Bulbous Nose Surgery: How to Increase Satisfaction and How It Differs from Tip Refinement

In the past, it was called a “bulbous nose” because it was thought to be a nose shape that brought wealth, but recently it has come to symbolize a rough and old-fashioned image because the nose tip looks large, round, blunt, or wide. That is why many people want to improve the bulbous nose that makes their impression look sulky and transform it into a slender, refined nose in order to improve the complexes they have felt in daily life and upgrade their appearance.

Q. Bulbous nose surgery and alar reduction surgery seem similar. Is there a difference?

Bulbous nose surgery is an operation that reduces a nose tip that is large and thick. Usually, when people say a bulbous nose, they mean a nose tip that is not well defined and is large. In the case of a bulbous nose, the alar base is also wide, so alar reduction surgery and bulbous nose surgery are often performed together. If we distinguish them, bulbous nose surgery reduces the blunt appearance of the nose tip, while alar reduction surgery reduces the width of the alar base.

Q. My nose is very blunt and fleshy, and I want nose surgery, but I heard it is difficult to improve with a general nose operation. What should be done to increase satisfaction?

Usually, when the nose area is fleshy and thick, people tend to think it is simply because there is a lot of flesh. But in reality, a large part of it comes from the cartilage at the nose tip being large and spread out widely. In such cases, the first step is a surgery that binds the cartilage to make the nose tip cartilage more defined. After that, it is better to perform an additional procedure that adds more support to the cartilage.

And if the nose is fleshy, surgery may also be performed to remove the perichondrium, muscle-fiber tissue, and fat inside the nasal skin so that the overall contour becomes more defined.

Q. Is it possible to use your own tissue in bulbous nose surgery? Are there concerns about side effects from artificial implants?

In bulbous nose surgery, autologous tissue can also be used. Usually, in cases of a bulbous nose, surgery is performed by binding the nasal cartilage at the tip, and the overall height is increased by adding ear cartilage or septal cartilage. For the bridge of the nose, implants are usually used to increase height, but this area can be augmented with autologous tissue using the patient’s own dermal-fat graft. However, since a dermal-fat graft tends to look less defined than an implant, it is effective to combine it with osteotomy so that the bridge of the nose appears higher.

With artificial implants, it cannot be said that there are no side effects. However, if surgery is performed to minimize side effects, careful observation is done after surgery, and appropriate measures are taken if a problem occurs, then in most cases the procedure can be completed without worrying about side effects.

Q. Can bulbous nose surgery improve the impression in men who have a high nasal bridge and a large nose?

Usually, when the bridge of the nose is high and the nose is large, it gives the impression that the nose is big rather than simply high. In such cases, even when viewed from the side, the nasal bone and alar base are wide, so the nose does not give the impression of being high despite its height. In these cases, an implant may be used for the bridge, or if the bridge is already sufficiently high, the wide nasal area is narrowed through osteotomy without using an implant. The nose tip is also reduced by binding the wide cartilage to make the tip smaller.

Usually, when the nose is large, the alar base is also wide, which makes the face look dull, so better results can be achieved by reducing the wide alar base together through alar reduction surgery.
