Treatment Methods for Refractory Pigmentation
When Cafe-au-lait Spots, Becker Nevus,
Agminated Lentiginosis, or Nevus of Ota Are Suspected
If brown spots remain on the face or body for a long time,
it is easy to assume, “They’ll fade someday.”
But if they gradually darken or spread, anxiety increases,
and it is hard to know exactly what the spot is
based on internet searches alone.
Also, when there are no special symptoms, it is easy to leave them alone.
Types known as refractory pigmentation may look similar
after appearance, but their mechanisms and patterns differ, making them hard to distinguish.
Brown spots that may look similar but are different types,
let’s take a look at refractory pigmentation!

- Cafe-au-lait spot
These often appear at birth or shortly after,
and have a light brown color, as if coffee were dropped into milk.
The shape varies from oval to irregular,
and they may grow larger or darker as the child grows.
They can appear not only on the face but on any part of the body,
and when there are many of them, it is important to differentiate them from other conditions.
Using sunscreen and reducing irritation are the basic measures.

- Becker nevus
This usually occurs around puberty and often spreads broadly on one side of the upper body,
including the shoulders, chest, and back.
It is also characteristically accompanied by increased hair growth, which can lead to cosmetic concerns. The color may darken over time,
but it does not come with any special symptoms.
Controlling body hair and reducing irritating friction
through lifestyle habits can help, so daily habits are important.

- Agminated lentiginosis
This is a form in which several brown spots appear clustered in a specific area,
and they may also be arranged along a skin segment or line.
It is usually found in childhood to adolescence,
and is characterized by a cluster of spots with uneven sizes and colors.
Strong scrubbing, heat stimulation, and sun exposure can
make the spots stand out more, so caution is needed.
If the area gradually expands, diagnosis may need to be considered.

- Nevus of Ota
This appears blue-gray or blue-brown and is clearly visible on one side of the face, especially around the eyes,
temples, or cheeks.
It is often identified in childhood or adolescence,
and in rare cases may also be seen around the eyes.
The color may appear more pronounced depending on the season or sun exposure,
so sun protection is important.
If these kinds of pigmentation are visible from an early age,
please keep checking for changes and be sure to use sunscreen.

- Daily care tips
Sun protection
Sunscreen, hats, parasols, and other
combined protection are necessary.
Reduce skin irritation
Avoid excessive cleansing, scrubbing, and heat irritation.
Inflammation caution
Do not touch troubled areas and moisturize sufficiently
so that discoloration does not remain for long.
Change tracking
Keeping a photo record of the time of onset, color changes, and lifestyle patterns
can help with future diagnosis.

Based on the type of pigmentation! Based on each person’s skin condition!
Customized care is needed for refractory pigmentation treatment
Although they may look similar on the surface, cafe-au-lait spots,
Becker nevus, agminated lentiginosis, and nevus of Ota
have different characteristics.
Refractory pigmentation varies greatly from person to person, so it is not easy to predict its course,
and even spots with the same name may look different depending on the area, skin type,
and lifestyle habits.
What matters is not to rush to a conclusion, but to watch the changes
and consistently maintain good care habits in daily life.

In addition, through dermatologic care such as laser treatment,
you may also consider treatment methods for this kind of refractory pigmentation.
However, we recommend gentle care from a dermatologist
who has a high level of understanding of refractory pigmentation.
