If your eyes feel heavy and droopy after getting Botox, it may be “ptosis.”
Hello, this is Yonsei Bomvit Dermatology.
After a light Botox treatment to smooth forehead wrinkles, some people are startled by a heavy feeling in the eyes and a sensation that the eyelids are drooping.
Today, we’ll explain the causes of ptosis that can occur after forehead Botox, how to respond, and the points that must be carefully considered depending on age and eye shape.

Causes and management of ptosis after forehead Botox
1️⃣ Why does ptosis occur after forehead Botox?
✔ Botox is a procedure that reduces muscle movement to soften wrinkles.
The forehead muscle (frontalis) also helps pull the eyebrows upward, so when movement in this muscle is reduced, the auxiliary action that helps lift the eyelids can also decrease.
In particular, ptosis is more likely to occur in the following cases:
-
When Botox is injected too widely or too deeply
-
When the muscles across the entire forehead become completely paralyzed and the strength to lift the eyelids is lost
-
When the muscle that lifts the eyelid (levator palpebrae superioris) was originally weak
As a result, the eyelids may feel heavy, sleepy, and droopy, and vision may be obstructed.
2️⃣ Special caution is needed for people in their 50s and older
✔ In particular, for people without double eyelids and those with naturally droopy eyelids,
✔ or for people in their 50s and older who have developed the habit of raising their eyes using their forehead,
the likelihood of ptosis after forehead Botox is much higher.
In these cases, the following must be considered:
-
❌ Avoid the Botox procedure itself, or
-
⬆ Inject only in a limited area as high as possible near the upper forehead boundary (close to the hairline)
→ This is designed to preserve the strength used to lift the eyes while still softening wrinkles to some extent.
At Yonsei Bomvit Dermatology, we evaluate facial expressions, eye-opening habits, and forehead muscle usage patterns before the procedure
and determine the appropriate treatment range for each individual.
3️⃣ Is ptosis a dangerous side effect?
✔ In most cases, it is temporary.
The effects of Botox diminish in 3 to 4 months, and forehead muscle movement gradually recovers, so it improves naturally.
However, there can definitely be inconveniences in daily life, such as feeling visually constricted or having heavy eyelids.
4️⃣ What should you do if ptosis occurs?
✔ Wait for it to improve naturally
In most cases, it gradually improves over time. It usually recovers almost completely after about 2 to 3 months.
✔ Use special eye drops (by specialist prescription)
Using sympathomimetic eye drops such as “iopidine” can stimulate the muscle that lifts the eyelid
and temporarily make the eyes less droopy.
However, because the effect is temporary, it is not strongly recommended.
✔ Visit the clinic where the procedure was performed
Taking into account the Botox injection site, depth, and the individual’s eyelid condition,
additional medication support or follow-up monitoring can be provided if needed.
5️⃣ Yonsei Bomvit Dermatology’s safe forehead Botox design
-
📍 Analyze each person’s eyelid shape, age, and facial expression habits
-
💉 Start with the minimum dose and design the treatment safely, focusing on the upper forehead area
-
🔍 For people without double eyelids or with heavy eyelids, the procedure is not recommended or alternatives are provided
-
🧑⚕️ A board-certified dermatologist performs the procedure directly and monitors the course afterward
Forehead Botox is not simply about looking at forehead wrinkles alone;
it is a delicate procedure that must also consider eye-opening strength, field of vision, and facial expressions.
🧭 In closing
Forehead Botox can create a natural, youthful impression when used well,
but a cautious approach is necessary for people whose eyelids are already heavy, who do not have double eyelids, and especially those in their 50s and older.
This does not mean you should avoid Botox unconditionally,
but it is most important to understand the structure of your eyes and forehead well and design the procedure in a safe way.
