VN revision rhinoplasty: How is the surgery performed?
CASE 01. In cases of tip drooping (a phenomenon in which the tip is raised without considering the condition of the patient’s skin or cartilage, and over time the tip sags), if the tip support structure is solid, additional grafts such as cartilage and dermal tissue can be placed in the tip to supplement the height. If the tip support structure is weak or has collapsed, the support structure can be rebuilt from scratch in a sturdy way using septal cartilage, rib cartilage, etc., and the tip height can be supplemented by additionally grafting ear cartilage, dermal tissue, etc.
CASE 02. In cases requiring silicone correction (when silicone was inserted too high or too low, or when the postoperative nose shape is unsatisfactory, silicone correction is needed), the implant can be replaced through minimal incision to find a more beautiful line and correct it. If both the tip and bridge are unsatisfactory, tip revision surgery can also be performed together with implant replacement to improve the overall harmonious and attractive line.
CASE 03. In cases where the nose appears crooked (when crooked nose correction was not sufficiently achieved in the first surgery, or when the inserted implant has shifted and the nose appears crooked), if the bridge is crooked because the original nasal bone is asymmetrical, the base should be made as symmetrical as possible through osteotomy, hump removal, and cartilage grafting, followed by silicone replacement. If the tip has rotated, the tip support must be straightened again from the start using septal cartilage, rib cartilage, etc., and delicate tip surgery should be performed.
CASE 04. In cases of contracture nose / upturned nose (when severe scar tissue forms due to inflammation after surgery, when hyperplasia of the silicone capsule and contracture occur for various reasons, when the support structure of the nose weakens due to an incorrect surgery, or when the tip becomes shorter or turns upward compared to before surgery), all nasal tissues including the skin, cartilage, and mucosa must be lengthened through delicate dissection and scar tissue removal, and the stable support structure of the nose must be rebuilt through cartilage grafting.
CASE 05. In cases of implant/cartilage visibility (when the skin becomes thin after nose surgery and the implant shows through or turns red, or when a thick implant was inserted), the implant used in the first rhinoplasty can be removed and the bridge can be rebuilt with autologous tissue, or the implant can be covered with dermal tissue and reinserted into the nose to improve the condition.
CASE 06. In cases where the tip appears pinched (when the tip cartilage is tied too tightly during surgery, or when a small, thin-skinned tip is excessively raised), the tip cartilage can be repositioned or the cartilage can be grafted to spread the tip cartilage. In addition, the area around the tip is reinforced with dermal grafting to address the issue.
