Today, I’d like to introduce the case of a man in his late 30s who resolved his concerns with double chin muscle-tying surgery.
People often think a double chin only appears when the whole body is overweight,
but even someone like this patient—176 cm tall and 76 kg, far from obese—can have a particularly prominent area of fat under the chin.
Because he did not consider himself to be heavy,
he tried dieting, but the double chin did not go away,
eventually deciding on surgery and visiting our clinic.
Patient’s Concerns and Condition
This patient said that when taking a front-facing photo, he looked more or less fine,
but in side profile, the bulging area beneath the jawline kept showing and caused stress.




People around him also often said,
“You don’t look overweight, but your chin area bothers me a little,”
so every time he looked in the mirror, it bothered him and became a complex about his appearance.
Moreover, because of his job, he had to meet people often,
so he strongly wanted nothing more than to get rid of the double chin.
What Is Double Chin Contouring Surgery?
In many cases, simple liposuction alone is not enough,
so I recommended double chin contouring surgery.
The key here is “how the platysma muscle is tied” and
“how precisely the structures that form the double chin are removed and refined.”
- Muscle Tying
- Most double chin surgeries tie only the center of the platysma,
which can allow sagging to recur on both sides.
- For this patient, we used a method that firmly pulled the platysma not only in the center but also on both sides.
In other words, we tightly secured it in all directions so the double chin would not sag sideways either.
Not only that, rather than simply pulling to both sides,
we also push inward while pulling.
In other words, it is a very powerful surgical method that can make the area sink further beneath the jawline.
- Removal of Salivary Glands, Grape-Cluster Fat, and Part of the Digastric Muscle
- In some patients, the area under the chin looks protruded because the salivary glands are enlarged,
or because there is a lot of “grape-cluster fat” (lumpy fat) that causes significant sagging.
- In this patient as well, by removing the salivary glands, grape-cluster fat,
and part of the digastric muscle,
we made the area under the jawline slimmer and sharper.
- Use of an Ultrasound Liposuction Device
- To precisely suction the fat layer remaining between the outer surface of the platysma and the skin,
we used an ultrasound liposuction device (LSSA, Liposound).
- Compared with regular liposuction, it is a higher-end device that relatively reduces bleeding and bruising,
and can remove fat more clearly and meticulously.

When viewed from the front, side, or from below, the fat under the chin appears clumped together.
The jawline looks blurred, making it look like a “double chin,”
and the overall impression of definition is quite weak.
This is the period when swelling and bruising gradually start to appear, so the area under the chin may look swollen,
but he said there was no major pain other than a pulling sensation.
Of course, it is hard to expect a perfect line by Day 1 alone,
but he said, “I can see that the double chin is already reducing.”
After about 1 to 2 weeks, a more natural jawline can be seen.
Because the results are visible right after surgery,
even if the procedure is done over a weekend, the recovery is usually not so inconvenient that daily life becomes difficult.
Recovery and Care
- Swelling & Bruising
- Even if the surgical area is not large,
the under-chin and neck area moves often,
so bruising can last a long time.
- For the first 3 days, apply cold compresses briefly and repeatedly,
and after that, warm compresses are recommended to help reduce the bruising.
- Wearing a Compression Bandage
- To firmly support the neck and jawline,
it is good to wear the compression bandage provided by the clinic for a certain period.
- Some people say it feels stuffy,
but wearing it early on is very important for the result.
- Returning to Daily Life

In most cases, swelling subsides to some extent within 1 week.

It is best to avoid strenuous exercise, saunas, and alcohol for at least 4 weeks.
As this case shows,
even if the bones are not large and the person is not overweight,
certain areas can protrude noticeably, which is a characteristic of a double chin.
When simple liposuction has limitations,
double chin triple muscle-tying surgery, which ties the platysma in multiple directions,
and double chin contouring surgery, which refines the salivary glands, fat, and muscle,
can be a fundamental solution.
Above all, when the jawline becomes sharper after surgery,
confidence about side profile increases significantly.
This patient also could not take a long break, but the swelling went down early,
so he canceled the longer leave he had planned and went back to work earlier than scheduled, saying he was very satisfied with the surgery results haha.
