What are the indications for double eyelid surgery?!

When the eyes look small

When there are folds on the eyelids

When the eyelids appear droopy

When only one side has a double eyelid

What are the basic principles and concept of double eyelid surgery?

People often mistakenly think that double eyelid surgery is simply a surgery to create a double eyelid line.

Its purpose is not merely to create a double eyelid line.

The goal of double eyelid surgery is to make the eyes look larger and brighter, creating an impression that is intelligent yet beautiful.

A surgery done only to create a double eyelid line can sometimes create an unpleasant impression.

The ultimate goal of double eyelid surgery is to create a natural and beautiful eye shape.

Examples of eye shapes that need double eyelid surgery

What are the types of double eyelid surgery methods?

Full incision method: The full incision method is applied when the skin is sagging.
Partial incision method: This is applied regardless of skin thickness when the skin is not sagging in younger patients.
Buried suture method: This is applied in younger patients with thin skin, no fat, and no sagging skin.

◈ Full incision method ◈

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 6

The full incision method is mainly applied when the skin has sagged due to age.

It allows removal of sagging skin, and when the fat or muscle is thick, it also allows removal of fat and muscle.

Advantages of the full incision method

○ This method is used when the skin has sagged and there is a lot of excess skin.

○ The double eyelid line can be designed to be clear and refined.

○ Unnecessary orbicularis oculi muscle can be removed, and fat can be removed.

○ The double eyelid is less likely to loosen.

○ If ptosis is present, ptosis correction is also possible.

Disadvantages of the full incision method

○ A long scar remains.

○ Swelling and bruising last longer than with the buried suture method or partial incision method.

○ If you do not like the double eyelid line, it is difficult to revise.

◈ Partial incision method ◈

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 7

The partial incision method compensates for the shortcomings of the buried suture method.

It is a method of creating a double eyelid by making only three small incisions of about 2 mm each along the intended double eyelid line.

It is a method that is less likely to loosen than the incision method and leaves almost no scar like the buried suture method, combining the advantages of both the incision and buried suture methods.

This is the eye surgery technique mainly used at our clinic.

Advantages of the partial incision method

○ The operation takes about 30 minutes, normal daily life is possible immediately, and washing the face and applying makeup are possible from the next day.

○ There is almost no swelling or bruising.

○ Orbital fat can also be removed, and stitches do not need to be removed.

○ A natural double eyelid line can be achieved quickly, and the surgery is not noticeable.

○ It is less likely to loosen than the incision method.

○ There is almost no scar.

◈ Buried suture method ◈

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 8

This method involves making a tiny incision in the skin and burying the thread.

It is mainly applied to young people with thin skin.

However, because the line can change or loosen easily, and because fat cannot be removed, it is not a particularly desirable method.

Advantages of the buried suture method

○ The surgery takes 30 minutes, normal daily life is possible immediately, and washing the face and applying makeup are possible from the next day.

○ A natural double eyelid line can be achieved quickly, and the surgery is barely noticeable.

○ There is almost no scar.

Disadvantages of the buried suture method

○ It loosens easily when the skin is thick.

○ Over time, the line easily becomes loose or changes shape.

○ Fat removal is not possible.

○ If internal bleeding occurs, there is no drainage path for the swelling, so swelling and bruising are often severe.

○ Because the thread is fixed above the tarsal plate, it is difficult to control the thickness of the line.

○ In cases such as the single-knot continuous suture method, the amount of thread buried is large, so the swelling may not subside easily.

Comparison of common double eyelid surgery methods

Please refer to the table below~^^

Incision methodBuried suture methodPartial incision method (quick double eyelid)
CandidatesPerformed when the upper eyelid skin is significantly saggingApplied when the skin is not sagging, is thin, and there is not much fat around the eyesApplied to eyelids that are not sagging
Advantages* Sagging skin can be removed.* The line is less likely to loosen.* Unnecessary fat can be removed.* Ptosis correction is possible.* There is less scarring.* Swelling goes down faster.* Almost no scar remains.* Swelling is minimal and subsides quickly.* The line is less likely to loosen.* Orbital fat can also be removed.
Disadvantages* A scar from the incision remains. * Swelling lasts a long time.* Cannot be used when the skin is sagging.* The line is likely to loosen or become thin.* Orbital fat cannot be removed.* Cannot be used when the skin is sagging.

Which surgery method is appropriate for you?

It is selected according to the shape and condition of the eyes,

and the surrounding circumstances.

For young people whose skin is not very saggy,

partial incision is used in most cases,

and for older patients,

if the degree of skin sagging is severe,

incision surgery is recommended.

What is an attractive and beautiful double eyelid?

There is no fixed standard

for an attractive and beautiful double eyelid line,

but let’s look at what is generally considered

an attractive double eyelid line.

A double eyelid that does not look surgical

and looks natural yet beautiful

can be called an attractive double eyelid.

Dr. Shin Duhan

A double eyelid line should not be too large.

In general, when the line is large, most cases end up with an awkward impression.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 9

Depending on the perspective of beauty, a large and distinct line can give an artificial and awkward impression.

The most beautiful and ideal double eyelid line is one that is just slightly visible by about 1 to 2 mm when the eyes are open.

It is good if the double eyelid line clearly extends all the way to the inner corner of the eye.

East Asian eyes sometimes have an epicanthal fold (inner canthal fold), and if this is overlooked during surgery, the inner line of the eye can disappear, causing a feeling of heaviness.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 10

Whenever possible, it is better for the double eyelid line to remain visible all the way to the inner corner of the eye. If the line does not form properly because of an epicanthal fold, epicanthoplasty is needed. If there is no epicanthal fold, the design must be done carefully so the line does not break off at the inner side.

The surgical marks should not be visible.

The biggest drawback of surgery using the incision method is the scar issue.

How much scar remains depends on the surgeon’s skill and the individual’s constitutional factors.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 11

Scarring is relatively more likely, especially in people with thick skin and thick orbicularis oculi muscle.

Once a scar forms, there is no way to erase it; however, in cases of severe scarring, scar revision surgery can be performed to make the scar less noticeable.

Therefore, at the initial surgery, it is important to remove the appropriate amount of skin and perform meticulous suturing so that scarring is minimized.

The eyelids should look thin.

Causes of eyelids looking puffy include thick skin or orbicularis oculi muscle, or a lot of orbital fat or pre-tarsal fat.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 12

It is best to address these issues while performing double eyelid surgery.

The most common cause is excessive orbital fat.

This can be removed during double eyelid surgery.

Alternatively, if the orbicularis oculi muscle layer beneath the skin is thick, part of the muscle can be removed. The orbicularis oculi muscle below the incision line can be partially removed, but the muscle above the incision line is difficult to remove.

If there is an epicanthal fold, it is better to remove it.

In East Asians, the epicanthal fold (inner canthal fold) sometimes causes the skin at the inner corner of the eye to cover the eyeball, making the eyes look heavy. Removing it makes the horizontal width of the eyes look larger and more refreshing, and the double eyelid line becomes clear all the way to the inner side.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 13

However, the operation must be performed carefully while considering the eye shape, in order to avoid unsatisfactory results.

If the intercanthal distance does not look wide and there is almost no epicanthal fold at the inner corner of the eye, performing epicanthoplasty can instead make the eyes look too close together or give the eyes a harsher appearance. Therefore, it should only be performed after an accurate physical examination and only when the indication is appropriate.

If ptosis is present, it should be corrected at the same time.

Ptosis refers to a condition in which the upper eyelid droops and covers the pupil because the function of the levator palpebrae superioris muscle that lifts the eyelid is weak, making the eyes look sleepy. If this is overlooked and surgery is performed, the double eyelid line may not turn out beautifully.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 14

Before double eyelid surgery, the eyes should look large and clear for the double eyelids to also look beautiful.

Depending on the degree of ptosis, if ptosis correction is judged to be necessary, it is better to correct it at the same time using the incision method.

A beautiful quick double eyelid (partial incision method)

that allows return to daily life with minimal swelling

What is a quick double eyelid?

A quick double eyelid is a surgery that uses minimal incision and minimal swelling, allowing immediate return to daily life after surgery.

At our clinic, quick double eyelid surgery is performed using a minimal-incision partial incision method, and since skin suturing is not required, normal daily activities are possible, swelling is minimal right after surgery, and the result is natural.

It also has the advantage of being less likely to loosen.

What are the four principles of quick double eyelid surgery (partial incision method)?

① Swelling must be minimalThis is one of the methods that causes less swelling than any other type of quick double eyelid surgery.
② The double eyelid line must not loosenBy removing orbicularis oculi muscle and orbital fat, the line is almost never loosened.
③ Scarring must be minimalBy using a minimal 2 mm incision, scarring is almost nonexistent.
④ The double eyelid line must look naturalThe most natural line is found by pre-designing the line while sitting, and surgery is performed accordingly.

Why does quick double eyelid surgery (partial incision method) cause no swelling?

Swelling occurs when exudate produced by surgery accumulates within the tissue.

The reason quick double eyelid surgery causes almost no swelling is that the orbicularis oculi muscle and fat tissue, which are the swelling-causing tissues, are removed through micro-incisions, and the exudate produced by the operation is immediately drained through the tiny incision sites, leaving no room for swelling to develop.

For example, it is similar to how a bruised area without a wound swells, while a cut wound hardly swells.

A bruise without a wound swells because exudate forms inside and is not drained, so it accumulates. A cut wound does not swell because the exudate drains out immediately.

What is the surgical process for quick double eyelid surgery (partial incision method)?

① Design the desired double eyelid line.

The design is made according to the height and shape the patient wants, or the surgeon determines the most suitable double eyelid line and designs it while the patient is seated.

The height of the double eyelid line is usually designed to be between 5 and 8 mm from the eyelashes, and the direction of the design line changes depending on whether it will be an outfold or an infold.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 17

② Mark the three incision points lightly with design ink.

The locations are lightly marked on the inner side, center, and outer side.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 18

③ Under sleep anesthesia, after about 5 minutes of sleep, local anesthetic is injected into the three marked areas.

The local anesthetic is administered in minimal amounts to prevent swelling as much as possible.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 19

④ Make very small incisions of about 2 mm at the three marked sites.

If the incisions are made too large, much scarring will remain, so the surgery is performed with minimal incisions to leave almost no scar.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 20

⑤ Through the incision sites, first pull the orbicularis oculi muscle and remove it with scissors.

The orbicularis oculi muscle is the muscle layer covering the eyelid, and because it interferes with double eyelid formation, it must be removed to prevent the line from loosening.

Along with removal of the orbicularis oculi muscle, the pre-tarsal fat beneath the muscle layer is also removed.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 21

⑥ If there is a lot of orbital fat, pull it out through the outer incision site and expose it.

Fat removal is mainly performed from the outer side.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 22

⑦ Hold the fat with a hemostatic forceps and cut the base of the protruding fat with a scalpel.

Excessive removal of fat can cause a hollowed eyelid, so it is better to remove only as much as protrudes.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 23

⑧ After fat excision, use an electrocautery device to stop bleeding so that no bleeding occurs.

If hemostasis is not properly achieved, orbital bleeding may occur, leading to severe bruising and swelling, so proper hemostasis is important.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 24

⑨ Through the three incision sites, use a very fine absorbable thread to connect the levator palpebrae superioris muscle and the skin to create the double eyelid line.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 25

⑩ After burying the thread, apply ophthalmic ointment and finish the surgery.

Washing the face and applying makeup are possible from the next day.

Apply cold compresses for 3 days using the cool pack provided by the hospital.

If cold compresses are done for more than 3 days, swelling may not go down any further, so it is best to do it for exactly 3 days only.

How should you care for yourself after quick double eyelid surgery (partial incision method)?

① Use the cool pack provided by the hospital for cold compresses.

Do this for 2 hours a day for 3 days only.

The 2 hours of cold compresses may be done continuously, or divided into 20 to 30 minute sessions.

Usually, it is convenient to do it before going to sleep.

② After surgery, return to the hospital the next day once to have the surgical site disinfected and the recovery observed.

③ Makeup and face washing are possible from the next day.

④ Swelling after surgery varies by person, and in most cases it mostly subsides in about 7 days, but it takes 1 to 3 months to fully settle.

Epicanthoplasty (inner corner eye surgery)

What is epicanthoplasty - inner corner eye surgery?

When the distance between the two eyes appears wide, the skin at the inner corner of the eye often excessively covers the eyeball. The skin covering the eyeball at the inner corner of the eye is called the epicanthal fold.

Epicanthoplasty (inner corner eye surgery) refers to a procedure that removes the skin at the inner corner of the eye to make the distance between the eyes normal and open up the cramped-looking inner corner of the eye.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 47

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 48

How is epicanthoplasty diagnosed?

Eyes with an epicanthal fold usually have an inner corner that is not sharp and looks rounded, or the angle descends sharply.

An epicanthal fold (inner canthal fold) is diagnosed when the interocular distance is 40 mm or more and the skin at the inner corner of the eye covers the white of the eye, creating a cramped feeling.

What are the principles of epicanthoplasty?

♥ There should be no scar. The incision line should be hidden inside the eyelid border so that scarring is almost nonexistent.
♥ The shape should look natural. The inner corner of the eye should be designed horizontally so that the epicanthoplasty looks natural.
♥ There should be no recurrence. The inner tail should be fixed and sutured to the ligament so that it does not return to its original position.

What are the indications for epicanthoplasty?

  1. When the distance between the eyes appears wide

As a rule, surgery is performed when the distance between the inner corners of the eyes exceeds 40 mm.

The normal distance between the eyes is about 36 to 37 mm, but if the distance is normal and an epicanthal fold is present, it is better not to perform surgery.

If done incorrectly, the distance between the eyes may become too short and the eyes may appear too close together.

  1. When you want to create an outfold double eyelid

If an epicanthal fold is present, an infold double eyelid is created, so epicanthoplasty must be performed to create an outfold double eyelid.

Comparison between externally visible epicanthoplasty scars and our clinic’s epicanthoplasty?!

The principles of epicanthoplasty are that the scar should be minimal, the recurrence rate should be low, and it should be possible to perform it together with double eyelid surgery.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 49

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 50

At our clinic, surgery is performed using the Furukawa, Y-V method.

The advantages of the surgery are that the scar is less noticeable and the result is natural. When performed together with double eyelid surgery, it also has the advantage of allowing the epicanthal fold to be removed naturally as an extension of the double eyelid line.

What are the various methods of epicanthoplasty (inner corner eye surgery) and what do the scars look like?

Although many methods have been developed and used, only a few are actually used.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 51

The principles of epicanthoplasty are that the scar should be minimal, the recurrence rate should be low, and it should be possible to perform it together with double eyelid surgery.

At our clinic, the Furukawa method shown at the lower right is mainly used.

The advantages are that the scar is almost invisible, the shape looks natural, and when performed together with double eyelid surgery, the epicanthal fold can be removed naturally as an extension of the double eyelid line.

What is the surgical process for epicanthoplasty (inner corner eye surgery)?

① Pull the skin toward the nose to make it taut.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 52

② While the skin is pulled tight, design the area with special ink.

The shape of the design varies depending on the surgical method, so there is no single standard shape.

At our clinic, the Y-V method is mainly used so that the scar is not noticeable.

③ Under sleep anesthesia, local anesthesia is applied to the designed area.

⑥ Reposition the skin tissue so that the epicanthal fold is pulled toward the nose.

At this time, the excess skin tissue is removed in an appropriate amount.

⑦ Suture the skin with fine thread.

No bandage is needed immediately after surgery, and stitches are removed after 5 to 6 days.

How should you care for yourself after epicanthoplasty (inner corner eye surgery)?

  1. After surgery, return the next day to disinfect the surgical site and observe the recovery.

  2. Stitches are removed on the 5th day after surgery.

  3. Swelling after surgery is minimal, and the period for it to fully settle is about 1 to 3 months. During this period, do not rub the surgical site excessively.

  4. For about 3 months, there may be slight scarring at the inner corner of the eye, but this is a normal process, and after 3 months it becomes almost unnoticeable.

  5. Be careful not to rub the surgical site when washing your face.

If you rub the surgical site strongly, the scar may become messy or widen, so be careful.

What are the precautions for epicanthoplasty, inner corner eye surgery?

It should be performed only when it suits your eye shape. During consultation, discuss with a specialist and carefully decide whether epicanthoplasty is truly necessary before proceeding.
Epicanthoplasty is difficult to perform without leaving any scar, no matter which method is used. It is better to use a method that leaves less scarring, and it is desirable to use a method that hides the scar inside. Because the scar is not visible externally, there are almost no signs that surgery was performed.

When epicanthoplasty should not be performed

① When the distance between the eyes is less than 40 mm

The normal distance is usually about 36 to 37 mm.

If it is less than 40 mm, epicanthoplasty should not be performed.

If you perform epicanthoplasty on eyes with a normal distance, the eyes may appear too close together, so caution is needed.

② When the interocular distance is wide, but the inner corner of the eyes has a sharp shape

Epicanthoplasty should not be performed.

The goal of epicanthoplasty is to open up the rounded inner corner of the eye and make it look more refreshing, but if the eye shape is already sharply open, surgery may make the eyes look harsher, so caution is needed.

Why scars do not appear in epicanthoplasty at Duhan Plastic Surgery Clinic’s double eyelid clinic

When skin is incised, a scar inevitably forms.

The same applies to epicanthoplasty,

so scars are unavoidable because the surgery is performed by incision.

If the surgery is performed in a way that exposes the scar externally,

the scar can look unsightly no matter how meticulously it is sutured.

However, if the scar line is buried at the edge of the eye corner

and hidden, the scar becomes hardly noticeable.

In other words, because the scar is not exposed externally, it becomes invisible.

What is revision surgery for epicanthoplasty?

It refers to surgery that restores the original eye shape

when unnecessary epicanthoplasty was performed even though it was not indicated,

thereby worsening the impression.

Why is revision surgery for epicanthoplasty performed?

If epicanthoplasty was done because it was necessary, the question is whether it really needs to be reversed again.

This problem occurs when epicanthoplasty is recommended unconditionally without considering the postoperative eye shape.

If epicanthoplasty is performed when it is not indicated, various problems arise.

Please refer to the problems that can occur below.

Problems caused by unnecessary epicanthoplasty

◇ The distance between the eyes becomes shorter than normal, making the eyes appear too close together.

◇ If epicanthoplasty is performed when the inner corner is already sharp, the eyes become sharper and look harsher.

◇ The mucosa at the inner corner of the eye is excessively exposed, making it look unpleasant.

◇ When epicanthoplasty is performed, the inner line of the eyes becomes thicker, and if an infold line was desired, it can become an unwanted outfold line.

◇ Revision double eyelid surgery was done to reduce the inner line, but because epicanthoplasty was performed at the same time, the line did not shrink or became even larger.

How is epicanthoplasty revision surgery performed?

It is revised by designing in the opposite direction from the original epicanthoplasty and using the V-Y advancement flap method or the Z-plasty method.

The method used varies depending on the shape of the individual’s eyes and the available tissue.

Revision surgery leaves more scarring than epicanthoplasty itself, so this must be taken into account.

Examples of epicanthoplasty revision surgery

It is important to avoid creating the situation that requires epicanthoplasty revision surgery in the first place.

Having epicanthoplasty make your impression worse and then needing revision surgery to restore it is a waste of both money and time.

You should carefully judge whether it is truly necessary for you before deciding.

How epicanthoplasty reversal is performed at Duhan Plastic Surgery Clinic’s revision double eyelid clinic

If epicanthoplasty has been done severely and reversal is desired,

if the current double eyelids look natural and there is enough excess skin in the front area,

one-stage treatment may be possible, but if there is no extra skin

and the double eyelids are awkward and large,

stepwise reconstructive plastic surgery may be necessary.

In general, the thickness (width) of the double eyelid

is based on about 6 mm, and if it is much larger than this,

and if the fixation position of the double eyelid is very high,

it is expected to take a long time before it looks natural.

Reconstruction can be performed to match the shape of your eyes.

It takes about 1 to 3 months for the swelling to go down and for things to settle.

Even if you undergo Mongolian fold reconstruction surgery,

it cannot be exactly the same as your old eyes,

and because the skin around the eyes must be brought toward the eye area during reconstruction,

you must be careful that the scar does not spread below the inner corner of the eye.

The scar does not form because it is sutured inside the skin.

How back corner eye revision surgery is performed at Duhan Plastic Surgery Clinic’s revision double eyelid clinic

Back corner eye surgery has recently gained attention by compensating for the drawback of re-adhesion, which is a problem in conventional back corner eye surgery.

Through skin dissection,

the tissue that would cause re-adhesion is sufficiently removed so that it does not reattach, and the surgery delicately sutures from the inner conjunctiva of the eye to the ligament-fascia layer, muscle layer, and skin layer according to anatomical structures.

Because the skin and mucosa are sutured separately, it does not form a D-shape,

and there is no need to worry about re-adhesion.

Also, unlike other hospitals, it is not performed by skin incision alone; by firmly fixing it to the periosteum, it does not loosen and is a safe surgery that can also be reversed later.

The suture site is naturally hidden along the eye corner line,

minimizing externally visible scarring, and due to more than 20 years of experience in revision double eyelid surgery,

delicate and precise suturing leaves almost no scar.

Frequently Asked Q&A about eye plastic surgery

Incision method

Q. Does the incision method leave a lot of scars?

A. Scarring cannot be avoided with the incision method, but the best result is when, with eyes closed, only a very faint incision line is visible; that is considered a successful surgery.

If the surgery is done poorly, a sunken, ditch-like scar may remain.

To minimize scarring in the incision method, meticulous suturing during closure is required.

In other words, the degree of scarring is determined by how the suturing is done and by individual constitutional factors.

If precise and meticulous suturing is performed, scarring can be minimized.

Q. Is swelling and bruising more severe than with the buried suture method or partial incision method?

A. It is true that the incision method causes more swelling and bruising than the buried suture method or partial incision method.

However, the difference is about 1 to 2 weeks.

The amount of swelling and bruising varies from person to person.

Q. When can daily life resume after the incision method?

A. With the incision method, stitches are usually removed on day 3 to 4.

However, because no bandage is applied, normal daily life is possible from the day after surgery.

When the eyes are open, the suture thread is hardly visible, so daily life is not very difficult.

Buried suture method

Q. What is the biggest problem with the buried suture method?

A. The problem with the buried suture method is that the line may loosen partially or completely.

Statistically, it is more likely to loosen than the incision method or partial incision method, so if a buried suture double eyelid loosens once, there is a high chance it will loosen again even if the buried suture method is done again.

Q. Does the buried suture method cause almost no swelling?

A. Since the line is created by merely looping the thread, this is a method with almost no swelling.

However, swelling varies from person to person, so in some cases it may last longer.

In particular, in the case of the single-knot continuous buried suture method, because the thread is buried continuously in a double layer, swelling often subsides more slowly than with other buried suture methods.

You cannot say that the buried suture method causes relatively less swelling than other methods.

Q. Should the buried suture method be avoided if the skin is thick?

A. If the skin is thick, or there is a lot of fat, or the orbicularis oculi muscle is thick, the buried suture method should be avoided as much as possible.

If you try the buried suture method even though it does not suit you, and the line is not natural and loosens partially, the result may be worse than not doing it at all.

Quick double eyelid surgery

Q. What is the biggest difference between quick double eyelid surgery (partial incision method) and the buried suture method?

A. The partial incision method is a way to gain the advantage of the incision method, which is that it does not easily loosen.

A simple buried suture method creates the double eyelid line by only burying thread, so when the skin is thick or there is a lot of muscle and fat, it can loosen easily.

The partial incision method has several advantages, such as the ability to remove orbital fat, minimal scarring, and resistance to loosening.

Q. Does the partial incision method leave no scar?

A. Like the buried suture method, it leaves almost no scar.

The period for the scar to settle is usually after 3 months.

Until 3 months, there is an erythematous phase and the scar is developing, so until around 3 to 4 months a slightly noticeable scar may be seen, but after 3 to 6 months the scar becomes almost invisible.

Q. How much swelling and bruising occurs with quick double eyelid surgery?

A. Swelling and bruising are minimal, so daily life is possible right away.

Even if a person’s constitution causes a little bruising, it is not very noticeable and almost disappears in about a week.

Q. From when can I wash my face and wear makeup after quick double eyelid surgery?

A. Washing the face and applying makeup are possible from the day after surgery.

After washing your face, apply the disinfectant and ointment prescribed by the hospital, and then you may apply makeup.

Epicanthal fold

Q. What is an epicanthal fold?

A. An epicanthal fold, also called the “inner canthal fold,” refers to a condition where there is too much skin at the inner corner of the eye, covering the white of the eye.

Usually, when an epicanthal fold is present, the eye shape looks cramped and the distance between the eyes appears wide.

Q. Does epicanthoplasty leave scars?

A. Most epicanthoplasty procedures leave some degree of scarring.

Many methods have been developed for epicanthoplasty, but only a few are actually used.

It is best to use a method that leaves as little scarring as possible and looks natural.

Since surgical methods differ from hospital to hospital, it is best to use a method that leaves less scarring, has no recurrence, and achieves natural results.

Q. Are epicanthoplasty and inner corner eye surgery the same thing?

A. They mean the same thing.

Epicanthoplasty is also called inner canthal fold surgery, and the commonly used term is inner corner eye surgery.

They all mean the same thing.

However, back corner eye surgery is a method that opens the outer corner of the eye, so it is the opposite of inner corner eye surgery.

Q. Can double eyelid surgery and epicanthoplasty be done at the same time, or should they be done separately?

A. The two surgeries can be done at the same time or separately.

You can do double eyelid surgery first and epicanthoplasty later, or epicanthoplasty first and double eyelid surgery later.

Double Eyelid Surgery at Duhan Plastic Surgery Clinic - Which Eye Surgery Is Right for Me? image 73

As the saying goes, the eyes are the window to the soul, and they are a very important part of the face.

Double eyelid surgery is the most basic and common of all cosmetic surgeries.

A person’s eye shape plays a major role in first impressions.

Double eyelids make the eyes more beautiful, and eye-shape plastic surgery is a procedure that makes even a person’s impression more beautiful.

The most important purpose of double eyelid surgery is, of course, to create beautiful eye shape.

If the surgery is done simply to create a double eyelid line, it can actually damage the impression.

That is why double eyelid surgery is also called eye-shape correction.

Every person’s eye shape is vastly different, so not everyone can be operated on with the same method and design.

The design must be tailored to each individual’s characteristics, and the height of the double eyelid must be precisely set.

And the surgery must be designed while anticipating how it will look when the line finally settles after 3 to 4 months.

The double eyelid line after it has fully settled is ideal when it is barely visible, or

when only about 1 mm is lightly visible as the eyes open.

However, some people end up with sleepy-looking eyes after surgery.

No matter how pretty the double eyelid line is, if the way the eyes open is not refreshing, that is also an incorrect double eyelid.

Also, when the eyes are closed, if the incision marks are visible, that is also a wrong double eyelid.

If the incision marks are visible just because the incision method was used, that is not acceptable.

A beautiful eye-shaped double eyelid should give others the impression that the eyes are large and beautiful.

Very few people have all of these conditions.

That is because double eyelid surgery is such a difficult and delicate procedure.

The reason it is the operation plastic surgeons perform most often yet still consider one of the most delicate is

that every individual’s eye shape and characteristics are different, so many variables must be considered during surgery.

If the doctor is experienced and skilled, even if an unwanted result occasionally appears after surgery,

there is no problem as long as it can be corrected immediately to obtain a satisfying result.

In other words, double eyelid surgery is the creation of beautiful eye shape.
