I think you may have heard of at least one of these.
These are the names of implant companies, and also types.
Each company’s fixtures have different characteristics,
with their own advantages and disadvantages.
Today, I’d like to explain a case using a fixture from Osstem.

This is a clinical photo for better understanding,
and it was written based on objective information.
- Fractured lower front tooth

If you look at the photo shown at the Bongcheon-dong dental clinic,
a lower front tooth has fractured.
The crown has snapped clean off,
and the broken fragment is gone.

Even with just a slight opening of the mouth,
the fracture is immediately visible,
so daily life must have been difficult right away.
It would have drawn a lot of attention,
and with speech escaping through the gap, food getting stuck, and saliva splashing out,
there must have been many awkward situations.
- Radiographic X-ray examination
At the Bongcheon-dong dental clinic,
when there is a problematic tooth inside the mouth,
we recommend taking a radiographic X-ray.
The image shown here is a type of radiograph called a *panorama,
and it allows us to examine not only all the teeth in the oral cavity,
but also the height of the alveolar bone,
and anatomical structures such as the temporomandibular joint, maxillary sinus, and inferior alveolar nerve.

In this case, after taking the panoramic image,
the fractured mandibular incisor stands out,
but what needs a closer look here is the apical inflammation.
It seems the tooth had previously undergone root canal treatment
and had been covered with a crown,
but over time, bacteria entered through microscopic gaps that formed inside the prosthesis and abutment,
and inflammation appears to have developed at the root apex.
Of course, because the patient had already completed root canal treatment,
they may not have felt pain,
but if there is a large apical abscess like this,
there is a very high chance that extraction will be necessary.
Otherwise, the inflammation can gradually worsen,
melting the surrounding gum bone
and negatively affecting the nearby adjacent teeth.
- Diagnosis: extraction of the four mandibular incisors
From the perspective of the Bongcheon-dong dental clinic,
including the fractured tooth,
it would be best to extract a total of four lower front teeth
that have root-apex inflammation.
Even if we were to treat only the broken front tooth right away,
if the tooth next to it has inflammation,
it will interfere with implant osseointegration,
which lowers the success rate of the surgery.
That is why, in situations like this,
a sufficient conversation between doctor and patient is especially important.
You may have come in thinking only one tooth would need treatment,
but if you suddenly need four extracted,
you may feel worried.
However, because we need to consider the long-term prognosis as well,
and provide treatment that the patient can use well for a long time,
for this case,
we decided to extract the clearly inflamed mandibular four incisors,
then place implants to restore both esthetics and chewing function.
- Osstem SOI fixture
At the Bongcheon-dong dental clinic,
I will explain Osstem SOI.
Among the Osstem implant lineup,
it is a premium line with the best osseointegration performance,
and it is a product with differences in initial stability and recovery speed.
With carbon adsorption prevention technology,
it maintains surface activity,
creating an environment where bone adheres well to the fixture,
and with an ultra-hydrophilic surface,
it reacts immediately with blood to promote bone formation.
As a result,
stable tissue can be established quickly,
and because treatment time can be shortened,
prosthetic work can be completed in about three months.

After placing two SOI fixtures,
the upper prosthesis was made as a four-unit bridge,
and the case was completed in just three months.

To reduce the patient’s cost burden,
only two implants were placed,
and instead of single crowns,
the prosthesis was made as a bridge.

The color was also selected carefully so that it would blend naturally with the surrounding teeth,
and the alignment was made even,
so it looks better than before as well.
Because this was a highly visible area,
a quick completion was the patient’s top priority,
so we used Osstem SOI,
and I think that was the right choice.
