What Matters Most for Safe Nose Surgery
After Removing Nose Filler

Hello.
I am Choi Joong-hyeok, a board-certified plastic surgeon at Apgujeong Seoul Plastic Surgery.
During consultations, I keep hearing similar questions over and over.
"Doctor, if I remove the nose filler, will it disappear completely?"
"Will the filler I had injected before cause problems during surgery?"
Nose filler is still a highly accessible procedure,
but many people do not have accurate information about filler removal.
Especially if you are considering rhinoplasty, filler removal is often not an option but a necessary step.

Why is nose filler removal necessary?

The purpose of nose filler is only one thing.
To make the bridge of the nose look higher!
But filler does not build bone;
it literally works by filling under the skin
with a jelly-like substance.
At first it may look natural, but as time passes it often spreads,
becomes thicker, and blurs the contours of the nose.
Especially if you are preparing for rhinoplasty,
existing filler interferes with an accurate structural analysis
and directly affects the surgical outcome.
That is why nose filler removal should be viewed as
the starting point of the surgical plan.
Dissolvable filler and non-dissolvable filler
have different levels of removal difficulty.

Nose filler is broadly divided into dissolvable filler (hyaluronic acid)
and non-dissolvable filler.
Dissolvable filler can be removed relatively safely when needed
with a medication called hyaluronidase.
This is the biggest reason we recommend dissolvable filler.

In contrast, non-dissolvable filler cannot be dissolved
with an injection, as the name suggests.
It must be surgically scraped out or removed,
but during that process normal tissue may also be damaged,
and in many cases complete removal is difficult.
The safest choice is not to use non-dissolvable filler from the start.

Why you should not dissolve nose filler on the day of surgery

Sometimes people ask, "Can’t you just dissolve it during the surgery?"
In theory, it may seem possible,
but in actual surgical settings it is not recommended.

The injection used to dissolve filler itself can cause swelling and an inflammatory response,
and in rare cases an allergic reaction may occur.
If severe swelling develops while dissolving the filler during surgery,
there may be situations where the already prepared operation has to be stopped.
That is why it is usually done about one week before surgery,
with enough time allowed.
For dissolving nose filler,
safety is more important than perfection.

The part patients worry about most is:
"Will it all disappear completely?"
To be honest, in the case of non-dissolvable filler,
100% perfect removal is not always possible.
If you forcefully remove filler that is deeply adhered just beneath the skin,
the skin may become thinner or the nose shape may actually become more unfavorable.
That is why an experienced specialist carefully judges the removal range and safety line.
It is not just a matter of technique, but a difference in clinical experience.

For those considering dissolving nose filler

People often choose nose filler because surgery feels scary
or the recovery period is burdensome.
But if time passes and you become unhappy with the shape of your nose,
or decide to undergo surgery,
nose filler dissolution is a step that must be gone through.
What matters is when, which filler, and how it is removed.
Rather than focusing only on price or convenience,
consulting with a plastic surgery specialist who is fully dedicated to the nose
and truly understands the nose is ultimately the safest path.
In actual consultations, there are many people who come in after worrying for a long time
without being able to clearly tell whether their current nose shape is due to filler
or due to their original structure.
So today, I briefly explained why nose filler dissolution is necessary,
especially the differences between dissolvable and non-dissolvable filler,
and the points to be careful about before surgery.
Nose filler dissolution is not simply a matter of getting one injection and being done;
because it is an important process that affects the subsequent result and safety,
the approach may differ depending on the condition even if the filler is the same.
