about a week after surgery.

7. Visit the clinic one month after surgery for a follow-up.
