Correcting Asymmetrical Double Eyelid Lines

The causes of asymmetrical double eyelid lines are very diverse.

There are also various methods for correction.

What causes asymmetrical double eyelid lines?

1. When the height of the double eyelid crease is different
2. When one eye has ptosis
3. When one double eyelid line has loosened
4. When the skin on one side has loosened
5. When the eyelid has sunken because of insufficient orbital fat, causing the line to be pulled upward
6. When it has been pulled upward due to scar adhesion

How are asymmetrical double eyelid lines corrected?

★ When the height of the double eyelid crease differs after the buried-suture method ★

If the surgery was done with the buried-suture method, there is a high chance that it will loosen again even if it is redone with the buried-suture method.

A new line is created at the desired position using the partial-incision method.

As the new line is formed, the existing double eyelid line naturally smooths out and disappears.

However, if the existing line is too distinct,

it cannot be corrected with the partial-incision method, so in some cases the complete-incision method is needed.

★ When the height of the double eyelid crease differs after the complete-incision method ★

1. When adjusting a smaller line to match a larger line

If one line is lower and the smaller line needs to be made larger,

a new double eyelid is created above the existing double eyelid line.

This is generally corrected with the partial-incision method,

and if the existing scar line is dark, it is corrected with the complete-incision method.

2. When adjusting a larger line to match a smaller line

To reduce a larger line created with the complete-incision method, the complete-incision method is performed again.

The existing scar line is excised and removed,

and a new double eyelid is created to match the opposite side.

To reduce a larger line, the tissue is dissected and fully repositioned.

★ Correction method when one eye has ptosis ★

A procedure is performed to make the eye with ptosis open wider.

Using the complete-incision method, the levator muscle (the muscle that lifts the eyelid) is pulled and

shortened, then attached to the tarsal plate using levator shortening surgery.

★ When one double eyelid line has loosened ★

If the line on one side has loosened, the loosened side’s double eyelid line is corrected

using the partial-incision method or the complete-incision method.

If the skin is very thick or the scarring is not severe,

it is standard to first correct it with the partial-incision method,

and if it still loosens, correction is performed with the complete-incision method.

★ When the skin on one side has loosened ★

If the double eyelid line height is the same but asymmetry occurs due to sagging skin,

then correction is performed by removing the sagging skin on the affected side with the complete-incision method.

★ When both sides are asymmetric and both lines look awkward ★

If the left and right sides are asymmetrical and both lines are unattractive and awkward,

both sides need revision surgery.

The line heights are designed equally, the existing scar lines are removed,

and the tissue is fully repositioned to create new lines.

★ When one eyelid has sunken, causing the line to be pulled upward and become asymmetrical ★

If the eyelid sinks because of insufficient orbital fat,

this causes the line to be pulled upward and appear larger.

In such cases, a double eyelid revision surgery is not necessary,

and it can be corrected simply with micro fat grafting.

Micro fat grafting is a simple procedure that injects fat with a syringe.
