What is under-eye fat grafting?

This is a procedure performed when the under-eye area looks unattractive because the area is sunken or has hollow wrinkle lines that create dark circles. Fat is harvested with a fine suction cannula from the lower abdomen or thighs, then grafted into the under-eye area using a micro fat grafting cannula to correct under-eye hollowness or wrinkle lines and brighten the skin tone.

How can you make the area around the eyes look brighter and younger?

Above the eyes * A double eyelid surgery is usually performed to make the eyes look larger visually. * Fat grafting is performed on sunken eyelids to restore a hollow eye appearance.

What are the types of under-eye dark circles?

What causes dark circles?

▶ Under-eye hollowing (depression) - When the under-eye area becomes hollow, a shadow effect occurs, making it look dark.
▶ Color of the orbicularis oculi muscle under the eye - All muscles have a red color, so the orbicularis oculi muscle is also red.
▶ High skin density - The higher the skin density, the darker the skin appears and the more fine wrinkles there are.

What is the principle of correcting dark circles with micro fat grafting?

Fat grafting corrects under-eye hollowness and reduces the dark shadow effect.
By expanding the skin, fine wrinkles and skin melanin pigment can be made lighter.
By laying yellow fat beneath the skin, dark red coloration can be diluted and lightened.

Beneath the skin layer under the eyes lies the red orbicularis oculi muscle layer, so the main cause of dark red circles is that the color of this muscle layer is projected through the skin.

The principle of the procedure is to precisely graft yellow fat into the muscle layer or just above it so that the red color cannot project outward, allowing skin-tone color to show through instead of red.

For this procedure to be performed, the under-eye area must be somewhat hollow.

When can fat grafting be effective?

It is suitable when the under-eye area is somewhat hollow.

If the under-eye area is not hollow but instead protruding, the orbital fat under the eye should be removed first, followed by fat grafting. If fat grafting is done when the area is protruding or flat, the under-eye area may appear even more protruded, leading to a counterproductive result.

The process of under-eye fat grafting

① Fat is harvested from places where a lot of fat is stored, such as the lower abdomen, waist, or thighs.

• Because anesthesia is performed with local anesthesia (sedation pain-free anesthesia), there is no pain at all.

• A fine liposuction cannula is used to collect the fat into a syringe.

② The collected fat is centrifuged to remove impurities (anesthetic, oil, blood, etc.).

③ Oil is removed through a two-step purification process.

Only when nearly all the oil has been removed can it truly be called concentrated fat.

④ The purified fat is placed into a fine 1cc syringe.

⑤ A micro fat grafting cannula is inserted along the planned design line under the eyes, and the grafting is performed.

Understanding the anatomy of under-eye dark circles and fat grafting

There are two causes of dark circles mentioned above.

They are dark circles caused by shadows from under-eye hollowing and dark circles caused by melanin pigment deposition in the skin.

To correct them, micro fat grafting is performed to eliminate the shadow effect and allow more light to enter, and yellow fat is grafted between the orbicularis oculi muscle layer and the skin to make the black color blend more toward skin tone and recover.

The goal is to make the skin-tone color show through with fat instead of black color.

The success of the procedure depends on how softly and precisely an appropriate amount of fat is grafted.

Understanding the anatomy of under-eye hollow wrinkles and fat grafting

The under-eye hollow wrinkle shown above is a wrinkle line in a nasolabial fold-like shape with no sagging skin at all.

To correct it, fat is grafted beneath the wrinkle line so that the wrinkle is pushed up to the level of the surrounding tissue and smoothed out.

Under-eye fat grafting must be performed delicately without any incision in the skin using a micro fat grafting cannula, and the success of the procedure depends on whether an appropriate amount of fat is evenly grafted.

What are the effects of fat grafting for dark circles?

Dark circles refer to the phenomenon in which the skin under the eyes becomes dark or dark reddish, making the eyes look dim.

In some cases, the skin color changes; in others, the skin wrinkles tighten excessively, producing many fine wrinkles that make the area look dark.

In still other cases, the red muscle layer beneath thin skin is visible, making it appear red.

Although fat grafting cannot change skin color, in cases where the dark circles are caused by skin tightening like a rubber band, creating aegyo-sal can expand the skin and make the skin tone appear lighter. In cases of dark reddish circles where the muscle layer shows through, grafting fat beneath the skin creates a color-complement effect between the yellow fat and the dark blue-black tone, making the skin appear lighter.

What are the effects of fat grafting?

How many times fat grafting should be done depends on the survival rate of the fat.

The survival rate varies from person to person.

For fat grafting, one to a maximum of about two sessions at intervals of around three months can produce a satisfactory result.

In the under-eye area, the fat survival rate is relatively high compared with other areas, so many people are satisfied after just one session.

Is fat harvested again for a second fat grafting session?

When fat is harvested from the lower abdomen or thighs, enough fat is collected for grafting, and any fat left over is not discarded but stored sterile in a freezer.

This stored fat can be kept for about 10 months, so if you return within 10 months, you can receive another procedure without harvesting fat again.

What are the side effects of fat grafting?

The side effects are only slight swelling and bruising.

Other than that, there can be said to be no side effects.

Some people worry that the contour under the eyes may not remain smooth and may become uneven, but this is related to the practitioner’s experience and skill.

Therefore, under-eye fat grafting requires extensive experience and know-how, and it is absolutely not a procedure that anyone can perform.

Under-eye procedures can achieve good results only when performed delicately and with an appropriate amount of fat distributed evenly.

The success of the procedure depends on how properly the practitioner performs it.

For this purpose, subcutaneous tunneling is performed, and a micro fat grafting cannula is used so that no incision is made under the eye and the appropriate amount is precisely grafted into the hollow area to achieve good results without side effects.

If an awkward shape occasionally appears, the practitioner must also have the ability to correct it immediately.

What are the methods other than fat grafting?

​​Of course, various fillers used in plastic surgery (Juvederm, Restylane, Perlane, AlloDerm, SureDerm, Juvederm, Artecoll) are available and in use.

However, these filling materials each have specific uses.

They are mainly used for fine wrinkles around the eyes, between the eyebrows, or on the forehead, or for small sunken scars such as chickenpox scars.

If they are used incorrectly to correct under-eye hollowness or dark circles, there is a high risk of side effects, and these filling materials should not be used to correct dark circles.

For correcting under-eye hollowness or dark circles, micro fat grafting is the only method that can be effective.

In addition, various procedures are performed in dermatology clinics to correct dark circles, but the truth is that the results are not very satisfactory.

It is difficult to expect meaningful results from representative whitening treatments and vascular laser treatments.

Simply removing only the under-eye fat can cause the under-eye area to hollow out, worsening the dark circles and creating a counterproductive effect, so fat should not be removed carelessly.

Since dark circles caused purely by skin color are also rare, whitening treatment has little effect. Also, the effect of whitening would mean turning dark skin into white skin, which is like saying you can turn a Black person into a White person, so realistically it is impossible.

Vascular laser treatment also cannot change the color of the dark red muscle layer under the eyes, so no treatment effect can really be expected.

Because there is no realistic way to change the skin color and the color of the red muscle layer, grafting yellow fat between the two layers is empirically one of the good ways to make the under-eye area’s color closer to skin tone.

Micro fat grafting for under-eye aegyo-sal procedures

Aegyo-sal is a colloquial term for a procedure that creates a slightly bulging image just below the lower eyelashes in order to emphasize an under-eye area that looks flat and unattractive and make the eyes appear more refined.

The relationship between dark circles and aegyo-sal

When dark circles are distributed directly below the eyelashes, fat can be grafted into this area to create aegyo-sal. During this process, the dark skin expands and the yellow fat fills in, which alleviates the dark circles.

It also creates aegyo-sal, changing the eyes to look more attractive.

However, if aegyo-sal is created in an eye shape where the under-eye area is sunken, the shadow effect beneath the aegyo-sal becomes stronger and the under-eye area becomes darker, so aegyo-sal should only be performed on people for whom it is truly suitable, after an accurate examination.

Examples of correction with micro fat grafting for aegyo-sal

How should you care for the area after aegyo-sal treatment?

Aegyo-sal is one of the areas in under-eye fat grafting where bruising occurs easily, so careful bruising management is necessary.

From the day after the procedure, the bruised areas should be carefully covered with skin-toned cosmetics.
Daily life is possible from the day after the procedure, and if swelling does not go down well around one week later, an injection to reduce swelling can be helpful.
After four days, it is fine to sweat in a sauna or jjimjilbang.

Another way to create aegyo-sal

□ Filler products (Restylane, Juvederm, Aquamid, Artecoll)

There are many types of commercially available filler products.

They were generally developed to improve fine wrinkles, so if they are used appropriately for that purpose, there is usually no major problem.

However, they are not suitable for use in large amounts.

Also, some products can easily cause inflammation, which may lead to serious side effects.

Because almost all filler products are colorless and transparent, when used to create aegyo-sal the under-eye area becomes bulging and translucent like a blister, often creating an awkward contrast with the surrounding skin tone.

Since there is concern about side effects and unnatural color when creating under-eye contours, this is not a recommended method.

□ A method using fat repositioning

Some clinics suggest creating aegyo-sal with fat repositioning.

It is impossible to create satisfactory aegyo-sal with fat repositioning.

Aegyo-sal should protrude smoothly just below the eyelashes, but orbital fat cannot be forced toward the eyelash line to create a natural aegyo-sal contour.

In most cases, the shape of aegyo-sal is barely formed, or the protrusion appears not directly beneath the eyelashes but lower down, making the shape awkward.

Caution is needed when trying to create aegyo-sal with fat repositioning.

□ A method using AlloDerm (artificial dermis)

Some people also create aegyo-sal using solid artificial dermis such as AlloDerm or SureDerm, but this is not a recommended method because it requires incisions in the skin, may leave scars, and does not produce a natural shape but instead an artificial appearance.

In the case of liquid AlloDerm, the cost is high and the absorption rate is high, so results are often insufficient when a substantial projection is desired.

There are cases where fillers or AlloDerm are used instead of fat grafting.

Fillers such as Restylane are transparent materials, so

after the procedure the under-eye area becomes pale like a blister with no color at all,

the shape looks awkward,

and because it is a foreign material, the shape may change and there is a risk of it sinking downward.

AlloDerm grafting through an incision is an overly aggressive procedure that leaves scars,

and the shape also does not come out naturally.

Because of this, many patients visit our clinic to dissolve the filler

and receive fat grafting again.

Therefore, empirically, fat grafting seems to be the best method.

The advantages of fat grafting are

that there are no side effects because it is the body’s own tissue,

the shape is natural,

and you can return to daily life right away.

so you do not need to worry about whether it will suit you.
