Depressed scars with an uneven surface are difficult to cover with makeup and may appear more prominent depending on the direction of the lighting. What Are Depressed Scars?

Depressed scars begin when inflammation extends down into the dermal layer. When collagen and elastic fibers are damaged, the body creates new fibrous tissue to fill the empty space. If the tissue does not grow evenly, bands form that pull the underside of the skin downward. The surface is unable to rise and becomes fixed in a depressed state, completing the visible indentation.

The appropriate approach also varies depending on the shape of the scar. Scars are classified as rolling scars, which have gently sloping and broad curves; boxcar scars, whose walls descend vertically; and ice-pick scars, which are narrow in diameter but deep. If the scar becomes less visible when the skin is pulled to both sides, this indicates strong adhesions underneath the skin:) If there is no change even when the skin is pulled, it means that the amount of dermal loss itself is substantial.

What Are Depressed Scars?

Subcision is a method that directly cuts the adhesions holding the scar down. After applying local anesthesia, a cannula or special needle is inserted around the scar and moved horizontally beneath the skin to cut the fibrous bands. The extent of the cutting is determined after first assessing the depth of the scar and the degree of adhesion. Once the tension is released, the depressed surface rises, reducing the difference in height from the surrounding skin.

A healing response begins at the cut area along with minor bleeding. New collagen fills the area during this process, allowing adhesions to be released while the tissue is replenished at the same time. Changes are relatively noticeable in rolling scars with gently sloping borders and boxcar scars with broad walls. However, bruising and swelling may last for approximately 5 to 7 days, so it is best to schedule the treatment accordingly if you have an important event coming up:)

What options are available if the anesthesia and recovery period are a concern? Mirajet is a device that delivers medication using only an instantaneous, high-pressure jet without inserting a needle. Because the emitted stream is thinner than a strand of hair, it causes less irritation when passing through the epidermis, and it also greatly reduces the raised, embossed appearance that can occur immediately after injection. Another characteristic is that the medication spreads across different layers instead of clumping in one spot.

After releasing the adhesions, the remaining step is to fill the empty dermis. Juvelook is a skin booster containing both PDLLA particles and hyaluronic acid. Immediately after injection, the hyaluronic acid draws in moisture and creates initial volume. This volume is absorbed over time, but the empty space is then addressed by the next process.
