Correcting a Thick Double Eyelid Line

What are the problems with a thick double eyelid line?

A double eyelid with a line that is too thick can look artificial and awkward, and it can give the impression that the surgery was done recently.

What causes a thick double eyelid line?

◆ When the line was designed too high during the first surgery

The average height of a natural Korean double eyelid is about 6 to 8 mm.

However, because each person’s eye size and shape are different, the double eyelid line must also be adjusted accordingly.

For the design, the priority is to temporarily create the double eyelid line while the patient is seated and find the line that best suits their eye shape.

However, when designing in the lying-down position, it is difficult to find a suitable line, and because the design is based on the tarsal plate, there is a high chance of creating a larger double eyelid line.

◆ When the skin is thick

Even if the double eyelid line is set at the same height as other people’s, thick skin can make the line look thicker and awkward.

When the skin is thick, it is often necessary to set the line slightly lower than the average height.

◆ When there is ptosis

If the eyes are droopy due to ptosis, the double eyelid line can look larger even when it is at the same height as other people’s.

This is because the eyes do not open fully.

When ptosis is present, the double eyelid line should be set lower, or ptosis correction should be performed at the same time.

This happens because as the eyes sink, the skin pulls the double eyelid line upward as well.

In cases of sunken eyelids, the double eyelid line should be set lower than in normal cases, or fat grafting should be performed first before double eyelid surgery to create a normal eye shape, and then double eyelid surgery should be done several months later in order to achieve a satisfactory double eyelid.

Why is it difficult to reduce the double eyelid line using the usual method?

In cases of a high line, the incision line is often 8 mm or more from the eyelashes.

This usually needs to be reduced to a line of about 5 to 6 mm, and to do that, it is common to excise about 2 to 3 mm of skin, including the existing scar line.

When the skin is excised and simply sutured, you must understand what results can occur.

If only excision and suturing are done, the skin below the excision line will be pulled upward and adhere to the skin above the line, creating once again a large line of 8 mm or more, so the surgery will fail 100%.

Therefore, the key to surgery is pulling the skin above the excision line downward.

To do this, tissue repositioning must be performed, and for tissue repositioning, the space between the levator palpebrae superioris and the orbital fat must be precisely dissected so that tissue repositioning can occur and the double eyelid line can be reduced.

1. Method using the complete incision technique

① The scar is excised.

3. Method using partial incision

In cases of a thick double eyelid line that occurred after a naturally formed double eyelid or after buried-suture surgery, there are cases where correction is possible without necessarily performing full incision surgery, depending on the degree of adhesion.

During examination, if a temporary double eyelid line is created and the existing double eyelid line disappears while the new line forms naturally, correction is also possible with the partial incision method.

◆ What is the difference between extreme double eyelid revision and ordinary double eyelid revision?!

In ordinary double eyelid revision, some steps may be simple or omitted depending on the condition, but in extreme double eyelid revision, when there is not enough skin redundancy or the existing scar line is too high and too prominent, making surgery almost impossible, the scar is not excised; instead, subcutaneous dissection is performed and a new line is created underneath it.

This means that all available techniques for restoring a natural double eyelid are brought together in double eyelid revision surgery, including multi-layer tissue dissection and repositioning to release scar adhesion, insertion of micro fat tissue, levator shortening, and dissection and partial excision of the orbicularis muscle below the incision line.

◆ Plan for extreme double eyelid revision

① If tissue or skin is insufficient, measure the limit to which it can be stretched and then stretch it.
