If the line loosens after the buried-suture method, even correcting it again with the buried-suture method carries a high chance of the line loosening again, so the basic principle is to correct it with a partial-incision method.

Correction method for loose double eyelids - micro fat grafting

If the eyelid has become sunken and the double eyelid line has been pulled upward, causing the double eyelid to loosen, the loose line can be corrected together with the sunken eyelid through micro fat grafting.

However, this applies only when the cause of loosening is solely due to eyelid depression.

The main purpose of the procedure is to correct the sunken eyelid, and correcting the double eyelid line is a secondary goal.

If the double eyelid line does not naturally restore itself even after the eyelid is restored, revision surgery is necessary.

A double eyelid line with large, half-open eyes is called pseudoptosis.

Most revision surgery patients are such cases.

The goal of revision double eyelid surgery is to make the double eyelid line as thin as possible and to help the half-open eyes open more clearly.

If the surgery went smoothly, in most cases the eyes should open clearly as soon as the operation is finished, and that is considered a successful surgery.

There should be mild lagophthalmos for a certain period after surgery.

If the eyes can close perfectly right after surgery, that is considered an incorrectly performed operation.

In most revision double eyelid surgery cases, there is not enough excess skin.

When surgery is performed in this state, if the eyes are to close well, the skin has to stretch from somewhere, and in most cases the skin stretches below the incision line.

If that happens, the double eyelid line becomes larger again.

Therefore, revision surgery is properly performed when the eyes temporarily do not close well after surgery.

However, during the adaptation period, the skin above the incision line gradually stretches, and after some time the eyes close naturally.

The duration of lagophthalmos after surgery varies somewhat depending on the degree of ptosis correction and the amount of excess skin.

If there was not enough excess skin, recovery from lagophthalmos is slower; conversely, if there is excess skin, recovery is faster.

If the line has loosened after a buried-suture method, a full incision is not necessary,

and the line can be recreated with a partial-incision method.

The difference is that the buried-suture method has the drawback of easily loosening,

By minimizing the partial incision to about 2 mm,

This is a method that performs surgery with very small incisions of about 2 mm in each area,

so it is natural like the buried-suture method, allows immediate return to daily life,

and usually causes almost no swelling.

If there is a lot of upper eyelid fat, fat removal is also possible.

With the buried-suture method, if swelling occurs,

there is no pathway for tissue exudate to drain, so people who swell a lot may swell as much as they would with an incision method.

With the partial-incision method, tissue exudate drains directly through the incision openings,

so swelling and bruising are less than with any other method, which is a major advantage.

It also has the advantage of not loosening as easily as the incision method.
