1. When the initial surgery was designed too high

The average natural double eyelid height for Koreans is around 6 to 8 mm.

However, because each person’s eye size and shape are different, the height of the double eyelid line must also be adjusted accordingly.

For the design 기준, it is essential to sit up and temporarily create the double eyelid line first, then find the line that best suits one’s own eyes.

However, designing while lying down makes it difficult to find a suitable line, and because the design is based on the tarsal plate, there is a higher chance of creating a large double eyelid line.

2. When the skin is thick

In such cases, the double eyelid line should be set lower than normal, or fat grafting should be done first before double eyelid surgery to restore a normal eye shape, and then double eyelid surgery should be performed a few months later in order to create a satisfactory result.

Why is it difficult to reduce the double eyelid line with a standard surgical method?

When the skin is excised and simply sutured,

you must understand what kind of result will follow.

In cases of a high double eyelid line, the incision is often 8 mm or more from the lash line.

This is usually reduced to around a 5 to 6 mm line, and to do so, about 2 to 3 mm of skin is typically removed, including the existing scar line.

If only excision and suturing are done, the skin below the excision line will be pulled upward and stick to the skin above the line, resulting in a large line of 8 mm or more again, so the surgery will fail 100%.

Therefore, the key to the surgery is to pull the skin above the excision line downward.

For this, tissue repositioning must be performed, and to reposition the tissue, the area between the levator aponeurosis and the orbital fat must be accurately dissected so that the tissues can be rearranged and the line reduced.

What methods are available for reducing a thick double eyelid line?

First, the method using full incision

① The scar is excised.
② The area between the levator aponeurosis and the orbital fat is precisely dissected so that tissue repositioning can take place.
③ Fat is advanced to prevent adhesion of scar tissue after the tissue is dissected.
④ In order to prevent the skin below the excision line from being pulled upward, the skin below the excision line is firmly fixed to the tarsal plate.
⑤ A levator shortening procedure is performed so that the eye opens a little wider. The more the eye opens, the more the double eyelid line can be reduced.
⑥ A firm layered suture is performed through the tissue to complete the tissue repositioning and prevent the tissue from being pulled upward.

① Sedation and local anesthesia are administered.

⑤ Tunneling is performed at the treatment area to secure space for grafting.
⑥ Precise grafting is performed into the orbital fat layer of the sunken eyelid.
⑦ Molding and retunneling are performed to adjust the contour of the eyelid and the height of the double eyelid line, then the procedure is completed.

Third, the method using partial incision

If a temporary double eyelid line is created during the examination and the line becomes natural while the existing line disappears, correction may also be possible with partial incision.

In cases of naturally occurring double eyelids or thick double eyelid lines that developed after buried suture surgery, there are cases where correction is possible without necessarily having to use incision surgery, depending on the degree of adhesion.
