However, they are not suitable for use in large amounts.

Because most filler products are colorless and transparent, if they are used to create aegyo-sal, the under-eye area can become bulging and look translucent like a blister, contrasting with the surrounding skin tone and often looking unnatural.

Since there is concern about side effects and unnatural color when creating under-eye contours, this is not a recommended method.

② A method using fat repositioning

Some hospitals recommend creating aegyo-sal through fat repositioning.

However, it is impossible to create satisfying aegyo-sal through fat repositioning.

Aegyo-sal should protrude smoothly just below the eyelashes, but orbital fat cannot be forced toward the eyelashes to create a natural aegyo-sal contour.

In most cases, the aegyo-sal shape is barely created, or the protrusion occurs not directly below the eyelashes but below that area, resulting in an awkward shape.

Caution is needed when trying to create aegyo-sal through fat repositioning.

③ A method using AlloDerm (artificial dermis)

Aegyo-sal can also be created using solid artificial dermis such as AlloDerm or SureDerm, but because it requires incision of the skin, involves concern about scarring, and may not produce a natural shape but rather an artificial feeling, it is not a recommended method.

In the case of liquid AlloDerm, the cost is high and the absorption rate is high, so when sufficient protrusion is desired, the results are often unsatisfactory.

6. Common questions and answers about the aegyo-sal procedure

Question) If aegyo-sal was done with filler, is it possible to correct it later with fat grafting?

Answer) Many people who received filler treatment later want fat grafting again because of various problems.

However, once a person has undergone filler treatment, aegyo-sal fat grafting cannot be performed.

The reason is that the area treated with filler is not in a condition where fat can take hold.

Because blood circulation is poor, the fat cannot survive, and instead side effects such as inflammation or fat necrosis may occur. Even if it does survive, the survival rate may be low or the shape may not look natural.

Some people ask whether it is possible to dissolve all the filler with a dissolving injection and then perform fat grafting. If you were treated with a hyaluronic acid filler (Restylane, Juvederm, etc.), it can be dissolved with an enzyme injection called hyaluronidase.

However, unless the filler is almost completely dissolved with the dissolving injection, fat grafting is not possible if part of the aegyo-sal shape remains.

Even if the filler is dissolved with a dissolving injection, some of it remains, so the concern about inflammation after fat grafting remains. In principle, people who have used filler even once cannot undergo fat grafting.

Question) I had lower blepharoplasty because of under-eye wrinkles. Is aegyo-sal fat grafting possible?

Answer) Lower blepharoplasty is a method that removes skin and then pulls it tight to smooth wrinkles caused by under-eye wrinkles or sagging.

After lower blepharoplasty, the skin is pulled tight, so there is no extra skin, and scar tissue is formed just below the eyelashes.

In this state, it is difficult to create aegyo-sal.

For aegyo-sal to form naturally, there must be enough skin to expand, but after lower blepharoplasty, the skin has been excised, leaving almost no room for expansion.

Also, because of scar tissue at the incision site, the shape cannot be properly formed.

Therefore, it is difficult to create aegyo-sal after lower blepharoplasty.

As an exception, if the scar line is faint and it has been a long time since the lower blepharoplasty, making it possible to have even a small amount of extra skin, it may be possible to create a small aegyo-sal.

Question) If aegyo-sal was done with AlloDerm, is it possible to correct it later with fat grafting?

Answer) People who have undergone treatment with AlloDerm (artificial dermis) often want fat grafting again because they are dissatisfied with the shape.

Because AlloDerm is a living tissue, if the AlloDerm shape is not protruding in a very awkward way, it is possible to create a shape with fat grafting.

However, if the shape is awkward, the protrusion is severe, or the protrusion is not directly below the eyelashes but occurs below that area, correction with fat grafting is difficult.

Conversely, fat grafting is possible only when AlloDerm has been inserted into the correct position just below the eyelashes and the shape has come out mildly.

Question) At other clinics, they say that doing aegyo-sal with fat grafting makes the shape look unattractive and causes unevenness, so it is not good. Is that true?

Answer) There are clinics that indiscriminately perform contour enhancement with fillers.

At our clinic as well, fillers are used, but they are used only for fine wrinkles or small depressed scars.

Fillers (Aquamid, Restylane, Artecoll, etc.) are not products made for contour enhancement.

Therefore, their use involves difficulties in treatment and various problems such as side effects and reduced effectiveness.

The problem is that some non-specialist plastic surgeons and doctors with limited experience overuse fillers in procedures.​

7. Examples of micro fat grafting for aegyo-sal

For under-eye micro fat grafting, 3D volumetric fat grafting must basically be performed.

To prevent a lumpy contour after fat grafting, an appropriate amount must be grafted evenly.

Usually, the graft is divided and placed under the orbicularis muscle layer, within the orbicularis muscle layer, and above the orbicularis muscle layer.

For this, subcutaneous tunneling is inevitably performed as part of the process, and bruising occurs at this time.

The area around the eyes has many blood vessels, so some bruising is unavoidable.

and the shape does not come out naturally either.
