Hello.
We are The Square Dental Clinic,
which focuses on conservative and restorative treatment.

If it seems like a wisdom tooth cavity has developed,
do you absolutely have to have it extracted?
One of the treatments that many patients are especially afraid of (^^;) is wisdom teeth.

Especially when you are already wondering whether you should extract it or not,
if a wisdom tooth cavity appears along with a dull ache,
it must feel like a bolt from the blue.
At this point, you may be wondering,
whether it is still possible to treat the wisdom tooth cavity and keep using it,
or whether you should hold back your tears and have it extracted.
In this post, we will have an honest conversation about the following three topics:
- Why wisdom tooth cavities form easily
- Advice on extraction for wisdom tooth cavities
- Problems that occur when wisdom tooth cavities are left untreated

Why do wisdom teeth decay so easily?
Wisdom tooth cavities are actually more common than you might think.
This is due to the structural limitations of wisdom teeth.

Because they are the last teeth to emerge and are located deepest in the mouth,
no matter how carefully you brush, food debris and plaque are not completely removed,
making them an ideal place for bacteria to grow.
In addition, when a wisdom tooth does not erupt straight and comes through partially covered by the gum,
small pockets can form between the tooth and the gum, allowing food to get trapped there.
When this happens, it can cause both cavities and gum inflammation, leading to bigger problems.

Cavity treatment vs. extraction
Which is the right choice?
So how should a decayed wisdom tooth be treated?
If the wisdom tooth
① has erupted normally and
② has a good bite with the molars,
then cavity treatment can be considered.
However, in most cases, extraction is recommended first.

Wisdom teeth are difficult to treat precisely because instruments cannot reach them well even when the mouth is opened wide,
and even after going through the trouble of finishing the treatment,
the oral environment that caused the cavity in the first place remains the same,
so the chance of recurrence is very high.
In the end, even if you spend time and money treating the cavity, there are many cases where the tooth ends up hurting again later and needs to be extracted anyway,
so medically speaking, extracting it early and removing the cause can be a much more economical and wise choice.

What happens if you leave a decayed wisdom tooth untreated?
Building on what was mentioned above,
what problems can occur if a wisdom tooth that needs extraction is left alone?

The first is pericoronitis, and it can even progress to cellulitis.

The second is that the healthy molar in front of the wisdom tooth (the second molar) can also decay.
Food trapped between the wisdom tooth and the front molar can cause cavities on the contacting surfaces of both teeth at the same time.

- Key point: According to the latest research published in a world-renowned dental journal in 2025,
if impacted wisdom teeth are left untreated,
the probability of cavities developing in the adjacent molar is
as high as nearly 30%,
and especially when the wisdom tooth is lying diagonally toward the front tooth,
the risk rises to 43%.

In fact, because of a single wisdom tooth, some patients in their 20s have had decay that reached deep into the root area of the front molar,
leading them to have both the wisdom tooth and the healthy molar extracted and then receive implants.

Therefore, if you notice bad odor, swelling, or pain around a wisdom tooth,
it is important not to delay and to visit a dental clinic for appropriate treatment.

In particular, if you visit a clinic with an orthodontics specialist who can accurately determine how close the wisdom tooth is to the inferior alveolar nerve canal,
and how it affects the overall dental alignment and bite,
as well as an integrated dentistry specialist who can also plan for space changes after extraction and conservative treatment for adjacent molars damaged by the wisdom tooth,
you can expect a safer and more successful treatment outcome.
If you are truly afraid of dental treatment and do not have the courage to go easily,
it may also be an option to look into a sedation dental clinic where
“you wake up and the treatment is already over.”
