Temporal Region, Temples

Temporal fat grafting aims to eliminate shadows and softly restore the contour of the side of the face when the area above the side cheekbone is sunken or shadowed.

When the side cheekbone is prominent,

it has the effect of making the cheekbone appear less prominent visually.

Indications for temporal region / temple fat grafting

① When there is a sunken area in the temporal region (temple)

② When the cheekbones appear relatively prominent

④ When the forehead appears narrow compared to the width of the cheeks

How the fat grafting procedure is performed for temporal region / temple correction

| 1. Fat is harvested from areas where a lot of fat is accumulated, such as the lower abdomen, waist, or thighs. - Because local anesthesia (pain-free sedation anesthesia) is used, there is no pain at all. - A thin liposuction cannula is used to collect the fat into a syringe. The amount of fat collected is usually around 150–200 cc in preparation for a second procedure. During collection, a thin suction cannula is used to gently harvest living fat cells. |

| 2. The harvested fat is placed in a centrifuge to remove the local anesthetic and serum. Because the local anesthetic and serum are heavier than the fat and oil, they settle to the bottom. The purpose of centrifugation is to remove unnecessary components, reduce the risk of complications from fat grafting, and increase the survival rate of the fat. |

| 3. After centrifugation, the fat goes through a second concentration process to remove moisture and oil. By removing the oil, almost 100% concentrated fat is obtained. The oil cannot be removed by a machine and is instead removed manually through natural absorption by gauze. The fat is transferred into 1 cc syringes as needed. For temporal correction, about 10–20 cc of concentrated fat is usually required. |

| 4. Our clinic uses a self-developed microneedle (micro-grafting cannula). The microneedle is a special grafting cannula used only at our clinic, enabling ultra-precise grafting. In fat grafting, the fat is transplanted after the tunneling process has created space for it. The grafting is performed through an entry point near the hairline of the temporal region, which has already been locally anesthetized. The fat is usually placed in three layers: the muscle layer under the fascia and the fat layer under the fascia. Distributing the grafting across multiple layers is called three-dimensional multilayer fat grafting. The purpose of distributed grafting is to increase fat survival and create a softer shape. |

Our clinic’s fat grafting tunneling technique

With the tunneling technique, rather than injecting a large amount into one area, we perform three-dimensional micro fat grafting by evenly distributing 100% concentrated fat across multiple layers. A delicate and meticulous injection method allows for custom design. Fat grafting also includes a process called “tunneling,” which allows the fat to maintain continuous nourishment by making maximum contact with the surrounding abundant blood vessels.
Tunneling means creating a space for the fat just beneath the skin using a microneedle. Because a space is created like drilling a tunnel and the fat is injected afterward, the area does not sag downward due to gravity or become uneven even after all the fat has been injected.​​
Since the tunneling technique includes the dissection process of wrinkles, fat is grafted beneath the dissected wrinkles, resulting in a clearer improvement in wrinkles compared to fat grafting at other clinics.

Examples of temporal region / temple fat grafting

When the temporal region is sunken and the cheekbones appear protruded or the face appears long, fat grafting can serve as an alternative to inserting silicone implants, naturally lifting the sunken area and creating a softer facial contour.

Advantages of correcting the temporal region / temples with fat grafting

① The procedure is simple, allowing you to return to daily life immediately after the procedure. (No bandage or dressing is applied to the face.)

② Because no foreign material is used, unlike commercial implants such as silicone or Gore-Tex, there is no need to worry about side effects.

The effects of correcting the temporal region / temples with fat grafting

② Because the graft is placed above the cheekbone, it can make a protruding side cheekbone appear less prominent.

③ In cases of left-right asymmetry in the temporal region, symmetry can be improved.

④ By removing facial irregularities, it creates an egg-shaped, balanced, and soft contour.

Recovery process after temporal region / temple treatment with fat grafting​

Temporal region / temple fat grafting is an area with very little swelling and bruising.

After the procedure, no bandage is applied and no adhesive strip is placed on the treatment area.

You can resume daily activities as they are, and wearing glasses is also possible.

In some cases, hidden bruising may move downward and cause slight bruising under or around the eyes. If this happens, it is fine to cover the bruised area with makeup or concealer and continue daily life.

As the swelling goes down and the fat is absorbed and survives over the course of three months, you should be careful not to press on the grafted area until it has taken hold.

Number of temporal region / temple fat grafting procedures

On average, temporal region and temple fat grafting can produce satisfactory results with one treatment, and at most two treatments.

The interval between procedures is between 3 and 10 months, and when a repeat procedure is performed, the fat that was collected during the first fat grafting can be thawed and used.

The second procedure is determined by observing the result after the first procedure over 3 to 10 months, so a second procedure is not always necessary.

Whether a second procedure is needed depends on the degree of temporal hollowing, the goal of the procedure, and the fat survival rate, and this is entirely up to the individual to judge and decide.

Long-term effects of temporal region / temple fat grafting

Some people say that fat grafting has no effect because much of the fat is absorbed, but this applies to cases where the procedure was performed at a clinic with somewhat insufficient experience.

If fat grafting is performed correctly, satisfactory results can be obtained. Once the grafted fat survives, it is no longer absorbed and remains as your own fat tissue for life, making it the safest and most effective method.

On average, one to two fat grafting procedures can achieve permanent, satisfactory facial contour correction.

Side effects after temporal region / temple treatment with fat grafting

Fat grafting itself has no side effects.

However, due to the inexperience of the practitioner, side effects such as severe swelling or bruising from excessive fat grafting, uneven grafting that causes lumpiness, or very weak results because the fat was not properly grafted and was largely absorbed, as well as vascular embolism caused by forceful fat injection under excessive pressure, may occur.

However, these side effects occur when the practitioner lacks experience or performs the procedure in a manner that goes against the principles, so in principle, if the fat grafting is done properly, such side effects will not occur, and you can receive the procedure with confidence.​
