Side effects of fat grafting for wrinkle treatment

Also, not all wrinkles are suitable for fat grafting, so it should be applied only to wrinkles that can benefit from it. Unlike contour correction, wrinkle procedures are more technically demanding, so as a rule, they should not be performed by someone without sufficient experience or by a non-board-certified plastic surgeon.

How to resolve side effects from overfilled fat grafting

When fat grafting is performed against the principles due to insufficient experience, or when too much fat is grafted because the amount was not properly controlled, the grafted area may become severely swollen, or even after the swelling goes down, the treated area may remain overly protruded and look unnatural.

This usually happens when a large amount of poorly purified fat containing many impurities is forcibly injected.

In most cases, it subsides over time, but if the swelling is too severe, hyaluronidase to reduce swelling may be injected into the treated area to help it go down faster. If too much fat has been injected, a fine cannula may be inserted to suction out the grafted fat and remove it.

Overfilled fat can be removed more easily if it is suctioned out again within 3 months.

If the grafted fat occasionally necroses and calcifies, becoming hard and bringing the patient to the clinic, it must be removed by suction.

① Method using fillers (Juvederm, Artecoll, Teosyal, Evolence)

Mouth corner wrinkles may also be corrected with fillers or collagen, but the use of fillers is very limited.

Fillers can be tried for fine, hairline-like wrinkles.

However, this is not an appropriate method for correcting long wrinkles, and various side effects may occur.

Because it is a foreign substance, it may feel unnatural when smiling, the contour may protrude, and the implant may shift position due to the force of smiling.

It can sometimes also cause problems such as inflammation, so inserting implants into nasolabial folds is not a desirable method.

There is a limit to how much facial skin can be pulled by the force of threads, and because there are many side effects such as quick recurrence or contour irregularities on the face, caution is needed.

④ Insertion or injection of acellular dermal matrix AlloDerm (SureDerm)

Some correction is performed using artificial dermis such as AlloDerm, made by processing human dermal tissue, but precise procedures are difficult.

Also, because liquid AlloDerm is significantly absorbed, it is difficult to predict the corrective effect, and dissatisfaction with the procedure is common.

Solid AlloDerm is mainly used for tip rhinoplasty to create an upturned nose and is not a desirable procedure for correcting nasolabial folds.

⑤ Insertion of gold threads or gold needles

There are clinics that mislead patients by saying that inserting gold needles into the face will make the skin elastic and tight and smooth out wrinkles.

It is mostly performed by non-specialists, and because it is a very expensive procedure, it is promoted as if the effect is outstanding, but consumers should be careful.

Side effects of fat grafting

Of course, good results are desired, but depending on the surgeon’s experience and technique, unavoidable problems sometimes occur.

Poor results due to low survival rates, unnatural shape, asymmetry, severe swelling, inflammation, fat necrosis, calcification, contour irregularities, sagging, and pigmentation may occur.

Pigmentation	In cases of severe bruising or swelling, depending on the person’s constitution, iron components left in the bruise may deposit in the skin and oxidize, turning brown. This may occasionally occur after under-eye procedures depending on the individual’s constitution
Asymmetry	Occurs when the survival rate differs between left and right sides, or when the amount was not properly controlled during the initial procedure, causing imbalance
High absorption rate	A phenomenon in which the fat does not survive enough to create the desired contour and much of it is absorbed, so the appearance before and after the procedure does not change much

Misconceptions and problems regarding fat grafting

You may think that fat grafting always produces good results, but that is absolutely not the case.

There is a tendency to think of fat grafting as easy, but it is absolutely not a simple or easy procedure.

As with all surgeries, the results depend on the surgeon’s experience and know-how.

Fat grafting is one of the most difficult procedures in plastic surgery, and along with a very complex process, it requires experience, know-how, and highly refined manual skill.

How to perform fat grafting without side effects

Procedure in a sterile state

As with all surgeries, fat grafting must be performed with fully sterilized instruments and materials.

After the procedure is finished, any remaining fat inside the suction cannula or grafting cannula should be thoroughly cleaned with a brush, and then sterilized in a high-pressure steam sterilizer. Anesthetic agents and saline solution must also be carefully managed so they are not contaminated.

Thorough disinfection and proper material management can prevent bacterial infection and inflammation.

Several stages of fat purification

Fat purification must remove impurities completely through several stages to maximize the effect.

Harvested fat usually contains about 80% impurities, and only about 20% is actually usable.

If the remaining impurities (blood, serum, anesthetic, oil, water, etc.) are not removed, the result will not be good no matter how well the grafting is performed.

Preliminary preparation before fat grafting

A high survival rate should make it possible to achieve satisfactory results with just one procedure whenever possible.

The micro-fat must be evenly placed in multiple layers, and before that, the grafted fat should not be compressed by surrounding tissue.

By securing space in advance where the fat will be grafted, the fat will not be compressed and can survive better.

Precise and uniform grafting across multiple tissue layers

The expression of a soft, natural contour is the core of fat grafting.

For a soft contour, it is better to graft in a three-dimensional manner, distributing the fat evenly across multiple layers.

The grafting sites include the subcutaneous layer, muscle layer, and fascial layer. To achieve a smooth contour without irregularities, the fat must be spread throughout the tissue as if sowing seeds evenly.
