we will look at the reasons for considering incisional double eyelid surgery

depending on the condition of the eyes.

How is the incisional method performed?

Incisional double eyelid surgery involves creating an incision line on the eyelid,

adjusting the condition of the skin, fat, and internal tissues,

and forming the double eyelid line.

Because the thickness and amount of fat in the eyelids,

skin sagging, and volume above the eyes vary from person to person,

the correction needed may also differ.

In addition, when revision surgery is performed using the incisional method,

the tissues that have changed as a result of the previous surgery must also be examined.

Therefore, incisional surgery is not performed in exactly the same way for everyone.

: Protruding eyes with multiple creases caused by hollowness above the eyes

The first incisional double eyelid surgery review

is a case involving protruding eyes with hollowness above the eyes.

With insufficient volume above the eyes,

the double eyelid line did not form as a single line,

multiple creases appeared,

and the existing line also looked faint.

With eyes like this, rather than simply creating

a new double eyelid line,

it is important to identify why the hollowness above the eyes

and multiple creases are appearing together.

For this case, incisional double eyelid surgery

was performed along with upper eyelid fat repositioning and fat grafting.

The double eyelid line was formed through an incision,

the fat above the eyes was repositioned,

and the insufficient volume above the eyes

was supplemented with additional fat grafting.

: Does everyone with multiple creases need fat grafting?

The second incisional double eyelid surgery review

also involves multiple creases above the eyes

and a faint double eyelid line.

Then, would fat grafting above the eyes, as in CASE 01, be appropriate?

The surgical method cannot be determined in the same way

based only on the symptom of multiple creases above the eyes.

In this case, after examining the location and distribution of the fat above the eyes,

the existing fat was moved to the necessary position

rather than adding new fat to areas lacking volume.

First, the faint line was made more defined through an incision,

and the position of the fat above the eyes was adjusted

to organize the uneven-looking volume.

Even when multiple creases are present,

the surgical method may differ depending on

the condition and amount of fat inside the eyelids.

That is the difference between these two cases.

: What if you have monolids but a lot of fat in the upper eyelids?

The third incisional double eyelid surgery review

is a case involving monolids and a large amount of fat in the upper eyelids,

which made the eyelids look heavy and the eyes look confined.

When there is a large amount of fat in the eyelids,

it is necessary not only to determine the height of the double eyelid,

but also to examine how the fat and skin will fold

when the line is created.

For this case, incisional double eyelid surgery

was performed together with the removal of excess upper eyelid fat.

While forming the line through an incision,

unnecessary fat was removed

so that the eyes would not look confined after surgery.

: What if there is almost no fat in the upper eyelids?

The fourth incisional double eyelid surgery review

is a case in which there was almost no fat in the upper eyelids,

causing significant hollowness above the eyes

along with pronounced fine and multiple creases.

With this eye shape, it would not have been easy

to form a line using the non-incisional method,

so the surgery had to be performed using an incision.

In addition, because the insufficient volume also needed to be improved,

incisional double eyelid surgery,

upper eyelid fat repositioning,

and fat grafting above the eyes were necessary.

The existing fat was repositioned to the necessary areas,

and fat grafting was performed where volume was lacking,

supplementing the volume above the eyes

while forming the double eyelid line.

: What if the double eyelid line has loosened?

The final incisional double eyelid surgery review

is a case of eye revision surgery.

The line created during the previous surgery had loosened

and become faint.

For eye revision surgery, it is necessary to examine not only the location of the existing line,

but also adhesions and tissue condition resulting from the previous surgery,

as well as the skin and fat that remain,

while identifying the issues with the first surgery.

In the case above, eye revision surgery was performed through an incision

to increase fixation strength and help prevent the line from loosening again.

Incisional double eyelid surgery may be considered for eyes like these.

So far, we have looked at five cases of incisional double eyelid surgery reviews.

Although the condition of the eyes differed from case to case,

the common point was that the entire condition of the eyelids was examined

rather than focusing only on the double eyelid line,

and surgery was performed using the incisional method.

Then, in what situations may incisional double eyelid surgery be considered?

① When the eyelid skin is thick or there is a large amount of fat

② When multiple creases and a faint double eyelid line are present

③ When the eyelids are significantly droopy

④ When correction is needed because the strength for opening the eyes is weak

⑤ When the double eyelid line is thick

However, meeting one of the conditions above

does not necessarily mean that incisional double eyelid surgery is required.

The surgical method should be determined after comprehensively examining

the eyelid skin, fat, cause of the multiple creases, sagging, and history of previous surgery.
