An accident in which a front tooth breaks
can happen to anyone.
That is why, in such cases,
a treatment plan must be carefully made
depending on the extent of the fracture.
In the case I will explain today,
a severe traumatic fracture occurred in tooth #22
(maxillary left central incisor).

More than two-thirds of the crown was lost,
and the fracture was so extensive that
less than one-third of the crown remained.
The pulp chamber was fully exposed,
leaving the pulp tissue directly exposed to the external environment,
and as a result, bacterial infection was progressing,
with severe pain and suspected pulp necrosis.
When such an extensive crown fracture occurs,
it is an emergency that requires immediate intervention,
and a prompt, systematic treatment plan is needed
to prevent the infection from spreading
while preserving the natural tooth.
If a front tooth is broken to this extent,
it is already contaminated by bacteria
and an inflammatory response is likely to have begun,
so at Seoul National University Entrance Dental Clinic,
we determined that root canal treatment was needed as soon as possible.

At Seoul National University Entrance Dental Clinic,
to briefly explain the treatment process,
the infected pulp tissue is carefully removed
over several sessions.
Using mechanical and chemical methods,
the area was repeatedly cleaned and disinfected,
and we proceeded thoroughly step by step
so that the filling material could fit properly all the way to the root tip.
Only then can the likelihood of inflammation
or reinfection be reduced.
One more point to mention through today’s case at Seoul National University Entrance Dental Clinic is that
if the amount of remaining tooth structure is very small due to an extensive crown fracture,
there is not enough structure to support a crown,
so a post placement is necessary to solve this.
A post is a method used when the crown portion defect of a tooth
that has completed root canal treatment is large,
by additionally placing a material inside the root canal that acts as a pillar
so that the root and crown can be stably fixed.
Through this,
retention for the upper restoration is increased,
and it can withstand strong chewing forces for a long time.
After post placement,
a resin core is used
to complete the abutment shape for the crown,
and finally the prosthesis fabrication begins.
Front teeth are an area where esthetics are extremely important,
and they significantly affect a patient’s smile and confidence.
That is why at Seoul National University Entrance Dental Clinic,
we believe that not only functional recovery,
but also esthetic results that are indistinguishable from natural teeth,
are very important.

As a way to achieve this,
a zirconia crown can be selected,
and while offering excellent strength and biocompatibility,
it can also reproduce a shade and translucency similar to natural teeth.
In addition, we made sure the boundary between the tooth and the gum (the margin)
could connect naturally.
If this area is fabricated inaccurately,
gum inflammation or esthetic problems may occur.

If you look at the final crown placement at Seoul National University Entrance Dental Clinic,
the shade and shape are indistinguishable from natural teeth,
and the natural continuity of the boundary between the tooth and the gum
can also lead to high patient satisfaction.
Even if a front tooth is severely broken or chipped,
treatment is still possible.
However, in such cases, the long-term prognosis can change depending on
what kind of plan is made and how the approach is taken,
