Q. Bulbous nose surgery and alar reduction surgery look similar. Is there a difference?
Bulbous nose surgery is a procedure that reduces cases where the nose tip is large and thick. Usually, when people refer to a bulbous nose, they mean that the tip is not clearly defined and is large. In the case of a bulbous nose, the alae are also wide, so alar reduction surgery and bulbous nose surgery are often performed together. To distinguish them, bulbous nose surgery can be seen as reducing the blunt appearance of the tip, while alar reduction surgery reduces wide nostrils.
Q. My nose is very blunt and fleshy, and I want rhinoplasty, but I heard that it is difficult to improve with a typical nose surgery. How can I increase satisfaction?
Usually, when the nose area has a lot of flesh and appears thick, people think it is simply because there is too much flesh. But in reality, a large portion of the issue comes from the cartilage at the tip being large and spread out widely. In such cases, the first step is usually a surgery that ties the cartilage together to make the nasal tip cartilage more defined. After that, it is better to perform an additional procedure to build up the cartilage further.
And in cases where the nose is fleshy, a procedure may also be performed to remove the perichondrium, muscle-fiber tissue, and fat inside the nasal skin to make the overall contours more distinct.
Q. Is it possible to use autologous tissue in bulbous nose surgery? Are there concerns about side effects from artificial implants?
In bulbous nose surgery, autologous tissue is also possible. Usually, in the case of a bulbous nose, the surgery is performed by tying the nasal cartilage at the tip, and then ear cartilage or septal cartilage is added to increase the overall height. For the bridge of the nose, implants are usually used to raise the height, but this area can be raised with autologous tissue using the patient's own dermal-fat graft. However, with dermal-fat grafting, the definition may be less distinct than with an implant, so it is effective to combine it with osteotomy to make the bridge look higher.
It cannot be said that artificial implants have no side effects. However, if the surgery is performed to minimize side effects and careful follow-up is provided after surgery, with proper response if any problems arise, most cases can be completed without worrying about side effects.
Q. Can a man with a high bridge and a large nose also improve his impression with bulbous nose surgery?
Usually, when the bridge is high and the nose is large, the impression given is more that the nose is big rather than that it is high. In such cases, even if the bridge looks high from the side, the nose bone and nostrils are wide, so it does not give the feeling of a high nose. In these cases, an implant may be used for the bridge, or if the bridge is already sufficient, only the wide nasal area is narrowed through osteotomy without using an implant. The tip is also addressed by tying the wide cartilage to reduce the size of the tip.
Usually, when the nose is large, the alae are also wide, making the face look dull, so better results can be achieved by reducing the wide nostrils at the same time through alar reduction surgery.
